Plasma prolactin in acromegaly before and after treatment.

Plasma human prolactin (hPRL) was measured in 73 untreated acromegalic patients and was found to be elevated in 32% of the total population. … within the normal range. Overall, these data suggest a closer relationship between hGH and hPRL in acromegaly than had been suspected, not only at the level of pituitary secretion but possibly also at the target cell.

Treatment of acromegaly, Cushing disease and Nelson syndrome.

Since 1957 we have treated more than 429 patients who had pituitary neoplasms, most of which were hormone-secreting tumors. Long-term follow-up in the large group of patients treated for acromegaly shows a median survival of better than 16 years, with improvement over time. The short-term follow-up results in patients with Cushing* disease, Nelson syndrome and chromophobe adenoma are very encouraging. To compare these excellent results with those following surgical procedures, a large study of patients followed for a long period after the operations is needed.

Conventional supervoltage irradiation is an effective treatment for acromegaly.

Forty-seven acromegalic patients were treated with conventional supervoltage irradiation (4000-5000 rads) and were followed-up at 2, 5, and … tage irradiation by 5-10 yr produces the same fall in plasma GH as all other forms of treatment for acromegaly, including transphenoidal microsurgery. The rate of fall in plasma GH after conventional irradiation is comparable to that of more aggressive forms of irradiation and is slower than that achieved by surgical methods but probably conventional irradia …

The prognosis in acromegaly treated by transanthro-sphenoidal operation.

The paper is a report of a follow-up study of 80 acromegalic patients treated by transanthro-sphenoidal removal of a pituitary adenoma. The average follow-up time was 3.7 years. Complete restitution or improvement of the visual field defects was noted in 77%. Total regression of the soft tissue swelling was observed in around 1/3 of the patients. Complete regression or improvement of the acromegalic activity was achieved in 94%. Total regression of the dominant symptoms was noted in 74%. The rate of tumour recurrence was around 5% and the total mortality rate in the disease during the follow-u

Gamma knife radiosurgery for acromegaly--long-term experience.

OBJECTIVE: The Leksell gamma knife (LGK) is one of the treatment options for pituitary adenomas. We report on our long-term experience treating acromegaly using LGK. DESIGN: Since 1993 we have followed 96 acromegaly patients through periods of from 12 to 120 months. The mean follow-up period was 53.7 +/- 26.8 months. Seventy-two patients were treated with neurosurgery prior to LGK; for 24 LGK was the primary treatment. Thirteen patients were irradiated twice, due to persistent activity of the adenoma or its residue. Pituitary functions were tested at 6-month intervals, post-irradiation. The t…

Acromegaly secondary to growth hormone releasing hormone secretion.

BACKGROUND: Acromegaly secondary to growth hormone releasing hormone (GHRH) secretion is exceptionally rare. AIM: To report a case of acromegaly diagnosed in 1984 and assumed to be pituitary in origin. Sixteen years later, the cause was found to be a GHRH secreting neuroendocrine pancreatic tumour. METHOD: A case report. CONCLUSION: Although ectopic GHRH production is very rare, endocrinologists should be aware of this possibility in acromegaly patients if a pituitary tumour was not detected using pituitary imaging.

Sphenoidal sinus mucocele after transsphenoidal surgery for acromegaly.

This report concerns one case of a sphenoid sinus mucocele occurring 17 years after transsphenoidal surgery for acromegaly. In 1979, a 51-year-old man was successfully operated by the transnasal transsphenoidal approach for a growth hormone (GH) adenoma 1 cm in diameter. In 1996, the patient was hospitalized for headaches and diplopia. He presented a loss of right visual acuity with paralysis of the right oculomotor nerve. The basal GH level was normal with a satisfactory decrease after oral glucose ingestion. Pituitary sellar radiography showed a disappearance of the posterior clinoid while m…

Kissing intrasellar carotid arteries in acromegaly: CT demonstration.

Acromegaly is a disease with protean manifestations that are well described in the literature. One finding infrequently encountered is aneurysmal dilatation of the internal carotid arteries and their branches. A case is presented, with CT documentation, in which the enlarged cavernous portions of the internal carotid arteries prolapsed into the sella turcica and enlarged it. Frontal bossing, frontal sinus enlargement, a dense cortical calvarium, prominent inion, and proptosis secondary to extraocular muscular enlargement were findings also present in this patient. The sella CT appearance has b…

New Horizons in Pharmacotherapy for Cushing's Disease and Acromegaly.

The recent development of new medical therapies has transformed Cushing's disease and acromegaly management, offering patients more individualized and effective treatment strategies beyond surgery and also addressing unmet clinical needs for persistent disease. In Cushing's disease, newer steroidogenesis inhibitors, pituitary-directed therapies, and glucocorticoid receptor antagonists coupled with increasing recognition of disease heterogeneity have highlighted the importance of individualized therapy selection based on corticotroph adenoma characteristics, severity of hypercortisolism, comorb…

Gender differences in the glycometabolic and cardiovascular features of acromegaly.

INTRODUCTION: Gender medicine focuses on disease differences between females and males regarding symptoms, treatment, prognosis, psychosocial effects and prevention. Acromegaly is a rare endocrine disorder caused by excessive growth hormone (GH) production, in the vast majority of cases due to the presence of a GH-secreting pituitary adenoma. Chronic GH elevation leads to increased insulin-like growth factor-1 (IGF-1), resulting in tissue overgrowth and a number of comorbidities. Among these, patients with acromegaly can experience various issues of the cardiovascular and metabolic system, inc…

Developments in Pharmacotherapy for Acromegaly: Current and Emerging Approaches.

Pituitary surgery is the primary therapy for most patients with acromegaly. Medical therapy has an important, albeit adjunctive role in the management of patients with persistent disease after surgery. However, primary medical therapy can be appropriate as an option in select patients. Medical therapies in current use for acromegaly are somatostatin receptor ligands (SRLs) (octreotide long-acting release [LAR], octreotide acetate, lanreotide depot, octreotide subcutaneous (SC) depot, pasireotide LAR, oral octreotide, paltusotine), dopamine agonists (cabergoline) and growth hormone receptor ant…

A Medically Managed Case of Acromegaly: A Case Report.

Endocrine glands are specialized cells responsible for producing and secreting hormones that serve many functions in various body tissues. T … gans. Excess growth hormone results in gigantism during childhood, while in adulthood, it is called acromegaly. In this report, we present a 39-year-old man with a large somatotroph macroadenoma, which was managed entirely with medical therapy, resulting in significant biochemical and anatomical improvement. This case highlights the potential role of combine …

Cardiomyopathy in Patients With Acromegaly - Not Truly a Concern Anymore?

Acromegaly is associated with increased mortality rates if not adequately treated. Cardiovascular and metabolic comorbidities are highly prevalent and have long been considered the main cause of death among patients with acromegaly. However, substantial advances in GH/IGF-I-lowering treatment, together with increased awareness and optimized management of other risk factors, have led to major improvements in mortality rates in the 2-3 past decades. Here, we review the effects of chronic excessive GH/IGF-I production and the successful treatment of this condition on relevant classical cardiovasc…

Recurrent acromegaly: a systematic review on therapeutic approaches.

BACKGROUND AND OBJECTIVE: Management of recurrent acromegaly is challenging for both neurosurgeons and endocrinologists. Several treatment options including repeat surgery, medical therapy, and radiation are offered for such patients. The efficacy of these modalities for the treatment of recurrence has not been studied previously in the literature. In this study, we aim to systematically review the existing cases of recurrence and come to a conclusion regarding the appropriate treatment in such cases. METHOD: A systematic review was performed through PubMed, Scopus, Web of Science, and Cochra…
