Identification and Validation of Novel Biomarkers for Unstable Angina Using Olink Proteomics.

Unstable angina (UA) represents a prevalent cardiovascular condition characterized by significant morbidity and mortality, necessitating the development of effective early diagnostic biomarkers. This study sought to identify plasma protein biomarkers associated with UA utilizing Olink proteomics. A cohort of 120 participants, comprising 60 individuals diagnosed with UA and 60 healthy controls, was recruited. A total of 92 inflammation-related plasma proteins were quantified through a proximity extension assay. In the discovery cohort, differential expression analysis revealed significant alter

The utility of SYNTAX score predictability by electrocardiogram parameters in patients with unstable angina.

BACKGROUND: SYNTAX score is one of the risk assessment systems to predict cardiac events in acute coronary syndrome patients. Despite the large number of SYNTAX score benefits, invasive methods such as coronary angiography are necessary to perform the scoring. We hypothesized that ECG parameters could predict the SYNTAX score in unstable angina patients. METHODS: During the retrospective cohort study, a total number of 876 patients were diagnosed with unstable angina. After applying the exclusion criteria, 600 patients were divided into tertiles based on the SYNTAX scores as low (0-22), inter

PECAM1, COL4A2, PHACTR1, and LMOD1 Gene Polymorphisms in Patients with Unstable Angina.

Coronary artery disease (CAD) is a syndrome resulting from myocardial ischaemia of heterogeneous pathomechanism. Environmental and genetic factors contribute to its development. Atherosclerotic plaques that significantly narrow the lumen of coronary arteries cause symptoms of myocardial ischaemia. Acute coronary incidents are most often associated with plaque rupture or erosion accompanied by local activation of the coagulation system with thrombus formation. Plaque formation and stability are influenced by endothelial function and vascular smooth muscle cell function. In this study, we invest

Alterations in the fecal microbiota and serum metabolome in unstable angina pectoris patients.

BACKGROUND: Unstable angina pectoris (UAP) is a type of Coronary artery disease (CAD) characterized by a series of angina symptoms. Insulin-like growth factor 1 (IGF-1) system may be related to CAD. However, the correlation between the IGF-1 system, metabolism, and gut microbiota has not been studied. In the present study, we investigated the alterations of serum IGF-1 system, metabolomics, and gut microbiota in patients with UAP. METHODS: Serum and stool samples from healthy volunteers and UAP patients were collected. Serum metabolomics, PAPP-A, IGF-1, IGFBP-4, STC2, hs-CRP, TNF-α, and IL-6

A Novel Index for Prompt Prediction of Severity in Patients with Unstable Angina Pectoris.

OBJECTIVES: Rapid risk stratification by emergency department (ED) physicians to evaluate patients with chest pain for predicting the short-term occurrence of major adverse cardiac event (MACE) is crucial. The aim of this study was to investigate the predictive value of platelet-lymphocyte ratio (PLR) levels and compare with the modified heart score (m-HS) and stress testing to predict the severity of high-risk patients with unstable angina pectoris (UAP) in the ED. METHODS: This study is prospective which included 316 patients with UAP and 316 control healthy subjects. The study took place f

Middle-Aged Man With Unstable Angina and an Inaccessible Right Coronary Artery.

Acute myocardial ischemia and infarction from retrograde dissection of the aortic root into the coronary ostia is a potentially fatal condition. Unrecognized type A aortic dissection at the time of angiography for acute coronary syndrome (ACS) carries a high burden of morbidity and mortality. Cardiac computed tomography angiography (CCTA) has emerged as one of the instrumental tools in the diagnosis of retrograde coronary involvement from type A aortic dissections. We present a case of ACS secondary to retrograde aortic dissection extending into the right coronary artery (RCA) suspected during

Reducing Unnecessary Noninvasive Testing for Inpatients With Unstable Angina: The RUNIT Protocol.

BACKGROUND: Routine inpatient transthoracic echocardiography (TTE) for patients with unstable angina is common, but it anecdotally adds little value to clinical care. A practice audit at our academic hospital demonstrated that 61.5% of patients with troponin-negative chest pain (TNCP) had normal left ventriculography (LVG) during coronary angiography and normal TTE on the same admission (duplicate testing). METHODS: We developed the Reducing Non-Invasive Testing (RUNIT) protocol, a clinical algorithm applied by clinical nurses to patient with TNCP. We performed a prospective assessment of rat

Use of resting myocardial 18F-FDG imaging in the detection of unstable angina.

OBJECTIVE: Increased myocardial glucose metabolism occurs with the onset of myocardial ischemia and may persist even after the restoration of blood flow, termed as 'ischemic memory'. Previous studies have demonstrated that 18F-fluorodeoxyglucose (18F-FDG) is a sensitive marker of myocardial ischemia and may have potential utility in diagnosing unstable angina (UA). This study aimed to explore the value of F-FDG PET/CT in diagnosing UA. PATIENTS AND METHODS: Thirty-four patients (17 male patients; mean age, 59 ± 6 years) with suspected UA were prospectively recruited. Resting myocardial F-FDG

[The effect of clopidogrel and aspigrel on myocardial ischemia in patients with unstable angina pectoris].

The modern cardiology pays much attention to research of myocardial ischemia, especially painless myocardial ischemia as the problem of sudden coronary death. Findings from the current study suggest that antianginal therapy which includes antiaggregant and anticoagulant is effective because of decreasing general myocardial ischemia and its painless form.

Coronary artery bypass grafting in a patient with unstable angina pectoris and bronchiectasis.

Bronchiectasis is characterized by the abnormal and permanent dilatation of bronchi. Clinical manifestations of bronchiectasis include persistent or recurrent cough, purulent sputum, hemosputum, and hemoptysis. A 75-year-old man with bronchiectasis required coronary bypass grafting for unstable angina pectoris with severe stenosis of the left main trunk. Computed tomography showed fistulae between the dilated bronchial arteries and the left pulmonary artery. Cardiac catheter examination showed significant left-right shunt and left ventricular dilatation. To avoid perioperative massive hemoptys

An evidence against the effect of chronic cytomegalovirus infection in unstable angina pectoris.

Recent reports have suggested that cytomegalovirus (CMV) infection may contribute to risk of cardiovascular disease. However, relationship between CMV infection and unstable angina (UA) is controversial and studies about this subject in Iran and even region are lacking. The aim of this study was to determine whether unstable angina is related to seropositivity to chronic cytomegalovirus infection. We measured serum CMV IgG levels in a case control study participants in CCU in Razi Hospital, Ahvaz, Iran, from 2004 to 2005. Blood samples were drawn during study period from 96 patients (mean age

Responsiveness to aspirin in patients with unstable angina pectoris by whole blood aggregometry.

AIMS: To evaluate aspirin responsiveness in patients with unstable angina pectoris (UAP) by whole blood aggregometry. Another goal was to differentiate aspirin-resistant patients into pharmacokinetic or pharmacodynamic type. METHODS: We measured platelet aggregation by determining impedance values in 70 normal volunteers and 104 UAP patients on aspirin (100 mg/day > or = 7 days) in four inducing conditions [1 microg/ml collagen, 2 microg/ml collagen, 5 micromol/l adenosine diphosphate (ADP) and 10 micromol/l ADP]. We calculated a cut-off value based on data from normal volunteers to define as

High plasma levels of matrix metalloproteinase-8 in patients with unstable angina.

Matrix metalloproteinases (MMPs) play a role in collagen breakdown, leading to plaque instability. High levels of MMPs mRNA and proteins, especially MMP-1, MMP-2, MMP-8, MMP-9, and MMP-13, were shown in human atherosclerotic plaques. However, among various MMPs, only MMP-1, MMP-8 and MMP-13, so-called interstitial collagenases, can initiate collagen breakdown. To elucidate whether MMP-1, MMP-8 and MMP-13 levels in blood were high in patients with unstable angina (UAP), we measured serum MMP-1 and plasma MMP-8 and MMP-13 levels in 45 patients with UAP, 175 with stable coronary artery disease (C
