This randomized prospective study evaluated the safety and efficacy of a single oral dose of OFL compared to a single parenteral dose of CTRX prior to TURP. 191 patients (mean age: 68.7 +/- 6.2 years) with bacterial free urine before surgery were enrolled and received either OFL: 400 mg per os (n = 95) or CTRX: 1 g intrasmuscularly (n = 96) at the pre-anaesthetic medication time (two hours prior TURP). Two urine cultures were obtained: on the day of the patient's discharge and within one month after surgery. Blood cultures were performed in case of temperature > 38 degrees 5C. Treatment failur

A prospective observational study of postoperative infection after gynecologic surgery assessed the need for antibiotic prophylaxis with special reference to the urinary tract. Catheterization requirements in the postoperative period were compared with the development of urinary tract infection after excluding both preoperative and postoperative bacteriuria. Forty-six of 115 patients (40%) developed a urinary tract infection in the postoperative period. Furthermore, this was not clearly related to the need for postoperative catheterization. Significant wound and vaginal vault infections were u

BACKGROUND: Pancreatoduodenectomy (PD) is a morbid procedure often associated with high rates of postoperative infectious complications, contributing to worse prognosis. We sought to investigate the optimal surgical prophylaxis antibiotic for PD patients using the ACS-NSQIP.
STUDY DESIGN: The ACS-NSQIP (2016-2023) was queried for elective PDs and their prophylactic antibiotics. Patients were categorized by antibiotic type: first-generation cephalosporin (FC), second/third-generation cephalosporin (STC), and broad-spectrum (BS - e.g., piperacillin-tazobactam, cefoxitin, and fourth-generation c

OBJECTIVE: To evaluate the compliance with antibiotic prophylaxis (AP) guidelines after pancreatic surgery, identify predictors of extended AP usage (>24 h), and assess the association between extended AP and surgical site infection (SSI).
METHODS: A retrospective cohort study was conducted involving 1,073 patients who underwent pancreatic surgery at the First Affiliated Hospital with Nanjing Medical University between January and December 2022. Based on the strict intent-of-use definition in the electronic medical record, patients were categorized into the AP > 24 h group (n = 135) and the N

Dermatologic surgery primarily involves clean procedures with low baseline infection rates. This 2-part continuing medical education article synthesizes current evidence to guide perioperative antibiotic decision-making for excisional dermatologic surgery, including standard excisions, Mohs micrographic surgery, and associated reconstructive techniques. Part I reviews indications for systemic prophylaxis, emphasizing a risk-stratified approach that incorporates procedural, anatomic, and patient-specific factors. Special attention is given to high-risk populations, including immunocompromised a

Through vaginal colonization, GBS causes severe outcomes including neonatal sepsis and meningitis. Although intrapartum antibiotic prophylaxis (IAP) has reduced neonatal disease rates, persistent GBS colonization has been observed in patients following prophylaxis. To determine whether IAP selects for genomic signatures that enhance GBS survival and persistence, a pangenome analysis was performed on 97 isolates from 58 participants before (prenatal) and after (postpartum) IAP/childbirth. Thirty-one of the 34 paired strains from participants with persistent colonization clustered together in th

Gustilo type III open fractures involve extensive soft tissue damage and wound contamination that pose significant infection risks. The historical standard for antibiotic prophylaxis has been cefazolin and gentamicin. We conducted a retrospective cohort study of lower extremity type III open fractures treated with ceftriaxone alone for prophylaxis. Eighty-six patients were identified. Nearly all (98%) were managed with appropriate antibiotics, but only 55 (64%) received prophylaxis within 1 hour. Overall, there were 12 infections. This infection rate was not statistically different than the re

Antibiotic prophylaxis is frequently used in plastic surgery procedures, despite the generally low rates of infection and few guidelines on the practice. The increasing prevalence of bacterial resistance to antibiotics calls for a reduction in unnecessary antibiotic use. The aim of this review was to create an updated summary of the available data on the effectiveness of antibiotic prophylaxis in reducing the postoperative infection in clean and clean-contaminated plastic surgery. A systematic literature search was performed on the databases Medline, Web of Science, and Scopus, limited to arti

INTRODUCTION AND HYPOTHESIS: To our knowledge, there are no evidence-based recommendations regarding the optimal prophylactic antibiotic regimen for intradetrusor onabotulinum toxin type A (BTX) injections. This systematic review and meta-analysis was aimed at investigating the optimal prophylactic antibiotic regimen to decrease urinary tract infection (UTI) in patients undergoing BTX for overactive bladder syndrome (OAB).
METHODS: A systematic search of MEDLINE, Embase, CINAHL, and Web of Science was conducted from inception through 30 June 2022. All randomized controlled trials and prospect

BACKGROUND: Although infections are a significant potential complication among patients undergoing left ventricular assist device (LVAD) implantation, standardized surgical infection prophylaxis (SIP) regimens are not well defined. At Montefiore Medical Center, a 4-drug SIP regimen containing fluconazole, ciprofloxacin, rifampin, and vancomycin was previously utilized. In January 2020, the antimicrobial stewardship program implemented a 2-drug SIP regimen of vancomycin and cefazolin to limit exposure to broad-spectrum antibiotics. This study evaluated LVAD-associated infection rates prior to a

OBJECTIVES: Transrectal ultrasound guided prostate biopsy (TRUSPB) is the standard of care for diagnosis of prostate cancer. Increased antibiotic resistance has led to the suspension of indication for fluoroquinolones use in prostate biopsy prophylaxis. Several classes of antibiotics have been recommended for routine use. Unequivocal consensus regarding antibiotic prophylaxis has not been made to date. The objective of the study was to assess the diversity of antimicrobial prophylaxis among Croatian urologists.

PURPOSE OF REVIEW: Patients with hematological malignancies are recognized for their high susceptibility and increased risk of developing infections associated with immunosuppression that can be caused by the infection itself or by the treatments that condition a decrease in the humoral and T lymphocyte response, so this review attempts to gather the main bacterial, viral, parasitic, and fungal agents that affect them and give recommendations for their approach and diagnosis.
RECENT FINDINGS: In recent years, with the discovery and use of new therapies including immunological and targeted tre

Secondary prophylaxis of rheumatic heart diseases is efficient in reducing disease recurrence, heart damage, and cardiac impairment. We aimed to monitor the clinical evolution of a large Brazilian cohort of rheumatic patients under prolonged secondary prophylaxis. From 1986 to 2018, a cohort of 593 patients with rheumatic fever was followed every 6 months by the Reference Center for the Control and Prevention of Rheumatic Fever and Rheumatic Cardiopathy (CPCFR), Paraná, Brazil. In this cohort, 243 (41%) patients did not present cardiac damage (group I), while 350 (59%) were diagnosed with rheu

BACKGROUND: Post-caesarean section infection is a cause of maternal morbidity and mortality. Administration of antibiotic prophylaxis is recommended for preventing infection after caesarean delivery. The route of administration of antibiotic prophylaxis should be effective, safe and convenient. Currently, there is a lack of synthesised evidence regarding the benefits and harms of different routes of antibiotic prophylaxis for preventing infection after caesarean section.
OBJECTIVES: The aim of this review was to assess the benefits and harms of different routes of prophylactic antibiotics giv
