Quetiapine indication shift in the elderly: diagnosis and dosage in 208 psychogeriatric patients from 2000 to 2006.

RATIONALE: Quetiapine was approved in Germany as an atypical antipsychotic for treatment of schizophrenia in 2000, followed by the approval as an antipsychotic for treatment of bipolar mania in 2003. The approval of quetiapine for treatment of bipolar depression is expected. We hypothesized that the psychogeriatric prescription pattern for quetiapine shifts from the psychotic to the affective spectrum. METHODS: Retrospectively we screened discharge reports of all geriatric inpatients of the psychiatric department of the Ruhr-University of Bochum in the period from January 2001 until March 200

Smoking in schizophrenia -- all is not biological.

High rate of tobacco smoking reported in schizophrenia has been related to the effect of nicotine on the neurobiology of schizophrenia. Nicotine is said to alleviate psychotic symptoms in some patients. The relationship between smoking and psychiatric status may not be simply a biological one as several sociocultural and economic factors could influence smoking behaviour. In this study in India on 286 urban male outpatients with schizophrenia, only 38% were found to be current smokers. This was significantly more than in other psychiatric patients studied (major affective disorders and non-psy

High prevalence of childhood trauma in patients with schizophrenia spectrum and affective disorder.

OBJECTIVE: Childhood trauma (CT) is a major risk factor for various psychiatric disorders. We wanted to determine the prevalence of CT in a catchment area-based sample of schizophrenia spectrum and affective disorder (including bipolar disorder and depressive episodes with psychotic features) and to explore potential differences in types of CT between the diagnostic groups. METHOD: Three hundred five patients were recruited consecutively from psychiatric units at 3 major hospitals in Oslo, Norway, diagnosed with Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Dis

Schizophreniform disorder, delusional disorder and psychotic disorder not otherwise specified: clinical features, outcome and familial psychopathology.

The relationship between DSM-III-R schizophreniform disorder, delusional disorder (DD) and psychotic disorder not otherwise specified (PD-NOS) and schizophrenia and affective illness (AI) remains uncertain. We explore this question in the Roscommon Family Study by examining symptoms, outcome and patterns of psychopathology in relatives. Probands were selected from a population-based case registry in the west of Ireland with an ICD-9 diagnosis of schizophrenia or AI. Personal interviews were conducted with 88% of traceable, living probands, a mean of 16 years after onset, and 86% of traceable,

Effects of Violence Trajectories on Treatment Outcomes in Schizophrenia Spectrum Disorders.

INTRODUCTION: The relationship between violence and schizophrenia spectrum disorders (SSDs) is complex and poorly understood. Moreover, violence takes different trajectories, depending on its onset relative to that of the illness. However, the effect of such trajectories on the illness is not fully understood in nonforensic populations. METHODS: Two hundred twenty-three participants with SSD were recruited and divided into different violence subgroups using the Brown-Goodwin scale. Psychotic, affective, cognitive, and functional outcomes were measured. RESULTS: Subgroups only significantly d

The trip of a lifetime: hallucinogen persisting perceptual disorder.

OBJECTIVES: The differential diagnosis of psychotic symptoms is broad and extends beyond primary psychotic and affective disorders. We aim to illustrate that the chronology and phenomenological nature of hallucinatory symptoms may provide clues towards alternative diagnoses, such as hallucinogen persisting perceptual disorder (HPPD). We describe the resurgence of visual pseudo-hallucinations in a young woman in the context of previous substance-induced hallucinatory symptoms and a prior diagnosis of occipital lobe epilepsy. She presented a diagnostic challenge, saw several emergency and specia

The 'at-risk mental state' for psychosis in adolescents: clinical presentation, transition and remission.

Despite increased efforts over the last decade to prospectively identify individuals at ultra-high risk of developing a psychotic illness, limited attention has been specifically directed towards adolescent populations (<18 years). In order to evaluate how those under 18 fulfilling the operationalised criteria for an At-Risk Mental State (ARMS) present and fare over time, we conducted an observational study. Participants (N = 30) generally reported a high degree of functional disability and frequent and distressing perceptual disturbance, mainly in the form of auditory hallucinations. Seventy

Fluoxetine trial in borderline personality disorder.

Borderline personality disorder (BPD) is defined by many of the symptoms associated with serotonin dysregulation, including affective lability, suicidal behaviors, and impulsive aggression, providing an ideal clinical model for studying the treatment of these serious disorders. The recent development of selective serotonin re-uptake inhibitors such as fluoxetine makes it possible to study the role of serotonin in the etiology of affective and behavioral dyscontrol in BPD. In this preliminary medication trial, 5 patients with BPD were treated openly with 20 mg to 40 mg of fluoxetine for 8 weeks

Seasonality in Major Depressive Disorder: Effect of Sex and Age.

BACKGROUND: Aside from the concept of seasonal affective disorder, the evidence for a seasonal pattern (SP) of major depressive disorder (MDD) is controversial. Furthermore, the effect of sex and age is still unclear. METHODS: This is a nationwide, registry-based study assessing all inpatient admissions in mental health hospitals due to MDD episodes according to ICD-10 (moderate (F32/33.1), severe (F32/33.2) and severe with psychotic features (F32/33.3)) in Austria across 14 years. Calculations were based on deviations from expected monthly admissions. RESULTS: The sample comprised 231,824 h

[Psychogenic depressions in forensic psychiatric practice].

Analyzes the problems of diagnosis, classification, and assessment of psychogenic depression in forensic psychiatric practice. Clinical manifestations of such depressions are variable and numerous; the range of psychogenic depressions includes the borderline and psychotic forms each of which may be a separate entity or represent a stage in the development of a reactive depression. Distinguishing of these variants may be reasonable to assess the severity of a depressive disorder when deciding expert problems and for assessing the capacity of the subject exposed to expert evaluation to take adva

New antidepressive and antipsychotic drugs in juvenile neuronal ceroid lipofuscinoses--a pilot study.

Patients with juvenile neuronal ceroid lipofuscinosis (JNCL) often have severe psychiatric symptoms. These are common in their mid-teens and include such symptoms as anxiety and affective and psychotic disorders. The older antidepressants and antipsychotics do not seem to be effective and often cause many adverse effects. Therefore, we wanted to try the new psychotropic drugs in Finnish patients with JNCL. We also wanted to determine the profile of these drugs in this patient group. Fourteen Finnish patients with JNCL receiving psychotropic drug treatment with citalopram, risperidone, olanzapi

Psychiatric symptoms after therapy with new antiepileptic drugs: psychopathological and seizure related variables.

The purpose of this paper is to understand the association between antiepileptic drugs (AEDs), patient characteristics, changes in seizure pattern and emergent psychiatric disorder, i.e. psychosis or affective disorder. To this end we carried out a retrospective casenote study on 89 patients who developed psychiatric symptoms during treatment with topiramate, vigabatrin or tiagabine. The psychiatric problem was either an affective or a psychotic disorder (not including affective psychoses). It was discovered that 99% of the patients suffered from complex partial seizures with or without second

A Network Approach to Psychosis: Pathways Between Childhood Trauma and Psychotic Symptoms.

Childhood trauma (CT) has been identified as a potential risk factor for the onset of psychotic disorders. However, to date, there is limited consensus with respect to which symptoms may ensue after exposure to trauma in early life, and whether specific pathways may account for these associations. The aim of the present study was to use the novel network approach to investigate how different types of traumatic childhood experiences relate to specific symptoms of psychotic disorders and to identify pathways that may be involved in the relationship between CT and psychosis. We used data of patie

Frontolimbic affective bias and false narratives from brain disease.

Since the nineteenth century, clinicians and investigators have systematically evaluated the origin of delusions and psychotic thinking. One major clue to understanding the neurobiological underpinnings of delusions is the emergence of false narratives from brain disease. In addition to delusions themselves, there are a range of other false narratives not due to deliberate lying and resulting from neurological disorders, including provoked confabulations, fantastic confabulations, false memories, magical thinking, dream delusions, and "fantastic thinking". A comparison of their characteristics

Relationships between psychiatric symptoms, functioning and personal recovery in psychosis.

BACKGROUND: Recovery from psychosis is increasingly being viewed as a combination of symptomatic, functional, and personal recovery. Negative and depressive symptoms have been linked to community functioning, and negative affect has been linked to personal recovery. The current study examines differential associations of symptoms with functional and personal recovery, and the interaction of cognitive and emotional components of psychotic experiences in predicting recovery. METHODS: Baseline data from four studies of individuals with schizophrenia-spectrum disorders were amalgamated for the cu

Effects of Cognitive Behavioral Therapy for Worry Versus Befriending Therapy in Individuals with Persecutory Delusions: A Randomized Trial.

INTRODUCTION: Worry is common and distressing in psychotic disorders, contributing to the onset and persistence of persecutory delusions. A previously developed 8-week manualized cognitive behavioral therapy (CBT)-worry intervention has shown efficacy in treating persecutory delusions, compared with standard care. We aimed to test CBT-worry's impact on persecutory delusions at 8 weeks, as previously reported, delusions at 24 weeks, and secondary clinical outcomes, relative to an active comparison therapy. MATERIALS AND METHODS: A two-arm, assessor-blinded, randomized controlled trial was cond

Hallucinations during bipolar depressive episode treated with bupropion after alcohol withdrawal surgery: a case report.

Alcohol Use Disorder (AUD) is a prevalent condition often co-occurring with affective disorders. Bupropion, a medication used to treat affective disorders, acts by inhibiting dopamine (DA) and norepinephrine (NE) reuptake, potentially helping to alleviate emotional dysregulation. However, the risk of delirium in AUD patients at therapeutic doses is poorly understood, despite reports linking bupropion overdose to delirium. We report here a rare case of a 49-year-old Chinese male with AUD who developed mood disturbances and relapsed following alcohol detoxification surgery. He was diagnosed with

Seasonality of the episodes of recurrent affective psychoses. Possible prophylactic interventions.

This study is an attempt to ascertain whether evidence for seasonal variations of the episodes of recurrent affective psychoses can be found in patients in Greece. Due allowance has been made for the fact that different socio-cultural and above all climatological factors prevail in Greece as opposed to other countries, which might be relevant in the phenomenon of seasonality. The seasonal distribution of the psychotic episodes, either depressive or manic, has been examined for 533 patients with various forms of affective psychoses, admitted to and treated in the State Mental Hospital of Athens
