Does using the bispectral index (BIS) during craniotomy affect the quality of recovery?

One of the aims of neuroanesthesia is to provide early postoperative recovery and neurologic examination in patients undergoing supratentorial surgery. Our aim was to investigate the role of using the bispectral index (BIS) in recovery from anesthesia and altering drug administration in patients undergoing craniotomy. Fifty American Society of Anesthesiologists' (ASA) physical status I-II patients undergoing craniotomy were included in the study. The patients were randomly divided into two groups, and all patients received standard induction drugs, and 0.8%-1.5% sevoflurane was used for mainte

[Conservative management of the airway following iatrogenic tracheal rupture].

The trachea of a 39-year-old man ruptured following intubation for fiberoptic bronchoscopy; the complication became evident during surgery as extensive subcutaneous emphysema developed. The emphysema resolved with conservative treatment. Tracheobronchial lesions are generally caused by direct trauma to the chest. Tracheal rupture due to intubation maneuvers is one of the complications anesthetists fear most. Although infrequent, such lesions are potentially fatal. Signs and symptoms can vary widely, from self-limiting asymptomatic subcutaneous emphysema immediately following surgery to severe …

The impact of an anesthesia residency teaching service on anesthesia-controlled time and postsurgical patient outcomes: a retrospective observational study on 15,084 surgical cases.

BACKGROUND: Limited data exists regarding the impact of anesthesia residents on operating room efficiency and patient safety outcomes. This investigation hypothesized that supervised anesthesiology residents do not increase anesthesia-controlled or prolonged extubation times compared to supervised certified registered nurse anesthetists (CRNA)/certified anesthesiologist assistants (CAA) or anesthesiologists working independently. Secondary objectives included differences in critical outcomes such as intraoperative hypotension, cardiac and pulmonary complications, acute kidney injury, and morta…

Prospective, randomized comparison of two supplemental oxygen methods during gastroscopy with propofol mono-sedation in obese patients.

BACKGROUND: Hypoxemia is a common complication in obese patients during gastroscopy with sedation. The Wei nasal jet tube (WNJT) is a new sp … icant difference in satisfaction among patients, the WNJT did result in improved satisfaction among anesthetists and physicians. CONCLUSION: During gastroscopy with propofol mono-sedation in obese patients, the WNJT, when compared with a nasal cannula for supplemental oxygen, can significantly reduce the occurrence of hypoxemia and improve both arterial oxyge …

Insights into the Use of Point-of-Care Ultrasound for Diagnosing Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a sleeping disorder caused by complete or partial disturbance of breathing during the night. Existing screening methods include questionnaire-based evaluations which are time-consuming, vary in specificity, and are not globally adopted. Point-of-care ultrasound (PoCUS), on the other hand, is a painless, inexpensive, portable, and useful tool that has already been introduced for the evaluation of upper airways by anesthetists. PoCUS could also serve as a potential screening tool for the diagnosis of OSA by measuring different airway parameters, including retropa…

[Bronchoscopy in intensive care units].

The authors report their 1535 urgent bronchoscopic examinations performed between 1998-2002. The examinations/interventions were requested m … oscopy performed on emergency wards and intensive care units. They conclude: it's necessary for the anesthetists to get a basic level ability to work with bronchofiberscope and for pulmonologist-bronchologists to be experienced to answer the questions of other specialists.

Concurrence of malignant hyperthermia and congenital abnormalities.

Two children about to undergo corrective surgery were required to be investigated for malignant hyperthermia (MH). These investigations arose out of concern by the anesthetist who had obtained a history of unexplained pyrexial reactions to anaesthetic in other members of the family. Because over the years we have encountered several children with multiple congenital abnormalities who have been found to be susceptible to MH, we stressed the advisability of biopsying not only the patient but also the patient's parents. Positive responses for MH were obtained in the patients and in one of the par

[Respiratory failure and surgery].

The question whether a patient may be amenable to surgery involving anesthesia depends on many different factors and can only be answered by the anesthetist-surgeon team. The internist, on the other hand, will determine the degree of functional impairment of particular organ systems and cooperate in preoperative preparation of the patient. Patients with obstructive pulmonary disease are particularly at risk, especially during the postoperative period. The search for pulmonary symptoms is therefore extremly important. The limiting values given in medical literature are not absolute. Through pre

Assessing the Quality of Operative Notes in the Orthopedic Surgery Department at Gezira Traumatology Center, Sudan, in Accordance With the Royal College of Surgeons Guidelines.

BACKGROUND: High-quality operative notes are crucial for patient safety, continuity of care, and medico-legal protection. Despite established Royal College of Surgeons (RCS) guidelines, audits globally reveal persistent deficiencies in surgical documentation. This study assessed the quality of operative notes in the Orthopedic Surgery Department at Gezira Traumatology Center, Wad Madani Teaching Hospital, Sudan, and evaluated the impact of targeted interventions. METHODS: A clinical audit was conducted in two cycles (in July 2023 and then three months post-intervention). In the first cycle, 2

A delayed spontaneous expulsion of a three teeth bridge after 6 months period of aspiration in the right lung following cardiac surgery.

Aspiration of loose teeth is a well-known complication of endo-tracheal intubation hence the importance of oral check by anesthetist prior to ventilation. Artificaial teeth crown (single) or bridges (multiple) can be fixed or removable by the patient. The presence of a foreign body in the lung tissue or airways is a clinical situation that needs aggressive management as it can lead to refractory infections and possible death. We report this unique case of aspirarin of a three bridge teeth (10 mm × 30 mm) following cardiac surgery. The case is complicated by pneumonia, chronic cough and severe

Laryngospasm from the anesthesiologist's viewpoint.

Laryngeal spasm is a problem constantly confronting the anesthetist. It can be serious and may produce fatal cerebral or cardiac complications. Etiologic agents include primary vagal hypertonicity, anoxemia, and painful stimulation of whatever source. Laryngeal spasm must be differentiated from simple obstruction by the tongue or foreign bodies, epiglottic impaction, laryngeal edema, tracheal spasm and collapse, and bronchial spasm. Proper checking of the patient before anesthesia and adequate premedication with atropine or scopolamine are preventive measures of great value. Once spasm has dev

Success of airway management in out-of-hospital cardiac arrest using different devices - a prospective, single-center, observational study comparing professions.

BACKGROUND: Endotracheal intubation is the gold standard for airway management in out-of-hospital cardiopulmonary resuscitation (CPR) but re … amedics used laryngeal masks more frequently (34% vs. 1.5%, p < 0.001). Among emergency physicians, anesthetists had the lowest FPS using DL (p < 0.001). INTERPRETATION: The FPS rate did not differ between DL and VL but was low overall. This low rate, as well as the fact that emergency physicians and paramedics achieved comparable results in intubation, might …

Optimal nasopharyngeal temperature probe placement.

BACKGROUND: Although the nasopharynx is a commonly used temperature-monitoring site during general anesthesia, it is unknown whether the position of nasopharyngeal temperature probes placed blindly by anesthesia practitioners is optimal. The purposes of this study were (1) to determine where the nasopharyngeal mucosa is in closest proximity to the internal carotid artery (ICA) and (2) to evaluate the tip position of nasopharyngeal temperature probes that were placed by anesthesiology residents and nurse anesthetists. METHODS: In the first phase of the study, we reviewed enhanced axial compute…

Peri-operative management of diabetes mellitus: a multidisciplinary consensus statement from the Association of Anaesthetists and the Joint British Diabetes Societies for Inpatient Care group.

INTRODUCTION: Surgery in patients with diabetes mellitus is associated with increased morbidity and mortality compared with those who do not have diabetes mellitus. This is likely multifactorial and could be attributed to organisational issues; dysglycaemia; hospital-acquired diabetic ketoacidosis; errors with insulin prescribing and administration; issues with fluids and electrolytes; and systemic and surgical site infections. There was a need to update guidance for the peri-operative management of diabetes mellitus given improvements in our understanding, introduction of novel drugs and deve

Managing Simulated Anesthesia Emergencies with the Use of Clinical Decision Support Systems in a Rural Hospital Setting.

Certified registered nurse anesthetists (CRNAs) regularly encounter stressful emergency situations that demand critical thinking and quick intervention; these scenarios often result in task saturation and compromised mental acuity. An intervention utilizing a clinical decision support system (CDSS) with anesthesia emergency checklists built into the electronic health record (EHR) at a rural hospital in New Mexico was tested in a simulated environment. The simulation compared CRNA performance and adherence to evidence-based protocols with and without the checklists. The purpose of this study wa…
