[A Skilled Typist with Typing Disorder Following Resection of a Left Frontal Lobe Tumor].

We reported a male who showed typing disorders after resection of a tumor in the left posterior superior and middle frontal gyri. He was a right-handed Japanese in his 50s and was good at touch typing as a system engineer. After the tumor resection, he presented typing errors and slightly impaired dexterity of his right fingers. The results of neuropsychological examinations indicated that his typing impairment was not due to aphasia or agraphia of kana letters (Japanese syllabogram). Typing errors were classified into adjacent key, non-adjacent key, omission, and insertion errors. Adjacent ke…

Some Thoughts About Thinking.

In this first-person case history, the writer Alberto Manguel chronicles the experience of losing his ability to write and speak during a stroke. He was reassured somewhat by his continued ability to read and to quote mentally from literature that he had memorized. Within hours after the stroke, he regained the ability to write. He remained unable to speak for a month. In this essay he ponders eloquently the relationship between thought and language, and describes how it felt not to be able to bridge the gap between thought and speech during his period of aphasia.

Surface dysgraphia in a regular orthography: apostrophe use by an Italian writer.

The dual-route model of writing assumes two basic procedures involved in writing: lexical and non-lexical. The lexical route is fundamental in opaque orthographies such as English; from its impairment surface dysgraphia arises. Evidence for the role of a lexical route in transparent languages such as Italian, which have a regular orthography, has been more limited. We report a case study of ES, an Italian patient suffering from degenerative brain disease, who presented with a selective disorder of writing. He showed the unusual phenomenon of inserting an apostrophe inappropriately in the spell

Developmental surface dyslexia and dysgraphia: an orthographic processing impairment.

This study presents a detailed investigation of a young man in his early twenties who has suffered from a severe spelling impairment since childhood, and currently has a spelling age of only 9 years and 2 months. In contrast with the developmental phonological dyslexics reported by Campbell and Butterworth (1985) and Funnell and Davison (1989), his performance on tests of phonological awareness is good. In addition, he can read and spell non-words competently and, unlike normal 9-year-old children, virtually all of his spelling errors are phonologically appropriate. Further analysis of these e

Levodopa-responsive depression associated with corticobasal degeneration: a case report.

A 60-year-old female was treated for depression with the antidepressant paroxetine for 13 years. The patient had experienced clumsiness and mild rigidity in the left hand, and had agraphia and mild subjective memory complaints for 3 years prior to admission in our hospital. She experienced exacerbated depression that included worsened depressive mood, lowered motivation, and suicidal ideation without precipitating stressful life events for 2 years prior to admission, and although she had continued taking the antidepressant, these symptoms were not ameliorated by increasing the dose of paroxeti…

Historical Perspectives on Ancient Greek Derived "a" Prefixed Nomenclature for Acquired Neurocognitive Impairment.

Distinct forms of acquired neurocognitive impairment are often described by "a" prefixed terms that derive from ancient Greek (and in one case Latin). Two modern English language neurological and neuropsychological reference books were searched to identify 17 such terms in contemporary usage: amnesia, akinesia, ataxia, aphasia, agraphia, anosmia, apraxia, athetosis, ageusia, achromatopsia, agnosia, alexia, amusia, anomia, anarthria, anosognosia, and acalculia. These were traced to their initial association with acquired neurocognitive impairment in German, English, and French language medical …

[Incomplete Gerstmann syndrome with a cerebral infarct in the left middle frontal gyrus].

A 65-year-old right-handed man noted a sudden onset of numbness and weakness of the right hand. On the initial visit to our hospital, he showed severe acalculia, and transient agraphia (so called incomplete Gerstmann syndrome) and transcortical sensory aphasia. Brain MRI revealed a fresh infarct in the left middle frontal gyrus. The paragraphia and aphasia improved within 14 days after onset, but the acalculia persisted even at seven months after onset In an 123I-IMP SPECT study, the cerebral blood flow (CBF) was found to be decreased in the infarction lesion and its adjacent wide area, the ip…

A specific deficit for numbers in a case of dense acalculia.

In this study we investigated the acalculic condition of a patient, C.G., with the classical signs of Gerstmann's Syndrome: finger agnosia; right-left disorientation; a profound agraphia (but with an equally profound alexia) and a remarkably dense acalculia. Using a series of number processing and number knowledge tasks, a selective impairment for numbers was demonstrated. Within the category of numbers C.G. showed a largely preserved ability to deal with numbers below 4, in all tasks and in all modalities, while she was totally unable to deal with numbers above 4. The consistency of responses…

Balamuthia mandrillaris Encephalitis Presenting as a Symptomatic Focal Hypodensity in an Immunocompromised Patient.

We present the case of a 61-year-old woman with a history of orthotopic heart transplant who was hospitalized with new-onset headache. Magnetic resonance imaging (MRI) of the brain revealed T2 hyperintense signal involving the left occipital lobe with leptomeningeal enhancement and mild vasogenic edema. Initial neurologic examination was normal; however, after 7 days she developed imbalance, visual disturbances, night sweats, bradyphrenia, alexia without agraphia, and right hemianopsia. Brain MRI showed enlargement of the left occipital mass and worsening edema. Stereotactic needle biopsy show…

Semantic dementia with lower motor neuron disease showing FTLD-TDP type 3 pathology (sensu Mackenzie).

We describe a case of frontotemporal lobar degeneration with semantic dementia and lower motor neuron disease. A 63-year-old man presented with the full clinical picture of semantic dementia, including semantic anomia, surface alexia, lexical agraphia, associative agnosia, prosopagnosia and phonagnosia. Flaccid dysarthria, bulbar dysphagia and fasciculations developed 7 years after onset, followed by death within a year. The neuropathological examination showed heavy neuronal loss in the anterior temporal lobe cortex, dorsal vagal and hypoglossal nuclei and anterior horns of the spinal cord. U…

Dysgraphia Phenotypes in Native Chinese Speakers With Primary Progressive Aphasia.

BACKGROUND AND OBJECTIVES: Most primary progressive aphasia (PPA) literature is based on English language users. Linguistic features that vary from English, such as logographic writing systems, are underinvestigated. The current study characterized the dysgraphia phenotypes of patients with PPA who write in Chinese and investigated their diagnostic utility in classifying PPA variants. METHODS: This study recruited 40 participants with PPA and 20 cognitively normal participants from San Francisco, Hong Kong, and Taiwan. We measured dictation accuracy using the Chinese Language Assessment for P

Prospective analysis of gross and fine electrode position and motor manifestations after STN-DBS and their correlation with electrode position.

BACKGROUND: Deep brain stimulation (DBS) has been proven to be a safe, reversible, cost-effective procedure for treatment of Parkinson's disease. Final electrode position remains a significant factor determining the outcomes of subthalamic nucleus DBS (STN-DBS). This study aims to analyze the final lead position in three-dimensional plane and its effect on gross and fine motor outcomes in cases of advanced Parkinson's disease operated for STN-DBS. METHODS: Patients who underwent bilateral STN-DBS were prospectively followed for improvement in gross motor outcomes at 6 months. Improvement in d

Lexical decision performance in developmental surface dysgraphia: Evidence for a unitary orthographic system that is used in both reading and spelling.

The relationship between spelling, written word recognition, and picture naming is investigated in a study of seven bilingual adults who have developmental surface dysgraphia in both Greek (their first language) and English (their second language). Four of the cases also performed poorly at orthographic lexical decision in both languages. This finding is consistent with similar results in Italian that have been taken as evidence of a developmental impairment to a single orthographic system that is used for both reading and spelling. The remaining three participants performed well at orthograph

Relationships of Attention and Executive Functions to Oral Language, Reading, and Writing Skills and Systems in Middle Childhood and Early Adolescence.

Relationships between attention/executive functions and language learning were investigated in students in Grades 4 to 9 ( N = 88) with and without specific learning disabilities (SLDs) in multiword syntax in oral and written language (OWL LD), word reading and spelling (dyslexia), and subword letter writing (dysgraphia). Prior attention-deficit/hyperactivity disorder (ADHD) diagnosis was correlated only with impaired handwriting. Parental ratings of inattention, but not hyperactivity, correlated with measures of written language but not oral language. Sustaining switching attention correlated

The making of a syndrome: Gerstmann's patients before Gerstmann syndrome.

When Gerstmann published the case report which later became known as the first case of Gerstmann syndrome, he did not claim the discovery of a new syndrome. It was only a few years later, after reporting on another two similar cases, that he isolated the famous tetrad of symptoms (finger agnosia, right-left disorientation, agraphia and acalculia) as a meaningful cluster with both localising and functional value. In this article, we provide the translation of key-excerpts of the second of Gerstmann's reports (Gerstmann, 1927) and a synoptic description of the symptoms as reported in the three o…

Left Parietal Tumors Presenting with Smartphone Icon Visual Agnosia: Two Cases of a Modern Presentation of Gerstmann Syndrome.

BACKGROUND: Gerstmanns syndrome-a clinical constellation of left-right confusion, finger agnosia, agraphia, and acalculia-is frequently attributed to pathology in the dominant inferior parietal lobe or temporo-occipital region. However, these unique clinical findings are often accompanied by more subtle signs, including aphasias, neglect, and agnosias. Associative visual agnosia, in which a patient is able to accurately perceive and describe but not recognize an object or symbol, is a well-documented but infrequently observed clinical entity. CASE DESCRIPTION: Here we detail 2 unique cases of…
