Anesthestic agents and status epilepticus.

There is little evidence to guide the choice of intravenous anesthetic agent to treat refractory status epilepticus but midazolam, propofol, and barbiturates are widely used. It is impractical to use inhalational anesthetic agents in most circumstances and there is little experience with non-GABA-ergic agents such as ketamine. A more aggressive treatment approach, aiming for EEG suppression, is most likely to result in sustained cessation of seizure activity but this is associated with increased treatment-related complications. Side effects of treatment, including hypotension, gastric paresis,

[Bronchodilator aerosol propellant interferes with an photoacoustic spectrophotometer respiratory gas analyzer].

A patient with bronchial asthma was scheduled for an operation under nitrous oxide-isoflurane anesthesia. We monitored isoflurane concentrations continuously using an anesthetic gas analyzer (BK 1304). Upon puffing procaterol hydrochloride aerosol for 4 times, the analyzer showed a rapid increase in end-tidal isoflurane concentration. The BK 1304 uses infrared photoacoustic spectrophotometry and it is susceptible to interferences caused by Freon propellants in bronchodilator aerosols. We should take care in monitoring inhalational anesthetics when using aerosols containing Freon propellants.

Fatal postoperative malignant hyperthermia following rhinoplasty in a patient with one variant of the ryanodine receptor 1 (RYR1) gene.

Malignant hyperthermia (MH) is characterized by a rapid rise in body temperature after using inhalational anesthetics and depolarizing muscle relaxants. A 19-year-old female had a rapidly developing fever up to 43.0 °C, after rhinoplasty surgery. Inhalational anesthetics and depolarizing muscle relaxants were used in general anesthesia. It was suspected that the patient died of MH. The medico-legal autopsy findings showed classical MH histopathological changes in the skeletal muscles, cardiac muscles, as well as lungs. Additionally, postmortem blood biochemical results indicated rhabdomyolysis

Clinical Assessment of an Ipsilateral Cervical Spinal Nerve Block for Prosthetic Laryngoplasty in Anesthetized Horses.

The nociceptive blockade of locoregional anesthesia prior to surgical stimulation can decrease anesthetic agent requirement and thereby potential dose-dependent side effects. The use of an ipsilateral second and third cervical spinal nerve locoregional anesthetic block for prosthetic laryngoplasty in the anesthetized horses has yet to be described. Anesthetic records of 20 horses receiving locoregional anesthesia prior to laryngoplasty were reviewed and compared to 20 horses of a similar patient cohort not receiving locoregional anesthesia. Non-blocked horses were 11 times more likely to requi

Enhanced recovery after laparoscopic or robotic-assisted ureteral reconstruction: identification of modifiable factors in a prospective study.

PURPOSE: To evaluate the bowel function recovery following ureteral reconstruction and identify the associated factors. METHODS: We conducted a prospective study enrolling 98 patients undergoing transperitoneal laparoscopic or robotic-assisted ureteral reconstruction. The time to resumption of gas evacuation and defecation were used to evaluate the bowel function recovery. Student's t-test, Chi-squared test, Pearson's correlation, and linear regression were performed to identify independent factors. Nomograms were developed to predict early resumption of gas evacuation and defecation. RESULT

Role of pterygopalatine ganglion in regulating isoflurane-induced cerebral hyper-perfusion.

Cerebral perfusion is functionally regulated by neural mechanisms in addition to the systemic hemodynamic variation, vascular reactivity and cerebral metabolism. Although anesthesia is generally esteemed to suppress the overall brain neural activity and metabolism, a few inhalation anesthetics, such as isoflurane, can increase cerebral perfusion, thus heightening the risks of higher intracranial pressure, bleeding, and brain edema during surgery. With the aid of laser speckle contrast imaging, we observed a transient yet limited effect of cerebral perfusion enhancement in mice from awake to an

Paediatric anaesthesia: pharmacological considerations.

The young infant differs from the adult in his quantitative responses to many anaesthetic drugs and adjuncts. In the neonate, the larger extracellular fluid volume and blood volume, the smaller muscle mass and fat stores, and presumable greater blood flow to the central organs, not only influence the distribution of drugs to their active site but also secondary redistribution. The neonatal hepatic anzyme systems responsible for the metabolism of drugs are incompletely developed or absent. Glomerular filtration, important for drug excretion, is inefficient by adult standards. The neonate has in

Conscious sedation and anesthesia in the cardiac electrophysiology laboratory.

Adequate anesthesia is important for performing EP procedures such as catheter ablation and implantation of cardiac devices. A wide range of anesthesia options are available, and all can be safely administered in EP laboratory with proper monitoring, drug selection, airway management equipment, and trained personnel. In addition to knowing the patient's clinical status, a thorough understanding of the clinical pharmacology of each anesthetic agent is essential in order to select appropriate drug among a variety of available anesthetics, to monitor for adverse effects, and to ensure adequate se

Effects of sodium pentobarbital, ketamine, halothane, and chloralose on brainstem auditory evoked responses.

A parametric study was performed in cats comparing the effects on the brainstem auditory evoked response of four anesthetic agents used either in the operating room or the laboratory: sodium pentobarbital, ketamine, halothane, and chloralose. Each anesthetic agent was administered to each of four cats in varying sequence. Click-evoked brainstem auditory evoked responses (BAER) were compared for differences in wave form configuration, latencies, and amplitudes of the component peaks. Five distinct peaks (I to V) could be identified in the brainstem auditory evoked responses of every animal usin

Propofol based total intravenous anesthesia versus sevoflurane based inhalation anesthesia: The postoperative characteristics in oral and maxillofacial surgery.

OBJECTIVE: Total intravenous anesthesia and inhalation/volatile anesthesia are the main general anesthesia procedures used in all surgical applications. The aim of this study was to compare sevoflurane anesthesia and total intravenous anesthesia with propofol in terms of postoperative complications, especially after oral and maxillofacial surgeries. MATERIAL AND METHODS: Each patient was taken to the recovery room following extubation, and the pulse rate, non-invasive blood pressure (NIBP) and oxygen saturation were monitored. Presence of hypoxia, tachycardia, bradycardia, hypertension and hy

Development of a hydrogen cyanide inhalation exposure system and determination of the inhaled median lethal dose in the swine model.

OBJECTIVE: Cyanide is a highly toxic chemical, and acute exposure depletes cells and tissue of oxygen, depressing the respiratory, cardiovascular and neurological systems and potentially leading to death. Cyanide has been used as a weapon since ancient Rome and continues to pose a potential threat today. A well-characterized animal model is necessary for the development of novel methods of rapid detection and treatment. This manuscript describes the development of an inhalation exposure system designed to evaluate the lethality of acute cyanide inhalation in the porcine model. MATERIALS AND M

Adenosine-induced bronchoconstriction and its inhibition by nedocromil sodium.

The inhalation of adenosine or adenosine-5'-monophosphate (AMP) causes a reproducible and transient bronchoconstriction in both atopic and nonatopic asthmatic subjects that is not followed by a late-phase response. The response may be prevented by prophylactic treatment with histamine H1-receptor antagonist, local anesthetics, and atropine, which suggests that both mast cell mediator release and neuronal reflexes are involved. Adenosine- and AMP-induced bronchoconstriction are both blocked by cromolyn sodium and nedocromil sodium, with the PD20FEV1 for the latter approximately four times lower

Intravenous Propofol Versus Volatile Anesthetics For Stroke Endovascular Thrombectomy.

BACKGROUND: The choice of anesthetic technique for ischemic stroke patients undergoing endovascular thrombectomy is controversial. Intravenous propofol and volatile inhalational general anesthetic agents have differing effects on cerebral hemodynamics, which may affect ischemic brain tissue and clinical outcome. We compared outcomes in patients undergoing endovascular thrombectomy with general anesthesia who were treated with propofol or volatile agents. METHODS: Consecutive endovascular thrombectomy patients treated using general anesthesia were identified from our prospective database. Base

Xenon anesthesia: shedding light on brain dynamics and clinical niches.

PURPOSE OF REVIEW: Xenon is reemerging as a clinically attractive inhalational anesthetic because it combines rapid kinetics and cardiovascular stability with mechanism features that differ from GABAergic agents. Simultaneously, new membrane-biophysics and systems-neuroscience data are reshaping how models in anesthetic state transitions worked, which expands the future of how anesthetic agents are used. RECENT FINDINGS: Contemporary work strengthens the view that xenon acts primarily via noncompetitive NMDA receptor inhibition, with additional effects on excitability-related ion channels. Wh

[An anesthetic case of a patient with multiple sclerosis and supranuclear palsy].

A 60-year-old female with multiple sclerosis (MS) and supranuclear palsy (PSP) was scheduled for right eye iridotomy, left eye phaco emulcification aspiration and insertion of the intraocular lens. Her medical conditions included prolonged immobility, spastic contracture, and a history of convulsion. She was administered with L-dopa, tizanidine, bacrofen, and dantrorane. Anesthesia was induced with propofol 50 mg and fentanyl 25 microg intravenously, and inhalation of oxygen and 1% sevoflurane. Tracheal intubation was performed without neuromuscular blocking agents. Anesthesia was maintained w

NARCOBIT: a newly developed inhalational anesthesia system for mice.

NARCOBIT is the first anesthetic system for mice and rats to incorporate a ventilator. Therefore, it is expected to improve the reliability of mice and rat experiments by accurately controlling and maintaining the depth of anesthesia. In this study, we used NARCOBIT for inducing inhalational anesthesia in mice and evaluated the changes in their hemodynamic parameters. ICR mice were anesthetized with 5% isoflurane and room air, followed by endotracheal intubation. Subsequently, they were mechanically ventilated, and anesthesia was maintained by 2% isoflurane for a 60-min period (maintenance sta

Steady-state minute volume determination by body-only plethysmography in juvenile rhesus monkeys.

When one is using nonhuman primates for studying the inhalation of infective or toxic agents, a respiratory minute volume (MV) range of +/- 50 ml is desirable to ensure the accurate delivery of calculated doses of the aerosolized agent. When one is working with highly infective or toxic agents, it is desirable to anesthetize the animals and to separate the plethysmograph, used to measure MV, from the aerosol chamber, used to administer agents, in order to minimize decontamination procedures and to maximize safety. In our laboratory the sequential completion of these procedures requires at leas

Influence of halothane, enflurane, and isoflurane on motor evoked potentials.

The influence of the inhalational anesthetics halothane, enflurane, and isoflurane on motor evoked potentials was studied in a total of 10 rabbits. Motor evoked potentials were recorded from the extremity muscles as well as from the epidural space of the spinal cord and cauda equina in response to electrical stimulation of the motor cortex at baseline conditions and equianesthetic concentrations (0.25 to 1.5 minimal alveolar concentration). Our results show a dose-dependent suppression of the electromyographic responses, which was similar with all anesthetics. Beyond 0.5 minimal alveolar conce

Glycine and gamma-aminobutyric acid(A) receptor function is enhanced by inhaled drugs of abuse.

Inhalable solvents possess significant abuse liability and produce many of the neurobehavioral effects typically associated with central nervous system-depressant agents, including motor incoordination, anxiolysis, and the elicitation of signs of physical dependence on withdrawal. We tested the hypothesis that the commonly abused solvents toluene, 1,1,1-trichloroethane (TCE), and trichloroethylene (TCY) affect ligand-gated ion channel activity, as do other classes of central nervous system-depressive agents. TCE and toluene, like ethanol, reversibly enhanced gamma-aminobutyric acid (GABA)(A) r
