[Fractures of the anterior process of the calcaneus-frequently overlooked injuries following ankle sprains].

Fractures to the anterior process of the calcaneus (PAC) have long been considered rare injuries and have received little attention in clinical research. On the contrary, recent studies have reported a distinct higher incidence, especially following ankle sprains. Decisive reasons are that fractures of the PAC are regularly missed on plain radiographs and that a clinical differentiation from injuries to the lateral ankle ligaments is difficult. With the broad availability of cross-sectional imaging modalities fractures of the PAC are diagnosed more frequently and more reliably. The purpose of

Human Foot-Ankle Injuries and Associated Risk Curves from Under Body Blast Loading Conditions.

Under body blast (UBB) loading to military transport vehicles is known to cause foot-ankle fractures to occupants due to energy transfer from the vehicle floor to the feet of the soldier. The soldier posture, the proximity of the event with respect to the soldier, the personal protective equipment (PPE) and age/sex of the soldier are some variables that can influence injury severity and injury patterns. Recently conducted experiments to simulate the loading environment to the human foot/ankle in UBB events (~5ms rise time) with variables such as posture, age and PPE were used for the current s

[Occult fractures of extremities in adults and children. Exemplified by foot and ankle injuries].

BACKGROUND: Occult fractures in children and adults cannot by definition be diagnosed by conventional radiographs. These injuries are usually recognized as bone marrow edema by magnetic resonance imaging (MRI). There are no randomized controlled trials or prospective cohort studies concerning the correct management of occult fractures and, therefore, no evidence-based treatment guidelines can be drafted. OBJECTIVES: This article summarizes the current diagnostic and treatment concepts for occult fractures under special consideration of foot and ankle injuries. METHODS: A selective search of …

Imaging in the assessment and management of overuse injuries in the foot and ankle.

Overuse injuries of the ankle and foot are common in the general and athletic populations. The wide spectrum of overuse injuries includes ligamentous injuries, soft tissue and osseous impingement, osteochondral lesions, tendon injuries, and stress fractures. Some conditions such as impingement syndromes and stress fractures may be missed on initial physical examination, and patients with such injuries often present to a sports or orthopedic clinic with persistent symptoms. With the increasing participation in sports, health-care professionals involved in the care of athletes at all levels must

Rates of ankle and foot injuries in active-duty U.S. Army soldiers, 2000-2006.

Ankle and foot injuries (AFI) are a major cause of Active-Duty Army (ADA) soldiers' time lost from training and combat operations. We used the Total Army Injury and Health Outcomes Database to compute the rates of AFI to identify high-risk ADA groups for the years 2000-2006. During this time, 16% of soldiers were clinically seen at least once for an AFI. Yearly, 60% to 70% of ADA soldiers with AFI had an ankle sprain/strain, and ankle sprain/strain had the highest 7-year rate of all AFIs (103 per 1,000). From 2000 to 2006, all AFI rates declined; however, enlisted male soldiers < or = 30 years

Interosseous membrane: The anatomic basis for combined ankle and common fibular (peroneal) nerve injuries.

Common fibular (peroneal) nerve palsy has many etiologies. A rare cause with a poorly understood pathophysiology is common fibular nerve palsy that follows an ankle injury. Since the original description of this pathologic entity a century ago, several mechanisms have been posited. We present two cases of common fibular nerve palsy following ankle injury. These cases provide anatomic and radiologic evidence to support a hypothesis that the interosseous membrane provides the anatomic basis for this combined pattern of injury. This theory unifies previously postulated mechanisms.

Reverse sural artery flap for reconstruction of blast injuries of the foot and ankle.

BACKGROUND: The reverse sural artery fasciocutaneous flap is a versatile soft tissue coverage procedure for traumatic soft tissue defects of the distal tibia, ankle, and foot. War-related blast injuries represent a unique injury mechanism. There are no reports on use of this reconstructive flap in treating highly contaminated war-related musculoskeletal trauma of the foot and ankle. The current study presents results using this soft tissue coverage procedure in a subacute fashion to treat a population of war-related blast injuries. MATERIALS AND METHODS: A retrospective chart study was perfor

[Ankle injuries without fracture in children. Prospective study with magnetic resonance in 116 patients].

PURPOSE OF THE STUDY: Appropriate assessment of ankle injuries in children and adolescents is a common emergency room problem. Many imaging techniques have been proposed, but with no consensus on the reality of anatomic lesions in ankles free of fractures, complicating the therapeutic decision. We analyzed the lesions observed with magnetic resonance imaging (MRI) in a large number of acute ankles in children. MATERIAL AND METHODS: This prospective study was conducted in a pediatric emergency room. The study population included all children aged eight to 15 years who presented an isolated inj…

Cost-effectiveness of the SEN-concept: Specialized Emergency Nurses (SEN) treating ankle/foot injuries.

BACKGROUND: Emergency Departments (EDs) are confronted with progressive overcrowding. As a consequence, the workload for ED physicians increases and waiting times go up with the risk of unnecessary complications and patient dissatisfaction. To cope with these problems, Specialized Emergency Nurses (SENs), regular ED-nurses receiving a short, injury-specific course, were trained to assess and treat minor injuries according to a specific protocol. METHODS: An economic evaluation was conducted alongside a randomized controlled trial comparing House Officers (HOs) and SENs in their assessment of

Overuse and sports-related injuries of the ankle and hind foot: MR imaging findings.

Professional and recreational sporting activities have increased substantially in recent years and have led to a rise in the number of sports-related and overuse injuries. Magnetic resonance (MR) imaging has become an important tool for evaluating the lower leg for providing the necessary information for patient management and rehabilitation following this injury. The purpose of this essay is to give an overview of the MR findings of common overuse injuries and sports-related injuries to the bones and soft-tissue structures of the hind foot and ankle.

[Magnetic resonance imaging in tendon and ligament injuries of the ankle. Our personal experience].

While Magnetic Resonance Imaging (MRI) has become a routine diagnostic method in knee studies, little has been published about ankle injuries. This is probably due to the anatomic complexity of the bone and ligament structures of this joint. Our study was aimed at assessing the yield of MRI of the ankle with a cost-effective, compact, low-field (0.2 T) M(R) system (Artoscan, Esaote). January, 1994, to August, 1995, 148 ankle examinations were carried out in a series of patients whose age ranged 7 to 75 years. In 56.7% of cases the exam was performed because of acute joint strain and in 43.3% f…

Ankle Syndesmotic Injury With Posterior Tibial Tendon Rupture.

Isolated ankle syndesmotic injuries are uncommon and are typically discussed in the context of athletic high ankle sprains; however, concomitant rupture of the posterior tibial tendon represents an exceedingly rare injury pattern. This case highlights the importance of maintaining a high index of suspicion for associated soft-tissue pathology in patients presenting with syndesmotic instability in the absence of fracture. This case underscores the need for comprehensive clinical and imaging evaluation of isolated syndesmotic injuries and supports primary tendon repair when rupture is identified
