Sulfasalazine is used as a therapy for various autoimmune conditions, including psoriasis; its effectiveness is presumed to be the result of its immunomodulatory effects. We have treated patients with severe alopecia areata with sulfasalazine as part of our dermatology practice and have noticed cosmetically acceptable regrowth in 23% of patients in whom a response could be determined. In view of its good safety profile, sulfasalazine may be considered for systemic treatment of severe alopecia areata.

Alopecia areata is a common disorder with a genetic predisposition where interaction with environmental factors leads to episodes of terminal hair loss. In this review article, we examine the evidence for a genetic basis to this disorder and discuss the prospects for future research into genetic susceptibility areas and the problems that are likely to be encountered in such research.

There has been a resurgence of syphilis in the past decade. Uncommonly, diffuse hair loss, termed essential alopecia, is the only sign of syphilitic infection. We describe two patients with syphilis in whom the first sign of disease was alopecia and discuss the clinical and histopathologic findings of essential syphilitic alopecia.

A 23-year-old woman was noted to have a hypoesthetic patch over the right mandible. A biopsy specimen revealed alopecia mucinosa. Anesthesia in the involved area has been reported rarely in the literature.

Two patients are described who had a lesion in the lateral cutaneous nerves of the thigh, both of which were verified by anamnestic, clinical and electrophysiological data. Both showed alopecia in the region supplied by the nerve. A biopsy performed on one patient showed atrophy of the epidermis and hair follicles. It is pointed out that this kind of trophic disturbance, which is very seldom described in literature on meralgia paraesthetica, may possibly be found more often in long-standing cases of this disease.

A 26-year-old man presented with an unusual skin lesion consisting of a hairless area on the left leg studded with reddish-brown papules, together with follicular hyperkeratosis and circumscribed anhidrosis. Skin biopsies showed dense lymphocytic infiltrates in the dermis and around hyperplastic sweat ducts and glands. Similar clinical and histological features have been described in two previous case reports. We propose the term syringolymphoid hyperplasia with alopecia for this unusual skin disease.

In order to create awareness of a potential adverse effect of extraoral orthodontic appliances, we report the development of alopecia areata in two patients. Both patients experienced the onset of alopecia areata directly under sites of pressure from thier "headgear." In one patient the hair loss began 2 weeks after she began using her appliance. The other girl noticed patchy alopecia 2 to 3 weeks following an increase in the pressure and duration of use of an extraoral appliance. Ten patients with alopecia areata (plaque type and totalis type) were treated with oral photochemotherapy. Successful results were obtained in 7 cases. No side effects were noted. Hair growth was related to the energy delivered and was not dependent on the duration of the disease. No relapses occurred after discontinuation of the treatment. The hypothesis concerning the mechanisms of hair regrowth are discussed.
