OBJECTIVE: The hearing level is a major point to take into account for managing a patient with an acoustic neuroma. The Gardner-Robertson classification is the most frequently reported as well as the AAO-HNS classification. It combines the use of pure tone audiometry and speech audiometry. The goal of this study is to determine the effect of the level of presentation for the speech material and the effect of the type of list selected for the determination of the speech discrimination score.
METHODS: The speech determination score has been determined for 94 patients with different type of list

Preference behaviors are often established during early life, but the underlying neural circuit mechanisms remain unknown. Adapting a unique nesting behavior assay, we confirmed a "critical period" for developing music preference in C57BL/6 mice. Early music exposure between postnatal days 15 and 24 reversed their innate bias for silent shelter, which typically could not be altered in adulthood. Instead, exposing adult mice treated acutely with valproic acid or carrying a targeted deletion of the Nogo receptor (NgR(-/-)) unmasked a strong plasticity of preference consistent with a reopening of

The present legislation establishes that workers' exposure to noise, which is the cause of chronic acoustic trauma, should be limited through the adoption of measures of technical and organizational nature. Organizational actions involve a plurality of aspects, not only technical in character, which are usually more difficult to apply and less rewarding in terms of acoustic benefits as compared to the technical ones; the latter should be primarily applied to the source, but are often adopted also at the noise propagation or reception level. Following a short summary of the normative framework,

OBJECTIVE: To establish the normal value of the vestibular evoked myogenic potential (VEMP), and to determine the characteristics of VEMP in patients with acoustic neuroma (AN) and to explore the significance of VEMP in diagnosis of AN.
METHOD: Click-evoked VEMP was recorded with surface electrodes attached on the sternocleidomastoid muscle. Latencies and amplitudes of specific waveform of VEMP were measured. The hearing normal subjects including 26 males and 20 females were chosen to establish the normal value of VEMP. VEMP was investigated in 14 patients with AN who underwent surgery during

An expectant, nontreatment strategy for acoustic neuromas implies an understanding of the natural biologic behavior of these tumors. This study describes the long-term follow-up of a group of unoperated acoustic neuroma patients. Patterns of tumor growth are discussed in light of clinical outcome. Patient selection and a follow-up protocol are recommended for those individuals in whom a nontreatment strategy is contemplated.

Birdsong is a learned behavior remarkable for its high degree of stereotypy. Nevertheless, adult birds display substantial rendition-by-rendition variation in the structure of individual song elements or "syllables." Previous work suggests that some of this variation is actively generated by the avian basal ganglia circuitry for purposes of motor exploration. However, it is unknown whether and how natural variations in premotor activity drive variations in syllable structure. Here, we recorded from the premotor nucleus robust nucleus of the arcopallium (RA) in Bengalese finches and measured wh

OBJECTIVE: To evaluate the complications and sequelae of acoustic neuroma surgery, according to tumour size.
PATIENTS AND METHOD: A retrospective analysis of 120 patients who underwent microsurgical resection of vestibular schwannomas between November 1994 and September 2006 was undertaken. Tumour size, extent of removal, preservation of facial and cochlear nerves, complications, and sequelae were considered. The degree of hearing preservation after surgery was determined by the Gardner-Robertson classification.
RESULTS: There were 39 small (<1.5 cm), 59 medium (1.5-3 cm), and 22 large tumou

Acoustic neurinoma is the most frequent benign tumour at the cerebellopontile angle. It causes compressive type lesions in adjacent cerebral structures. Its usual symptoms are sensorineural hearing loss and tinnitus with or without unilateral vertigo, which is why we have to know the wide variety of clinical signs it can present from its beginning, including facial palsy. We present the case of a vestibular schwannoma in a pregnant lady diagnosed by a peripheral facial palsy.

Skilled cardiologists perform cardiac auscultation, acquiring and interpreting heart sounds, by implicitly carrying out a sequence of steps. These include discarding clinically irrelevant beats, selectively tuning in to particular frequencies and aggregating information across time to make a diagnosis. In this paper, we formalize a series of analytical stages for processing heart sounds, propose algorithms to enable computers to approximate these steps, and investigate the effectiveness of each step in extracting relevant information from actual patient data. Through such reasoning, we provide

Coronary artery disease (CAD) occurs when the arteries to the heart (the coronary arteries) become blocked by deposition of plaque, depriving the heart of oxygen-bearing blood. This disease is arguably the most important fatal disease in industrialized countries, causing one-third to one-half of all deaths in persons between the ages of 35 and 64 in the United States. Despite the fact that early detection of CAD allows for successful and cost-effective treatment of the disease, only 20% of CAD cases are diagnosed prior to a heart attack. The development of a definitive, noninvasive test for de

The authors present their experience of more than 25 years, now in excess of 1200 patients, with cerebellopontine angle tumors. This article focuses on the management of planned subtotal resection of acoustic tumors in five subjects, and unexpected "residual" discovered by MRI scanning in 10 cases, which represents, to the best of the authors' knowledge, a residual rate of 1% of operated patients. The rate of residual tumor is as high as 19% in some series and, in part, depends on the surgical approach. For the purpose of this article, the authors did not include their cases of neurofibromatos

PURPOSE OF THE REVIEW: The purpose of this review is to examine the role of acoustic rhinometry in clinical practice. Although acoustic rhinometry was first described for clinical use in 1989, it is not in common use today. Should we be using it? Yes. I think we should be using it more often. This review provides an update of the new standard for interpretation and expanded clinical uses.
RECENT FINDINGS: The most significant advances in the past year in this area have been the publication of standards for its clinical use. In addition, the repertoire of clinical problems that can be analyzed

OBJECT: The authors sought to assess the safety and efficacy of stereotactic radiotherapy when using a linear accelerator equipped with a micromultileaf collimator for the treatment of patients with acoustic neuromas.
METHODS: Fifty patients harboring acoustic neuromas were treated with stereotactic radiotherapy between September 1997 and June 2003. Two patients were lost to follow-up review. Patient age ranged from 20 to 76 years (median 59 years), and none had neurofibromatosis. Forty-two patients had useful hearing prior to stereotactic radiotherapy. The fifth and seventh cranial nerve fun

OBJECTIVE: To study the clinical features and treatment of cystic acoustic neuroma.
METHODS: Twenty-two patients with cystic acoustic neuromas were diagnosed by CT and MRI preoperatively, and the tumors were resected by retrosigmoid approach, and verified by pathological studies.
RESULTS: Of the 22 patients, 18 had tumors totally resected (postoperative house Brackman facial nerve grading: grade II in 4 patients, III in 7, IV in 3, V in 2, VI in 2) and 4 had tumors subtotally resected.
CONCLUSION: Because of its specific clinical features and poor operative results, cystic acoustic neuroma
