Topological data analysis for predicting disease outbreaks in humanitarian settings: A machine learning approach.

BACKGROUND: Humanitarian settings are highly vulnerable to infectious disease outbreaks because displacement, crowding, disruption of health services, insecurity, and inadequate water and sanitation often interact in ways that are difficult to capture with conventional prediction models. There is a need for forecasting approaches that can integrate heterogeneous data sources and better represent complex system structure. METHODS: We developed and evaluated a machine-learning framework incorporating topological data analysis to predict cholera and measles surge events (binary indicators per LG

Alternate Choice Organ Counselling in Altruistic Non-Directed Solid Organ Donation: An Ethical Analysis.

INTRODUCTION: Currently, recruitment of non-directed altruistic (NDA) kidney and liver lobe donors in the UK regards these individuals as potential NDA donors of the particular organ type they initially express an interest in donating. Conceptualising these individuals instead as potential NDA donors of either a kidney or a liver lobe would require them to be counselled on both kidney donation and liver lobe donation. This can be referred to as 'alternate choice organ counselling'. METHODS: This paper conducts an ethical analysis of alternate choice organ counselling using the ethical framewo

The Quadratic Association Between Resting Respiratory Sinus Arrhythmia and Adolescents' Prosociality: The Moderating Role of Parental Marital Conflict.

Although both vagal regulation and parental marital conflict have been demonstrated to influence individuals' prosociality, research on their interactive influences is almost silent, especially in adolescents. The present study examined the potential quadratic association between resting respiratory sinus arrhythmia (RSA) and adolescents' prosociality and the moderating role of parental marital conflict in this association. Three hundred and twenty-seven junior high school students (Mage = 13.24 years, SD = 0.47, 45.3% female) participated in this study. The Agreeableness Subscale from the NEO

The altruistic elderly, a valuable but unrecognised kidney donor group. A case report of an 85-year-old unspecified kidney donor.

BACKGROUND: Kidney transplantation is the definitive treatment for end stage renal disease (ESRD), offering improved quality of life and survival benefit over remaining on dialysis. There is, however, a prevailing significant mismatch between patients awaiting transplantation and available donor kidneys. Over time, initial stringent donor criteria have broadened and organs from extended criteria donors (ECDs) and older donors are now being accepted for transplantation. The spectrum of living donors has also undergone a change from close family members to an increasingly non-related, non-direct

Violence against children and natural disasters: A systematic review and meta-analysis of quantitative evidence.

OBJECTIVES: Reviews of violence against children in disaster settings focus on armed conflict. Little is understood about natural disasters which has implications in planning humanitarian response. We examined the magnitude and direction of the association between exposure to natural disasters and physical, emotional, and sexual violence against children, and assessed the quality of the evidence. METHODS: We searched 15 health and social science databases from first record until May 16, 2018. Publications describing all types of quantitative study design were eligible for inclusion. We presen

The Development of a Humanitarian Health Ethics Analysis Tool.

UNLABELLED: Introduction Health care workers (HCWs) who participate in humanitarian aid work experience a range of ethical challenges in providing care and assistance to communities affected by war, disaster, or extreme poverty. Although there is increasing discussion of ethics in humanitarian health care practice and policy, there are very few resources available for humanitarian workers seeking ethical guidance in the field. To address this knowledge gap, a Humanitarian Health Ethics Analysis Tool (HHEAT) was developed and tested as an action-oriented resource to support humanitarian workers

Priority in organ allocation to previously registered donors: public perceptions of the fairness and effectiveness of priority systems.

A priority system is one in which previously registered donors receive a preference in the allocation of organs for transplant ahead of those who have not registered. Supporters justify these systems on the basis that they are fair and will encourage donor registration. This article reviews existing studies of public reactions to priority systems, as well as studies of the extent to which the moral principle of reciprocity affects decision making in organ donation. The role of reciprocity in the public discourse surrounding the enactment of priority systems in Singapore and Israel is described

The search for organs: halachic perspectives on altruistic giving and the selling of organs.

Altruistic donation of organs from living donors is widely accepted as a virtue and even encouraged as a duty. Selling organs, on the other hand, is highly controversial and banned in most countries. What is the Jewish legal (halachic) position on these issues? In this review it is explained that altruistic donation is praiseworthy but in no way obligatory. Selling organs is a subject of rabbinic dispute among contemporary authorities.

Living donors' experiences 1 wk after donating a kidney.

In Norway living kidney donors account for approximately 40% of all renal transplants. There is a shortage of information about how living kidney donors experience the donation process during the initial recovery from surgery. The aim of this study was to explore physical and psychosocial issues related to the experiences of living kidney donors 1 wk after open donor nephrectomy. A total of 12 living kidney donors participated in the study. Data were collected by individual in-depth interviews and analysed using empirical phenomenological method. Being a living kidney donor is a complex experi

Motivations to donate blood: demographic comparisons.

BACKGROUND: Understanding blood donor motivations is crucial to improving effectiveness of donor recruitment and retention programs. STUDY DESIGN AND METHODS: Data from a 1998 survey of 92,581 U.S. blood donors were used to evaluate factors influencing the decision to donate in various demographic groups. Data were weighted to adjust for response and sample design. RESULTS: Of 52,650 respondents, 45,588 gave whole-blood (WB) donations. Among all demographic groups, the major reasons to donate were altruism (75-87%) and awareness of the need for blood (34-43%). Except for first-time donors an…

Adjusting for race in metrics of organ procurement organization performance.

The Scientific Registry of Transplant Recipients has previously reported the effects of adjusting for demographic variables, including race, in the Centers for Medicare & Medicaid Services (CMS) organ procurement organization (OPO) performance metrics: donation rate and transplant rate. CMS chose not to adjust for most demographic variables other than age (for the transplant rate), arguing that there is no biological reason that these variables would affect the organ donation/utilization decision. However, organ donation is a process based on altruism and trust, not a simple biological phenome…

Gi4SaveLife: a community-led model for sustainable voluntary blood donation in rural Sierra Leone.

Motivated by identity: A social identity analysis of why people become foster carers.

Understanding why individuals are motivated to take on new identities, particularly why people choose to become foster carers, is critical to addressing persistent shortages in foster care systems. Despite policy consensus that foster care is the preferred alternative when children cannot remain with birth families, shortages persist across many countries. Research has traditionally emphasised individualistic motivations, such as altruism, neglecting broader social and identity-based factors. This study applies a Social Identity Approach, drawing on the Social Identity Model of Identity Change…

Cardiac vagal modulation predicts decision-making in sacrificial and everyday moral dilemmas.

Cardiac vagal modulation indicates the self-regulatory activity of the autonomic nervous system and can thus be used to assess an individual … lso a faster cardiac vagal recovery compared to high utilitarian women. Findings suggest differences in the acquisition of self-regulatory capacities between decision-makers in both types of moral dilemmas, which can be measured by HRV.
