Optical correction of aphakia following unilateral infantile cataract removal.

PURPOSE: To compare primary intra-ocular lens (IOL) implantation with use of press-on spherical lens for aphakia correction following unilateral infantile cataract removal. METHODS: Sixty infants with a unilateral cataract underwent cataract surgery and were randomly assigned to the IOL (A group) or no IOL group (B group). Residual refractive error was corrected with spectacles in the A group and a press-on spherical lens was used to treat aphakia in the B group. Grating visual acuity (VA) was measured and patients were followed for up to 1 year. RESULTS: LogMAR VA in the operated eyes of bo…

[Aphakia correction with rigid contact lenses in congenital cataract].

PURPOSE: To describe follow-up and evaluate functional results of rigid gas-permeable contact lens wear for the correction of aphakia in infants after surgery for congenital cataract. PATIENTS AND METHODS: This retrospective study was performed on 23 eyes of 17 children (12 boys and five girls), between three days and 36 months of age (median 3.5 months), undergoing cataract extraction without primary intraocular lens implantation and fitted only with Menicon(®) Z material contact lenses. Initial fitting visits included history and family education. Subsequent examinations were performed at e…

Visual outcomes following Artisan aphakia iris claw lens implantation.

OBJECTIVE: To assess the visual outcome of eyes undergoing Artisan lens implantation for aphakia, to identify reasons for poor outcomes, and to report incidences of post-operative uveitis, glaucoma, or hyphaema. METHODS: A retrospective analysis of 32 eyes rendered aphakic through various causes, which subsequently underwent Artisan lens implantation. RESULTS: A total of 32 eyes from 31 patients were reviewed. Follow-up ranged from 6 to 42 months, mean 17.3 months. The commonest cause of aphakia was following complicated phacoemulsification in 17 of the 32 eyes (53.1%). At final follow-up, 2…

Glaucoma in aphakia and pseudophakia after congenital cataract surgery.

Glaucoma is one of the most common causes of visual loss despite successful congenital cataract surgery. The overall incidence does not appe … re any suspicious findings. Unlike congenital glaucoma, the first line of treatment for glaucoma in aphakia/pseudophakia may be medical. Traditional trabeculectomy in paediatric glaucoma following congenital cataract surgery has met with limited success. The addition of antimetabolites to trabeculectomy is known to inhibit fibrosis and enhance the success …

Congenital aphakia in Peters' anomaly syndrome. A case report.

The authors report a case of congenital bilateral corneal opacities in which one of the eyes was enucleated because of malignant glaucoma and corneal perforation. Corneal defects and iridocorneal adhesion were found, but aphakia was the major pathologic ocular finding. The clinical picture and pathology study indicated this case as a Peters' anomaly presenting congenital aphakia.

Intracorneal correction of aphakia: paraxial approach at null incidence.

The technique of intracorneal implantation, recently proposed by de Laage de Meux and Cinquin [Implants, 2, 26 (1984)] for the correction of aphakia, is analyzed in terms of geometrical optics. The analysis is done by using matrix optics and by assuming a null incidence. The characteristics (diameter, radii of curvature, axial thickness) of the hard or soft lens to be implanted are determined and given. The aniseikonia of such lenses is shown to be nearly the same as that of contact lenses.

The correction of aphakia. XXXVI Edward Jackson Memorial Lecture.

Major advances in the correction of aphakia have been made in the past few years. Extended-wear contact lenses constitute one advance, if not a perfect one. Intraocular lenses are now safer to use, and new research is seeking ways to improve their safety through protection of the corneal endothelium during surgery and stimulation of endothelial healing where damage does occur. Altering the refractive power of the cornea offers another possible approach to the correction of aphakia. Modifications of Barraquer's keratomileusis procedure using preserved, preground corneas may have definite indica…

[Fusion and aniseikonic problems in unilateral aphakia (author's transl)].

In 67 unilateral aphakic patients corrected with three different optic systems (conventional contact lens, spheric overcorrection and intraocular lens) aniseikonia and binocular vision are tested. Conventional contact lens leads to 7% aniseikonia and to fusion in only half of the cases. Patients with the two other systems show less than 3% aniseikonia and fusion in most cases. With each system only 10% of the patients reach normal stereoscopic vision. The poor results in unilateral aphakia corrected by conventional contact lens are pointed out.

Secondary lens implantation in aphakia: visual results and complications.

We reviewed 70 cases that had undergone secondary lens implantation. The lenses used included 34 Medallion and 36 Choyce Mark VIII lenses. A 20/40 or better vision was present in 74.3% of the cases before implantation, and could be achieved postoperatively in 95.7% of the cases. Corneal changes, i.e. endothelial corneal dystrophy and induced astigmatism, were not noted in cases where Choyce Mark VIII lenses were used. Cystoid macular edema occurred in three cases where surgery followed an intracapsular cataract extraction (5.3%).

Refractive growth variability in the Infant Aphakia Treatment Study.

PURPOSE: Prediction of refraction after cataract surgery in children is limited by the variance in rate of refractive growth (RRG3). This study compared RRG3 in aphakic and pseudophakic eyes with their fellow, normal eyes in the Infant Aphakia Treatment Study. SETTING: Twelve clinical sites in the United States. DESIGN: Randomized clinical trial. METHODS: Infants randomized to unilateral cataract extraction had RRG3 calculated based on biometric data (axial length and keratometry) at cataract surgery and at 10 years of age, for both the normal and cataract eyes. Subjects were included if co…
