Spinal extradural arachnoid cysts (ACs) have an infrequent predilection for the sacrum. As with their counterparts in other regions of the spine, cysts in this location are mostly asymptomatic. Common presentations in symptomatic cases include pain in the low back or perineum, radiculopathy, and sphincteric dysfunction. The authors report a hitherto undescribed presentation in which the predominant symptoms are those related to an associated holocord syrinx. This 15-year-old boy presented with fluctuating, spastic paraparesis and a dissociated sensory loss in the trunk. Admission MR imaging of

Arachnoid cysts are the most common congenital cystic lesions in the brain occurring in the middle fossa, suprasellar region and occasionally in the posterior fossa. Conventionally all cysts are considered as benign and symptoms are attributed to expansion of cysts causing compression of adjacent neurological structures, bleeds within the cyst or due to the development of acute hydrocephalus. We are reporting this case of a 15-year-old female patient with non-progressive weakness in the limbs since the age of seven years who presented with acute onset syncopal attacks and respiratory failure.

OBJECT: Arachnoid cysts are commonly encountered fluid collections in the central nervous system. Recent advances in neuroimaging have translated into an increased detection of these cysts; that is, diagnoses can be made more frequently at earlier stages. Significant advances have also been made in the surgical management of these lesions. The authors report on a case series that illustrates the diverse forms of presentation and the treatment modalities commonly used for arachnoid cysts.
METHODS: Between January 2002 and December 2006, 20 patients with arachnoid cysts underwent surgery perfor

OBJECT: Neuroendoscopy is increasingly used in neurosurgery. The authors report their evolving experience in the management of arachnoid cysts using endoscopic techniques and, more recently, the use of these techniques in combination with neuronavigation systems. The aim of this study was to assess the efficacy of this approach and the factors influencing the final outcome of treatment.
METHODS: The authors reviewed 39 cases in which patients were treated endoscopically for intracranial arachnoid cysts over a period of 8 years. The indications and techniques used were reviewed and the surgica

OBJECTIVE: Biochemical and radiological properties of sellar and parasellar lesions are quite similar in some instances. This leads to a difficulty in preoperative diagnosis. Here, a pituitary adenoma and a suprasellar arachnoid cyst in the same patient is presented, and possible etiopathogenetic mechanisms and surgical treatment are discussed.
CASE: A 56-year-old male patient was admitted to the hospital with a history of seizures, urinary incontinence and visual disturbances. Preoperative MRI revealed a mass lesion in the sella turcica with suprasellar extension and a coexisting large supra

Arachnoid cysts may be intracranial or extracranial in the neuraxis and may present with headache and neurological signs or acutely with rupture and its complications. We report a case of spinal anaesthesia for elective caesarean section in a woman with an intracranial arachnoid cyst of the posterior fossa. We are unaware of any other such case reports published in the English language.

A 68-year-old woman presented with a rare huge right occipital cystic lesion manifesting as progressive left hemiplegia. Cranial computed tomography revealed a 4 x 7 cm right occipital lobe thin-walled cyst with midline shift and compression of the ipsilateral ventricle. The patient underwent a single burr hole operation for cystography and delineation of the cyst anatomy, then a separate right parieto-occipital craniotomy with complete cyst evacuation, corticotomy, and ventriculostomy. The presumptive diagnosis was arachnoid cyst. The symptoms had completely resolved by 4 months after surgery

BACKGROUND: It is not difficult to find localized skull ballooning or macrocrania in patients with intracranial arachnoid cysts. However, there have been no previous reports regarding large localized skull protuberant deformities resembling a war helmet. The authors report with a review of literature a case of an adult with helmetlike skull deformity resulting from a large supratentorial arachnoid cyst.
CASE DESCRIPTION: A 35-year-old man presented with a large head deformity since his early childhood that had been the result of gradual progression from infantile macrocrania. He also had ment

We present a case of a symptomatic frontal arachnoid cyst associated with hydrocephalus in an elderly patient. It is well known that symptomatic arachnoid cysts usually develop at an early age. This condition is rarely observed in the elderly. The authors hypothesized that compression of the brain and obstructive hydrocephalus caused the arachnoid cyst to become symptomatic.

BACKGROUND: The frequent association between optic nerve hypoplasia (ONH) and other central nervous system abnormalities has been widely reported in the literature. Occasional reference has been made to arachnoid cyst as one of the associated findings.
METHOD: The charts and neuroradiologic findings of the 40 patients with ONH seen in our department as well as the Visually Impaired Program during the past 8 years were reviewed.
RESULTS: An intracranial arachnoid cyst was present in 5 of the 40 patients (12.5%) with ONH and visual impairment. These patients' clinical and neuroradiologic findi

OBJECT: The optimal management of children with middle fossa arachnoid cysts (MFACs) remains controversial. In this study the authors evaluated the relationship between two preoperative variables, hydrocephalus and nonspecific macrocephaly, in children undergoing fenestration of temporal arachnoid cysts and hydrocephalus-related shunt placement.
METHODS: During a 16-year period, 40 children (30 boys and 10 girls) underwent treatment of MFACs. All but one patient experienced either worsening symptoms or progressive serial imaging-documented cyst enlargement. Hydrocephalus was present in six pa

OBJECTIVE: Spontaneous cerebrospinal fluid (CSF) leaks from the fallopian canal are exceedingly rare, with only 6 reports appearing in the world literature. We report a seventh case that is unique in that it involves an arachnoid cyst of the fallopian canal and an associated facial nerve palsy.
STUDY DESIGN: Case report.
SETTING: International tertiary care referral center.
CONCLUSION: CSF otorhinorrhea associated with a facial nerve palsy may be indicative of an arachnoid cyst of the fallopian canal. These fistula are extremely rare. Surgical management involves sealing the fistula while p

BACKGROUND: Nodules of cells showing a striking histological similarity to those of arachnoid villi have previously been found closely adjacent to pulmonary venules in several diseases associated with alveolar hypoxia or pulmonary oedema including mitral stenosis, plexogenic pulmonary arteriopathy, pulmonary thromboembolism, and chronic obstructive pulmonary disease.
METHODS: Histological sections of the lungs of seven adult native highlanders from La Paz (3600 m) were examined.
RESULTS: Arachnoid nodules were found in the lungs of one Aymara and one Mestizo Indian.
CONCLUSIONS: These bodie

Three operated cases of the interhemispheric arachnoid cyst were reported. Case 1: a 58-year-old female suffering from progressive right hemi-rigidity and gait disturbance for the past two years. Case 2: a 66-year-old female was admitted with chronic headache. MRI demonstrated a large interhemispheric cyst in these two patients. Case 3: a 6-month-old male had frequent episodes of tonic seizure. MRI demonstrated interhemispheric cyst and agenesis of the anterior part of corpus callosum. The resection of the cystic wall via the interhemispheric approach was performed for all cases. General convu

The authors report a series of three children with symptomatic congenital arachnoid cyst of the lateral ventricles. Presenting symptoms consisted of macrocephally, delay in psychomotor development, and seizures. CT findings were of a well-defined cystic lesion placed in the atrium of the lateral ventricle. One child was treated by direct cyst exposure and cysto-peritoneal shunt. The other two were treated with ventriculo-cysto-peritoneal shunts; in one of these, we used a ventriculoscope both for cyst fenestration and for accurate shunt placement. The origin of intraventricular arachnoid cysts

The diagnosis and evaluation of temporal bone arachnoid granulations (AGs) is based on characteristic bone defects in the posterior or middle fossa surfaces of the temporal bone in adult patients with persistent serous otitis media or septic meningitis following acute otitis media. The cerebrospinal communication caused by AGs should be repaired by a middle fossa craniotomy (middle fossa AGs) or an intact canal wall mastoidectomy (posterior fossa AGs) to preserve normal sound transmission. In adult patients with chronic mastoiditis, the dural defect of an AG may permit intracranial extension o

Intracranial arachnoid cysts are usually non-symptomatic. Intracystic and subdural haematomas induced by even minor head injury may turn an asymptomatic AC into a symptomatic one, necessitating surgical treatment. We present a case of a previous asymptomatic AC, spontaneously complicated with subdural hygroma and development of intracystic and subdural haematoma. Clinical follow-up and control CT regime of patients with AC are recommended.

Supratentorial extra-axial arachnoidal cysts are congenital benign formations of intracranial location filled with cerebrospinal fluid. The clinical symptomatology of arachnoidal cysts depended on the age of the patient and presence of complications. The pathogenetically justified surgical method of treatment of arachnoidal cysts in children is excision of cyst wall, emptying of its cavity, creation of shunt with the subarachnoidal spaces and basal cisternae in association with cystoperitoneostomy. In the adult patients additional cystoperitoneotomy is not performed.
