Accidental brimonidine poisoning following nasal instillation of eye drops in an infant.

We report the case of a young infant who presented with an acute episode of apnoea and hypotension requiring ventilation and stabilisation. The investigations did not show evidence of metabolic derangements, central nervous system infections or sepsis. At 15 hours of admission, the treating team obtained a history of accidental nasal instillation of brimonidine eye drops. The caregiver mistakenly administered glaucoma eye drops at home instead of the normal saline nasal drops and noticed the error only when the brimonidine drops were found in the medicine kit of the baby 15 hours after admissi

Recent Advances in Smart Polymers-Based Therapeutics in Ophthalmology.

Ophthalmic diseases represent a significant burden on global health, contributing to widespread visual impairment and blindness. The distinct anatomical and physiological barriers of the eye significantly restrict the use of conventional ocular delivery techniques. This has prompted research into cutting-edge drug delivery methods that optimize therapeutic benefits and reduce adverse effects. The use of polymer-based drug delivery systems is a promising technique that provides regulated drug release over long periods of time, enhances drug stability, and guarantees localized distribution to sp

Latest trends & strategies in ocular drug delivery.

Ocular drug delivery is one of the most challenging routes of administration, and this may be attributed to the complex interplay of ocular barriers and clearance mechanisms that restrict therapeutic payload residence. Most of the currently approved products that ameliorate ocular disease conditions are topical, i.e., delivering therapeutics to the outside anterior segment of the eye. This site of administration works well for certain conditions such as local infections but due to the presence of numerous ocular barriers, the permeation of therapeutics to the posterior segment of the eye is li

Biological topicals in ocular surface disorders.

Topical biological agents represent a significant advancement in the treatment of ocular surface diseases, offering a regenerative and therapeutic approach beyond conventional therapies. These agents are derived from serum (autologous or allogeneic), platelets, amniotic membrane, and pooled intravenous immunoglobulin. Their efficacy stems from their rich composition of growth factors, cytokines, and anti-inflammatory molecules that promote tissue healing, reduce inflammation, and enhance corneal regeneration. Autologous serum eye drops, closely mimicking natural tears, have been widely utilize

Breaking barriers in ocular drug delivery for Uveitis: Advanced drug delivery systems, challenges and future prospects.

Uveitis is inflammation of the uvea, the middle layer of the eye which comprises of iris, ciliary body and choroid. Complications associated with uveitis include chronic pain, vision impairment and even blindness if not treated adequately. Conventional treatments for uveitis include immunosuppressive medications such as corticosteroids and biologics, which present challenges of low bioavailability due to complex anatomical structure of eye, rapid drug elimination, enzymatic degradation and the blood-retinal barrier. Consequently, they require frequent administration and are often associated wi

Topical Chloramphenicol in Ophthalmology: Old is Gold.

PURPOSE: Topical chloramphenicol is one of the most ubiquitous antibiotics used in ophthalmology and oculoplastic surgery globally. It shows broad-spectrum activity against a variety of different pathogenic organisms, is well tolerated on the ocular surface and displays excellent topical pharmacokinetics. Chloramphenicol has been available for purchase over the counter in the United Kingdom since 2005. Despite this, the largest health economy in the world, The United States has had a de-facto moratorium on its use for the past 30 years. In this review, we aim to evaluate topical chloramphenico

Review on the Use of Difluprednate in Inflammatory Eye Disorders: The Topical Steroid That Goes the Distance.

Difluprednate (DFP) (difluoroprednisolone butyrate acetate, or DFBA) ophthalmic emulsion 0.05% (Durezol®) was the first potent corticosteroid to be approved for both postoperative pain and inflammation in 2008. In June 2012, it was approved for the treatment of endogenous anterior uveitis. It is a synthetic difluorinated prednisolone derivative that was originally developed in Japan as a dermatologic ointment. The glucocorticoid binding affinity of its active metabolite was demonstrated to be 56 times stronger than prednisolone. Experimental models showed that it reached the anterior and poste

Evaluation of the bacterial ocular surface microbiome in clinically normal horses before and after treatment with topical neomycin-polymyxin-bacitracin.

Next generation sequencing (NGS) studies have demonstrated a rich and diverse ocular surface-associated microbiota in people that was previously undetected by traditional culture-based methods. The ocular surface microbiome of horses has yet to be investigated using NGS techniques. This study aimed to determine the bacterial composition of the ocular surface microbiome in healthy horses, and to identify whether there are microbial community changes over time and following topical antibiotic use. One eye of 12 horses was treated 3 times daily for 1 week with neomycin-polymyxin-bacitracin ophtha

Age-related cataract and drug therapy: opportunities and challenges for topical antioxidant delivery to the lens.

OBJECTIVES: The search for anticataract drugs has been continuing for decades; some treatments no longer exist but antioxidants are still of much interest. KEY FINDINGS: The primary function of the human lens, along with the cornea, is to refract light so that it is correctly focused onto the retina for optimum image quality. With age, the human lens undergoes morphological, biochemical and physical changes leading to opacification. Age-related or senile cataract is one of the main causes of visual impairment in the elderly; given the lack of access to surgical treatment in many parts of the

Ocular silicone oil in the lateral cerebral ventricle.

Silicone oil used for endotamponade of retinal detachment may migrate into the subarachnoid space of the brain, including the cerebral ventricles, presumably by extension through silicone oil-filled spaces in the optic nerve. Silicone oil has characteristic appearances on CT scans and MRI, which can be utilized to distinguish it from more ominous entities. We describe a case of intraventricular silicone in a patient who presented with seizures.

Immunomodulating agents.

Inflammation involves a universally recognized, although incompletely understood, cascade of molecular events orchestrated by lymphokines and other innate biochemicals of immunity. Repeated or extended contact with immunogenic agents results in adaptive immunity involving antigen-induced events that stimulate down-stream immune cells and result in expansion of the inflammatory cascade. When immunogenic stimulation persists or autoregulatory immune mechanisms go awry, however, adaptive immunologic events can result in immune-mediated processes detrimental to systemic or organ-specific homeostas

Improving the therapeutic index of topical phenylephrine by reducing drop volume.

This study examined the effect of reducing eyedrop volume on the efficacy and systemic absorption of a given quantity of phenylephrine hydrochloride. Aqueous phenylephrine hydrochloride (2.5%) given in the commercially available drop volume of 32 microliters was compared with 10% aqueous phenylephrine given in an 8-microliters volume. Both preparations contained the same total amount of phenylephrine per drop. Ten subjects had both eyes dilated with each of the phenylephrine solutions according to a randomized, double-crossover sequence. The mean final pupillary diameter was nearly 1.0 mm larg

Polystyrene Sulfonate Resin as an Ophthalmic Carrier for Enhanced Bioavailability of Ligustrazine Phosphate Controlled Release System.

Topical ocular sustained-release drug delivery systems represent an effective strategy for the treatment of ocular diseases, for which a suitable carrier has yet to be sufficiently developed. Herein, an eye-compatible sodium polystyrene sulfonate resin (SPSR) was synthesized with a uniform particle size of about 3 μm. Ligustrazine phosphate (LP) was adsorbed to SPSR by cation exchange to form LP@SPSR. LP@SPSR suspension eye drops were further developed using the combination of Carbopol 934P and xanthan gum as suspending agents. The LP@SPSR suspension showed a sustained release in vitro, which

A more efficient ocular delivery system of triamcinolone acetonide as eye drop to the posterior segment of the eye.

As a leading cause of vision impairment of the retina, macular edema (ME) has one of the highest clinical demands for treatment. Current treatment of ME relies heavily on invasive techniques resulting in complications and poor patient compliance. To enhance the efficiency of triamcinolone acetonide as eye drop to the posterior segment of the eye, we developed and characterized a novel formulation, namely, triamcinolone acetonide chitosan-coated liposomes (TA-CHL), prepared by the calcium acetate gradient method with some modifications. TA-CHL provided the mean particle size of 135.46 ± 4.49 nm

Polymeric micelles for ocular drug delivery: From structural frameworks to recent preclinical studies.

Effective intraocular drug delivery poses a major challenge due to the presence of various elimination mechanisms and physiological barriers that result in low ocular bioavailability after topical application. Over the past decades, polymeric micelles have emerged as one of the most promising drug delivery platforms for the management of ocular diseases affecting the anterior (dry eye syndrome) and posterior (age-related macular degeneration, diabetic retinopathy and glaucoma) segments of the eye. Promising preclinical efficacy results from both in-vitro and in-vivo animal studies have led to

[Management of herpes zoster neurotrophic ulcer using a new matrix therapy agent (RGTA): A case report].

Neurotrophic keratopathy is a potential consequence of herpes simplex virus (HSV) or varicella zoster virus (VZV) infection. The treatment is based on artificial tears and the withdrawal of preserved eye drops or other types of epitheliotoxic topical medicines. Autologous serum or amniotic membrane transplantation may also be used in severe cases, but their cost and safety are still under debate. We report a case of a patient with a history of herpes zoster ophthalmicus, who developed a persistent epithelial ulcer after cataract surgery, with no improvement despite 3 weeks of artificial tears
