The question of priority in describing 'Dupuytren's disease' as a result of permanent retraction of the palmar aponeurosis has been debated for a long time. This article tries to identify the pioneers who were involved in the research of the origin of permanent contracture of fingers and palm of the hand, and to find out to whom the priority of this affection should be attributed. Selected passages of several authors, as well as of Dupuytren's writings (mainly in italics and translated into English) are used to try and find the truth concerning the primary description of the disease.

Cleft palate (with or without cleft lip) occurs in about 1: 750-1: 2000 births in different societies in the world. Cleft palate individuals have a greater incidence of hearing loss than the general population. The primary cause of the ear problem in cleft patients is eustachian tube dysfunction. The cause for which is abnormal insertion of levator veli palatini and tensor veli palatini muscles into the posterior margin of the hard palate and the palatal aponeurosis and associated muscular hypoplasia.

From the anatomical and physiological point of view th eyelid may be divided into three layers: a deep layer, comprising the tarsi and the canthal tendons with the levator aponeurosis and the lower lid retractors, the orbicularis muscle layer, and the skin. The surgical particularities of each of these layers are outlined. In the management of eyelid injuries the meticulous reconstruction of the medial canthal tendon and canthus is of paramount importance. Only after this are the lids closed from medially to laterally and from deep to superficial tissues.

The forehead is the protective helmet of the brain, if we refer to the etymology of the word calvaria. Destroyed, it must be rebuilt; priority is given to its bone infrastructure, even though we can live without it, as long as the other tissues (skin, muscle, aponeurosis, meninge) are intact. Complex surgical project which will be mentioned when the multiplicity of techniques described demonstrates their imperfection and until now the best restoration of a missing tissue requires its replacement by the same tissue. Invitation to invent a true regenerative surgery.

During the educational dissection of a 69-year-old Chinese male cadaver, an extensor digiti minimi (EDM) with five slips on the right hand was discovered. Except for the two slips of the little finger, the two radial slips were inserted into the dorsal aponeurosis of the middle finger and the ring finger, respectively. The middle slip was connected to the junctura tendinum in the fourth intermetacarpal spaces. Variations in this region are of paramount importance for the reconstructive surgeons, who may utilize the accessory slips to restore functional capacity of the fingers.

Inguinal hernia sometimes surprises surgeons with its unexpected content. Epiploic appendagitis in hernia sac is a very rare entity. We report a 60-year-old male patient with a painless inguinal mass. Surgical exploration showed a 4-cm mass beneath the external oblique aponeurosis that consisted of a hernia sac containing an inflamed and remarkably swollen appendix epiploica of the sigmoid colon secondary to torsion. The patient recovered after the resection of epiploic appendix and a tension-free hernia repair.

There have been numerous reports on dislocation of the extensor tendons in the fingers. We describe an acquired dislocation of the extensor tendons of the thumb. We report a case with posttraumatic dislocation of extensor pollicis brevis (EPB) over the metacarpophalangeal (MCP) joint of the thumb. Ulnar dislocation of the EPB took place during a firm grip on a badminton racket and the patient could not extend the thumb from this position. After reconstruction of the extensor aponeurosis to centralize the EPB tendon the patient recovered normal function of thumb extensors.

Pseudarthrosis in the hand are defined as a non-union of a fracture after 6 month. Reasons might be biological disorders, mechanical instability or infection after fracture treatment. Pseudoarthrotic bone demands stable osteosynthesis and often bone grafting. The presented case is about a dislocated osseous fragment attached to the Aponeurosis dorsalis mimicking clinical and radiological a Pseudarthrosis of the proximal phalanx of the thumb. We would like to draw attention that osseous fragments may present very similarly to a pseudoarthrosis of a phalanx.

We describe an operation to relieve compression of the lateral antebrachial cutaneous nerve at the elbow. Between 1987 and 1997 we operated on seven patients, one with bilateral compression. In two the compression was associated with injury to biceps. A longitudinal or a transverse incision was carried out and the nerve was released from the deep fascia. Partial excision of the biceps aponeurosis was undertaken in the patients who did not have injury to biceps; some additional procedures were required for those patients with injuries. All patients had symptomatic relief.

Ptosis may develop after cataract surgery because of a dehiscence of the levator aponeurosis. A series of patients undergoing entropion repair was examined in order to determine the mechanism of entropion and the correlation with cataract surgery. It is suggested that involutional senile entropion may develop related to cataract surgery, on the same basis as ptosis, due to disinsertion of the capsulopalpebral fascia. It is suggested that the cataract surgeon should examine the patient closely for preoperative entropion to prevent or anticipate the development of frank entropion after the catar…

A 70-year-old man underwent oral mucous membrane grafts to the right eye for presumed trachoma at the ages 26 and 32 years. A superior limbic mass within the site of the graft was found in 1983; the mass was composed of mature sebaceous glands compatible with the lesions called Fordyce nodules when located in the buccal mucosa. Recession of the levator aponeurosis and partial excision of the mass restored the upper eyelid's normal position, and visual acuity in this eye improved from 20/300 to 20/40.

In many aging individuals, dermatochalasis and involutional ptosis appear together. Therefore, for functional and aesthetic purposes, ptosis correction and upper blepharoplasty are performed together. The aim of this article is to investigate factors that should be considered in order to achieve good results when simultaneously performing involutional ptosis correction and upper blepharoplasty in aging patients. Involutional ptosis is usually corrected through aponeurosis advancement in mild cases. In moderate or severe ptosis, the Muller muscle and aponeurosis are used together to correct pto…

OBJECTIVE: Although the mixed orbicularis oculi muscle lacks the muscle spindles required to induce reflex contraction of its slow-twitch fibers, the mechanoreceptors in Müller's muscle function as extrinsic mechanoreceptors to induce reflex contraction. We hypothesize that strong stretching of these mechanoreceptors increases reflex contraction of the orbicularis oculi slow-twitch muscle fibers, resulting in blepharospasm.
METHODS: We examined a 71-year-old man with right blepharospasm and bilateral aponeurosis-disinserted blepharoptosis to determine whether the patient's blepharospasm was w…

We assessed the medium-term outcome of three methods of isolated calf lengthening in cerebral palsy by clinical examination, observational gait analysis and, where appropriate, instrumented gait analysis. The procedures used were percutaneous lengthening of tendo Achillis, open Z-lengthening of tendo Achillis and lengthening of the gastrosoleus aponeurosis (Baker's procedure). We reviewed 195 procedures in 134 children; 45 had hemiplegia, 65 diplegia and 24 quadriplegia. We established the incidence of calcaneus and recurrent equinus and identified 'at-risk' groups for each. At follow-up, 42% …

OBJECTIVE: To identify sonographically the changes of the forefoot in patients with metatarsalgia.
METHODS: Sonography of the foot was performed in 112 patients with metatarsalgia and in 50 healthy controls. Metatarsophalangeal (MTP) joints, intermetatarsal web spaces, flexor and extensor tendons, and plantar aponeurosis were examined.
RESULTS: Sonography showed intermetatarsophalangeal bursitis in 20.5% of cases, Morton's neuroma in 15.2%, and effusion of MTP joints in 11.7%.
CONCLUSION: Sonography gives useful information about the possible alteration responsible for metatarsalgia.

A case of irreducible complete dorsoulnar dislocation of the proximal phalanx of the thumb is presented. The pathologic anatomy included intersubstance tears of the ulnar collateral ligament and dorsal capsule and avulsions of the palmar plate and radial collateral ligament from their metacarpal attachments. The palmar plate blocked reduction, as did the radial collateral ligament that was caught on the margin of a tear in the adductor aponeurosis and on the tendon of the extensor pollicis longus. A review of the English-language literature indicates that this pathologic anatomy has not been d…

What is the central question of this study? Do tendon and/or muscle-tendon unit stiffness influence rate of torque development? What is the main finding and its importance? In our experimental conditions, some measures of relative (to maximal voluntary torque and tissue length) muscle-tendon unit stiffness had small correlations with voluntary/evoked rate of torque development over matching torque increments. However, absolute and relative tendon stiffness were unrelated to voluntary and evoked rate of torque development. Therefore, the muscle aponeurosis but not free tendon influences the rel…

Injury to the central slip of the extensor tendon may occur with open and also with closed injuries, such as volar dislocation of the proximal interphalangeal (PIP) joint. For adequate treatment, it is necessary to identify all injured structures. Without appropriate primary management, the patient is likely to develop a subacute to chronic posttraumatic boutonnière deformity. A fixed boutonnière deformity requires recovery of joint mobility. Once joint mobility is achieved, secondary surgical reconstruction of the central slip can be performed with a tendon transfer or a tendon transplant.

BACKGROUND: Ptosis and upper eyelid depression were previously diagnosed as 2 separate diseases. The authors found that in patients with aponeurotic ptosis accompanied by upper eyelid depression, the main cause of upper eyelid depression was impaired dynamic transmission of the levator aponeurosis.
OBJECTIVE: To analyze the causes of upper eyelid depression in patients with aponeurotic ptosis accompanied by upper eyelid depression and to introduce the best treatment methods.
METHODS: The authors enrolled 15 eyes (11 patients) with aponeurotic ptosis accompanied by upper eyelid depression fro…
