A 49-year old right-handed taxi-driver experienced right upper limb weakness and global aphasia following internal carotid artery occlusion. Five months later, aphasia and hemiparesis had resolved but he complained of difficulties in his daily activities, termed "inner conflict". Specific testings disclosed typical features of a callosal syndrome: left unilateral ideomotor apraxia, left hand agraphia, left tactile dysnomia, right hand constructional difficulties, and left ear extinction on dichotic listening. Naming and recognition of tachistoscopically presented images or words was similar to

The paper describes the spontaneous recovery of language abilities of 52 stroke patients who were aphasic for more than 4 weeks. These patients had been randomly allocated to receive no speech therapy and had been assessed at 6-weekly intervals after a stroke. There was improvement in language abilities over time. Age, sex and aphasia type were not related to the amount of improvement. An aphasic patient's level of language ability at 6 months could be predicted on the basis of the test score on the Porch Index of Communicative Ability at 4 weeks.

PURPOSE: This Phase I study investigated behavioral and functional MRI (fMRI) outcomes of 2 intensive treatment programs to improve naming in 2 participants with chronic moderate-to-severe aphasia with comorbid apraxia of speech (AOS). Constraint-induced aphasia therapy (CIAT; Pulvermüller et al., 2001) has demonstrated positive outcomes in some individuals with chronic aphasia. Whether constraint to the speech modality or treatment intensity is responsible for such gains is still under investigation. Moreover, it remains to be seen whether CIAT is effective in individuals with persistent seve

OBJECTIVE: To develop a quantitative method of assessing cognitive anosognosia in six cognitive and two noncognitive domains.
METHODS: Control (n = 32) and probable Alzheimer disease (pAD) (n = 14) subjects self-estimated memory, attention, generative behavior, naming, visuospatial skill, limb praxis, mood, and uncorrected vision, both before and after these abilities were assessed. Based on this estimate and their performance the authors calculated an anosognosia ratio (AR) by dividing the difference between estimated and actual performance by an estimated and actual performance sum. We assessed spelling and reading abilities in 14 patients with semantic dementia (with varying degrees of semantic impairment) and 24 matched controls, using spelling-to-dictation and single-word reading tests which manipulated regularity of the correspondences between spelling and sound, and word frequency. All of the patients exhibited spelling and reading deficits, except at the very earliest stages of disease. Longitudinal study of seven of the patients revealed further deterioration in spelling, reading, and semantic memory. The performance of both subject groups on both spelling and read

We report investigations of the face processing abilities of SP, a right-handed woman who had suffered a subarachnoid haemorrhage from a right middle cerebral artery aneurysm. Although she could correctly assign visual stimuli to the 'face' category without difficulty, SP performed poorly on all other face processing, tasks, including 'closure' (Mooney faces), perception of facial expression, unfamiliar face matching, and identification of familiar faces. Identification of familiar people from nonfacial cues (names) remained relatively well-preserved, but severe impairments were evident on all

INTRODUCTION: Aphasia, particularly in persons with nonfluent aphasia (PWNA), disrupts speech fluency. Script treatment-delivered with recursive self-feedback (RSF) or external feedback (EF)-has been shown to improve fluency in PWNA. Evidence from anomia treatment suggests that structured practice schedule (continuous vs. discontinuous) may further influence outcomes. However, it remains unknown how feedback type and PWNA's self-selected practice schedules-reflecting the variability of real-life routines-interact to influence language outcomes. This study introduces a novel self-selected pract

Fifty-five patients who suffered from COVID-19, who were still very ill after several months, with extreme fatigue, effort exhaustion, brain fog, anomia, memory disorder, anosmia, dysgeusia, and other multi-systemic health problems have been followed in a family practice setting between May 2021 and July 2022. Data extracted from the medical records of the 55 patients (40 women), mean age 42.4 (12 to 79 years), and a qualitative study of 6 of them using a semi-open-ended questionnaire allowed to highlight the clinical picture described by WHO as post-acute COVID-19 syndrome (PACS) also known a

We describe the clinical features, neuropsychological tests, laboratory, electroencephalography (EEG), magnetic resonance imaging (MRI) and positron emission tomography (PET) findings of a 59-year-old woman who presented to our Centre for cognitive impairment since few months, with language disturbances, particularly anomia, dyscalculia, and memory loss. The clinical and neuropsychological features were non-specific and overlapping with those of other rapidly progressing neurodegenerative disorders. However, brain MRI played a pivotal role in the diagnosis, showing cortical diffusion restricti

The consensus criteria for the diagnosis and classification of primary progressive aphasia (PPA) have served as an important tool in studying this group of disorders. However, a large proportion of patients remain unclassifiable whilst others simultaneously meet criteria for multiple subtypes. We prospectively evaluated a large cohort of patients with degenerative aphasia and/or apraxia of speech using multidisciplinary clinical assessments and multimodal imaging. Blinded diagnoses were made using operational definitions with important differences compared to the consensus criteria. Of the 130

BACKGROUND: Language sites in the cortex of the brain vary among patients. Language mapping while the patient is awake is an intraoperative technique designed to minimize language deficits associated with brain-tumor resection.
METHODS: To study language function after brain-tumor resection with language mapping, we examined 250 consecutive patients with gliomas. Positive language sites (i.e., language regions in the cortex of the brain, 1 cm by 1 cm, which were temporarily inactivated by means of a bipolar electrode) were identified and categorized into cortical language maps. The tumors wer

We investigate the on-line processing of verb-phrase ellipsis (VPE) constructions in two brain injured populations: Broca's and Anomic aphasics. VPE constructions are built from two simple clauses; the first is the antecedent clause and the second is the ellipsis clause. The ellipsis clause is missing its verb and object (i.e., its verb phrase (VP)), which receives its reference from the fully specified VP in the antecedent clause. VPE constructions are unlike other sentence types that require displacement of an argument NP; these latter constructions (e.g., object-relatives, wh-questions) yie

AIM: To investigate the relationship between performance in language tests and levels of brain metabolites in two selected left temporal lobe regions.
METHODS: Ninety-five subjects were included: 26 controls, 30 amnestic mild cognitive impairment subjects, 27 Alzheimer's disease and 12 frontotemporal lobar degeneration (FTLD) patients. Language was assessed by a naming test: Boston Naming Test (BNT) and by a semantic verbal fluency test. Other cognitive functions: verbal and visual memory, visual perception, attention and executive function, and praxis were also assessed. Single voxel magneti

A striking dissociation between the inability to identify living things plus food, along with a preserved ability to identify inanimate objects has been observed in patients who are recovering from herpes simplex encephalitis. In order to find out if a similar dissociation is also present in Alzheimer's disease (where the suspected initial site of parenchymal atrophy involves the same areas affected by herpes simplex virus), we carried out five experimental linguistic tasks to compare Alzheimer's patients' performance therein with that of vascular dementia patients and controls. Our results in

There are two alternative interpretations of how word recognition is accomplished in letter-by-letter reading. One postulates that it relies on the spelling system operating "in reverse," whereas the other claims that it is mediated by the reading system. Because of the close association between patterns of reading and spelling in previously reported cases of letter-by-letter reading, both hypotheses may be considered equally plausible. We now report a patient with letter-by-letter reading who demonstrated a striking dissociation between reading and spelling. Our observations in this patient a

Semantic memory (SM) was investigated in six patients with probable Alzheimer's disease (AD) by on-line measurement of semantic priming in a lexical decision task, and off-line tests of comprehension. Detailed assessment was carried out on naming, name comprehension, and probes of semantic knowledge with a battery of 150 items. The patients performed normally on perceptual tests and displayed an item-specific loss of knowledge on the semantic tests. In a primed lexical decision task, greater semantic priming was found relative to age-matched normals. The priming was substantially greater for i

The tip-of-the-tongue state (TOT) is the feeling that accompanies temporary inaccessibility of an item that a person is trying to retrieve. TOTs have been studied experimentally since the seminal work of Brown and McNeill (1966). TOTs are experiences that accompany some failed or slow retrievals, and they can result in changes in retrieval behavior itself, allowing us to study the interplay among experience, retrieval, and behavior. We often attribute the experience of the TOT to the unretrieved target, but TOTs are based on a variety of cues, heuristics, or sources of evidence, such as partia

The case of a patient with selective left hemispheric medial and lateral temporal lobe damage is described. The patient was of slightly supra-average intelligence and had no problems in normal memory functions, but was severely anomic with respect to people's names. One month post-onset, this deficit held for names of colleagues and friends she had gotten to know during the last 10 years prior to the infarct and for all names confronted with post-infarct. On the other hand, learning of face-name associations was preserved and was independent of the ability to generate context-specific informat

We report the case of a patient who, after sequential bilateral strokes in the occipital regions sparing the primary visual cortex, developed a severe deficit of colour perception. At variance with other reports of acquired achromatopsic patients, she showed a perfectly vivid visual imagery for colours. These findings, together with similar data in domains other than colour processing, challenge the theories which posit that the same cognitive processes are involved in both the perception and the retrieval from memory of a given stimulus.

Experiments were designed to examine the imagery abilities of an agnosic patient, M.S., who has consistently shown more severe deficits in recognizing visually, and in retrieving knowledge of living as compared with non-living items. Judgements of visual similarity were required for named objects and for object-pictures, as well as for the factual properties of these stimuli. The same disproportionate difficulty in processing living ('natural') objects was found in these tasks as well as in forced-choice recognition. In contrast, no deficit was found on analogous tasks concerned with word-shap
