A 76-year-old man was admitted for recurrent hemoptysis of 3 weeks' duration. Chest X-ray revealed lung emphysema, but no findings of thorac … optysis, the possibility of aortopulmonary fistula should be considered, and computed tomography or aortography should be done to rule out other pulmonary diseases.

A case of bilateral fractures of the first rib occurring in an otherwise fit road traffic accident victim is described. The only other injuries sustained were of the peripheral limbs. The driver's air bag was inflated during the crash, leading to speculation as to whether this may have contributed to the mechanism of injury. The patient was well oxygenated and cardiovascularly stable with no evidence of neurovascular damage to the thoracic aorta or its branches. Aortic arch aortography was not performed before internal fixation of the peripheral fractures was undertaken under general anaesthes…

Reported herein is the case of a 47 year old man with mitral and aortic bioprosthetic valves who developed endocarditis. Blood cultures turned out to be positive for Gemella haemolysans, a commensal of the upper respiratory tract. Proper antibiotic therapy was immediately started according to antibiogram, and continued after the acute stage of the disease had been overcome. The aortic prosthesis appeared much more to be incompetent at Doppler and aortography (from 2/4 to 3/4). At surgery, the gross appearance of the explanted xenograft was consistent with infective endocarditis. The case is de…

A 62-year-old man who had complained of swelling of bilateral lower extremities, exertional dyspnea and jaundice for 3 months was referred to our hospital with a diagnosis of large abdominal aneurysm and congestive heart failure. His aortography and Doppler echocardiography revealed that he had the aorto-caval fistula due to rupture of abdominal aorto-iliac aneurysm. He underwent a reconstruction of abdominal aorta with woven Dacron bifurcation graft together with a closure of fistula with 3 mattress sutures. The postoperative course was uneventful and he is free from any symptoms. Problems of…

Obstruction of the left subclavian artery is one of the rarest causes of the cogenital subclavian steal syndrome with a right-sided aorta. Radiological diagnosis can be made from a standard film which shows the aorta on the right, and a barium examination of the esophagus which demonstrates the absence of any compression posteriorly. Angiographic exploration (aortography, pulmonary angiography) is essential for positive confirmation of diagnosis and to exclude other types of congenital subclavian steal with a right-sided aorta. Certain cardiac malformations may be associated, but forms have be…

In one patient with pulmonary sarcoidosis, hypertension occurred during the course of the disease. Aortography showed extensive narrowing of the right renal artery. Surgical exploration disclosed extensive periaortic and perirenal fibrosis. This fibrosis encircled the right renal artery and caused extrinsic compression. Pathological examination disclosed a large amount of histiocyte epithelioid infiltration in various samples of the fibrosis and particularly in the adventitia of the renal artery, highly suggestive of sarcoidal angiitis. Surgical biopsy was performed on both kidneys. The right …

Aortography and angiography of the aortic arch were carried out in 176 patients with peripheral arterial occlusive disease. In 63,6% there were haemodynamically relevant stenoses or occlusions of the supraaortic vessels, the carotid arteries with 39,2% and the vertebral arteries with 35,2%, being the most commonly involved. Peripheral arterial disease included haemodynamically relevant changes in the femoro-popliteal arteries in 83%, in the aorto-iliac region in 67,1% and in the calf vessels in 58,5%. There was a statistically significant correlation between the clinical severity of the cerebr…

For surgical treatment of patients with tracheoesophageal fistulas the localization of the aortic arch is great practical importance. Aortography and CT scanning have been advocated as the radiographic techniques of choice. The location of the aortic arch can be diagnosed on a chest x-ray by the deviation of the trachea alone. A barium swallow is not required. The optimal radiographic technique for this purpose was investigated in phantom studies and tested in animals and newborns. It is proposed that in addition to the standard chest x-ray a high kvp film with hardened beam (1 mm copper -- 1 …

The distinction between nondissecting aneuryms of the thoracic aorta and thoracic neoplasms may be difficult. The aortographic findings associated with aneurysms may be subtle. However, when the aortogram is properly performed and interpreted and the findings correlated with the plain chest roentgenograms the distinction between aneurysms and neoplasms may be made consistently. The thoracic aortogram should be filmed in at least 2 projections and abdominal aortography and ultrasonography should be performed. With aneurysms the aortographic signs include widening (often slight) of the aortic lu…

Of 450 patients undergoing internal mammary artery grafts for coronary artery bypass, proximal subclavian artery stenosis with reversal of flow in the internal mammary and vertebral arteries ("coronary-subclavian steal") subsequently developed in two patients. Carotid-subclavian bypass successfully reestablished antegrade blood flow in the ipsilateral internal mammary and vertebral arteries in both patients. Arch aortography is indicated preoperatively in myocardial revascularization patients in the presence of cerebrovascular symptoms, upper extremity blood pressure gradients, and carotid or …

Aneurysms of the thoracic aorta rarely occur in children. We present a female neonate who was referred to our hospital due to a heart murmur associated with cough and fever at 22 days of age. Both the echocardiography and aortography displayed an aneurysm of the ascending aorta at the aortic root. A patent ductus arteriosus (PDA) flow was detected on admission but it was not detectable when she was 3 months old. Neither physical characteristics of Marfan nor Turner syndrome were found, but she has had a huge cutaneous hemangioma over the right trunk since birth. The aneurysm did not progress d…

A case of adrenal cortical carcinoma with inferior vena cava (IVC) involvement is presented. Ultrasonography, computed tomography, and venacavography all presumptively showed a large mass over the upper pole of the left kidney with tumor thrombus in the IVC. However, aortography demonstrated that this mass was receiving its blood supply from the left inferior phrenic artery, aorta, and left renal artery. Radical surgery, including resection of the tumor and its adjacent organs (kidney, distal pancreas, spleen) and the tumor thrombus in the IVC, with the aid of cardiopulmonary bypass, was perfo…

A 62-year-old male patient was admitted with acute dissociation of the descending thoracic aorta and an infrarenal abdominal aortic aneurysm (AAA). Investigation revealed that the thoracic dissection probably had arisen retrogradely in the posterior wall of the AAA and extended superiorly to the left subclavian artery as a blind sac. Implantation of an endoluminal graft device below the renal arteries enabled simultaneous treatment of the AAA and the thoracic aortic dissection. The patient had an uncomplicated recovery. Postoperative aortography and computed tomography demonstrated normal flow…

In a 24-year-old female patient with sudden collapse an acute tricuspid insufficiency was presumed by echocardiography after the exclusion of pulmonary embolism. Aortography, however, revealed an aneurysm of the right sinus of Valsalva which had ruptured into the right atrium. Despite emergency surgery the patient died in protracted cardiac shock. Biopsy and autopsy showed pathognomonic findings of a congenital aneurysm (medial degeneration, reduction of elastic lamellae). Aneurysms of the sinuses of Valsalva are mainly caused by a rare connatal defect of the aortic wall; the right aortic sinu…

We report a successful surgical treatment of acute dissecting aneurysm of the ascending aorta associated with annuloaortic ectasia. The patient was a 29-year-old male, who was admitted to the hospital 5 hours after the onset of anterior chest pain. Aortography revealed acute dissection of the ascending aorta (DeBakery type I) with AAE. The intimal tear surrounding the right coronary ostium was found. The procedure between the right coronary artery and aorta (replaced artificial graft) using saphenous vein graft was also required in addition to Bentall's operation. Postoperative studies reveale…

Stenosis of the coeliac artery by compression occurs predominantly in youngish patients and three times more frequently in women than men. It gives rise to abdominal symptoms, due to chronic disturbance of intestinal arterial supply. In eight patients, operated on for this condition, the characteristic symptoms were of abdominal pain, epigastric vascular sounds, loss of weight, nausea and vomiting. Diagnosis could only be made by angiography of the coeliac and (or) mesenteric arteries, and lateral aortography. Analysis of 240 cases reported in the literature indicates that the condition is cau…

A 16-year-old girl with Loeys-Dietz syndrome presented with an acute, complicated type B aortic dissection (AD) with mesenteric and right renal malperfusion owing to a dynamic obstruction. The anatomy of her AD and her genetic aortography were suboptimal for thoracic endovascular aortic repair. Given the concern for anticipated late aortic degeneration and the need for open aortic repair, she underwent successful transfemoral endovascular septal fenestration with stenting of the fenestration into the superior mesenteric artery and additional stenting of the right renal artery. Her renal failur…

Congenital quadricuspid aortic valve is rare, which may be a cause of sever aortic regurgitation. We report a case of a 55-year-old man who had severe aortic regurgitation with congenital quadricuspid aortic valve. Preoperative aortography showed severe aortic regurgitation. Preoperative trans-esophageal echocardiography revealed abnormal quadricuspid aortic valve. We performed minimal invasive aortic valve replacement with SJM # 21 mm HP prosthetic valve through the limited upper sternotomy. Skin incision was 9 cm. Aortic cross-clamping time was 92 min, cardiopulmonary bypass time was 108 min…

A case of a 55-year-old female with tuberculous false aneurysm of the descending aorta is reported. She was admitted to our hospital on February, 1989 with hemoptysis and lumbago. Active pulmonary tuberculosis, caries of the 1st and 2nd abdominal vertebrae, and abnormal left lower mediastinal tumor shadow were confirmed by roentgenogram. Aortography and computer tomography revealed a false aneurysm of the descending aorta. After anti-tuberculous chemotherapy, fist sized false aneurysm was resected and the punch out hole of the descending aorta, 8 mm in diameter was closed successfully using pa…

A three-year-old boy with unusually flattened facies, multiple joint dislocations and eye abnormalities suggesting the presence of Larsen syndrome presented with a broad mediastinum on routine chest radiography. Computed tomography revealed a huge dilated tortuous aortic arch. Transthoracic echocardiography demonstrated aortic valve dilation, aneurysmal dilation of the ascending aorta with a high aortic arch and a kink in the arch at the isthmus level. Aortography showed a dilated elongated aorta with two acute curves, one at the ductal level and the second at the diaphragmatic level. These ao…
