Readhesion of Tongue-Tie Following Neonatal Frenotomy: Incidence and Impact of Postoperative Exercises in a Prospective Observational Study.

Background/Objectives: Frenotomy is the procedure of choice for treating ankyloglossia. The literature reports that readhesion of the frenulum occurs in 2.6-13% of cases. There is no published evidence to support performing tongue exercises to prevent it. We aimed to determine the readhesion rate of ankyloglossia, the benefits of tongue exercises to prevent it, and the characteristics of patients who experienced readhesion. Methods: This is a prospective, observational study of neonates who underwent a frenotomy between January and August 2024. Following the frenotomy, we recommended that all …

Outcomes of stretching exercises after lingual frenotomy in infants: A prospective, interventional study.

OBJECTIVE: To evaluate the safety and efficacy of stretching exercises after frenotomy for ankyloglossia. STUDY DESIGN: Prospective cohort study. SETTING: Single academic tertiary care hospital. METHODS: Subjects with ankyloglossia were enrolled from 4/5/2021-4/5/2023. All patients underwent in-office frenotomy using scissors. Patients enrolled from 4/5/2021-10/5/2021 and 4/6/2022-4/5/2023 were assigned to the stretching group; those enrolled from 10/6/2021-4/5/2022 were assigned to the non-stretching group. Caregivers of patients in the stretching group were instructed on how to perform st…

Reducing Racial, Ethnic, and Socioeconomic Disparities in Frenotomy Practice.

Objective: Surgical correction of ankyloglossia through frenotomy has increased over 10-fold in the past decade, despite not seeing any significant improvement in breastfeeding rates. Due to heightened attention to this diagnosis and lack of consensus on treatment, we sought to evaluate our regional practice and standardize the screening for ankyloglossia. Materials and Methods: A retrospective cohort study of frenotomy practice within the birth hospital stay was conducted from June 1, 2019, to June 30, 2021. Patients were identified through billing data and grouped according to race, ethnicit…

A quick snip - A study of the impact of outpatient tongue tie release on neonatal growth and breastfeeding.

A prospective study (62 neonates) of ankyloglossia and breastfeeding difficulty is presented. At 2 weeks post-frenulotomy, there were significant improvements in weight (15+/-1.2 centiles) and breastfeeding, including number of sessions/24h (19%) and bottle top-ups/24h (81%) (p<0.0001).

Tessier 30 facial cleft.

A case of midline cleft of the lower lip with cleft of the mandible and complete duplication of the tongue is reported here. Median cleft of the lower lip, mandible and bifid tongue with ankyloglossia is reported in the literature, but complete duplication of the tongue as part of the Tessier 30 cleft is not yet reported.

Ankyloglossia was present in nine percent of newborns, and congenital eruption cysts and neonatal teeth were present in 0.5 percent and 0.2 percent, respectively. Ankyloglossum superius syndrome: diagnosis and surgical management.

Ankyloglossum superius (or ankyloglossia superior) is a rare congenital craniofacial condition characterized by tongue tip adherence to the hard palate and associated limb abnormalities. There have been few reports of this condition in the literature. We present a patient with ankyloglossum superius syndrome and discuss diagnosis and surgical management.

Lingual Frenectomy in 3-9-Year-Old Children Using Diode Laser: A Case Series.

Ankyloglossia, or tongue-tie, not only causes speech impairment but also causes problems like difficulty in breastfeeding in infants, restricts tongue movement, which affects its mobility to clean teeth, restricts mandibular growth, and results in a reverse swallow pattern. Conventional surgical techniques for frenectomy can be difficult in children. However, with diode laser, there is controlled bleeding and surgery is painless. The purpose of this article is to present five cases of lingual frenectomy using the diode laser in young children with moderate-severe ankyloglossia. Five children a…

Tongue and Mandibular Disorders of the Pediatric Patient.

Robin sequence, macroglossia, and ankyloglossia are disorders affecting the tongue and mandible in the pediatric population. Each of these can have a significant impact on breathing, feeding, speech, dentition, and craniofacial growth. This review discusses the interdependent and coordinated development of both the tongue and mandible, the functional impacts of these disorders, and appropriate management strategies.

Acromelia-oligodontia syndrome.

This case report describes a patient with ankyloglossia, oligodontia, unilateral hypoplasia of the zygoma and mandible, along with bilateral distal reduction anomalies of his limbs without long bone abnormalities. This may represent a mild variant of oromandibular limb hypogenesis syndrome, expanding the phenotypic spectrum, or a previously unrecognized malformation syndrome.

Unmasking familial CPX by WES and identification of novel clinical signs.

Mutations in the T-Box transcription factor gene TBX22 are found in X-linked Cleft Palate with or without Ankyloglossia syndrome (CPX syndrome). In addition to X-linked inheritance, ankyloglossia, present in the majority of CPX patients, is an important diagnostic marker, but it is frequently missed or unreported, as it is a "minor" feature. Other described anomalies include cleft lip, micro and/or hypodontia, and features of CHARGE syndrome. We conducted whole exome sequencing (WES) on 22 individuals from 17 "a priori" non-syndromic cleft lip and/or cleft palate (CL/P) families. We filtered t…

Lingual frenotomy in neonates: past, present, and future.

During the last decade, increasing awareness of breastfeeding and its health benefits has not been reflected in the provision of lingual frenotomy in neonates with tongue-tie. This could be because of inconsistencies in our understanding of the importance and treatment of ankyloglossia. In this review, we discuss the current clinical guidance on diagnosis and management, and the future of such a service in the early postpartum period.

Developing a frenotomy policy at one medical center: a case study approach.

The objective of this study was to change procedures in our medical center regarding frenotomy for ankyloglossia (tongue-tie). The medical and breastfeeding outcomes of 36 fullterm infants who received frenotomies were studied. The information was used to develop frenotomy eligibility standards that would guide other physicians and insure timely treatment to avoid breastfeeding cessation.

Allergic Reactions to Tetracaine-Phenylephrine in Neonatal Lingual Frenotomy: Clinical Insights.

BACKGROUND Infants born with a short lingual frenulum, a condition known as ankyloglossia or tongue-tie, can experience breastfeeding difficulties and often require surgical frenotomy to restore functional mobility. Topical anesthesia is commonly used to minimize discomfort during the procedure, and adverse reactions are rare. This case report describes a 48-day-old female infant who experienced an allergic reaction to topical anesthesia with tetracaine and phenylephrine during frenotomy. CASE REPORT A 48-day-old infant with ankyloglossia and breastfeeding difficulties was referred for special…

Tessier 30 Facial Cleft with Duplication of Tongue.

A case of midline cleft of the lower lip, mandible, and the neck with complete duplication of the tongue repaired at neonatal period is reported here. Median cleft of the lower lip, mandible, and bifid tongue with ankyloglossia is reported in the literature, but cleft of the neck with complete duplication of the tongue as a part of the Tessier 30 cleft is very rare. We could not find such report in the available English literature.
