Fulminant systemic necrotizing arteritis: CT findings.

Fulminant necrotizing arteritis can be a rapidly fatal disease with protean manifestations often suggesting other diagnoses. We present two cases with angiographic and CT correlation and discuss CT findings that are suggestive of the diagnosis.

Takayasu's arteritis and juvenile rheumatoid arthritis.

We describe a patient with the onset of polyarticular juvenile rheumatoid arthritis at age 7; she developed Takayasu's arteritis 12 years later. Her arthritis had been inactive until 15 months before the diagnosis of Takayasu's arteritis. The association of arthritis and Takayasu's arteritis is discussed.

[Intracranial arteritis with circulating immune complexes].

Thirty-eight patients below the age of 50 years were investigated and in twelve, circulating immune complexes were found. Eight of these had angiographic evidence of an arteritis of the intracranial vessels. The causative factors, the role of the circulating immune complexes and the angiographic appearances in intracranial arteritis are discussed.

Tonic pupil with giant cell arteritis.

A patient with giant cell arteritis and ischaemic optic neuropathy developed bilateral tonic pupils. This pupillary abnormality may result from ischaemia of the ciliary ganglia. Despite the propensity for patients with giant cell arteritis to develop vascular occlusions in the eye and orbit, tonic pupils have rarely been described, and several reasons for this are proposed. The arteries supplying the ciliary ganglia are frequently involved in giant cell arteritis, but their anastomotic pattern may confer protection from ischaemia. Tonic pupils may also be overlooked amidst the more dramatic ma…

Giant cell arteritis and its variants.

In addition to cranial arteritis, the three other variants of giant cell arteritis may also lead to neurological symptoms: polymyalgia rheumatica, occult giant cell arteritis, and inflammatory aortic arch syndrome of elderly persons. This survey discusses the nosological, clinical, diagnostic, and therapeutic aspects of giant cell arteritis and its subgroups on the basis of experience with over 100 cases.

Giant cell arteritis with pulmonary nodules.

A patient presenting with polymyalgia rheumatica and biopsy-proved temporal arteritis showed response to corticosteroid therapy. Nodular pulmonary lesions developed twice, and diplopia and unilateral visual loss once, after reduction of the steroid dose. Response to reinstitution of full-dose steroids was dramatic. The clinical and pathologic findings in this patient suggest that the pulmonary lesions represented a previously undescribed manifestation of giant cell arteritis, but the possibility of limited Wegener's granulomatosis involving the temporal artery could not be ruled out.

[Retinal arteritis in Liebow's lymphomatoid granulomatosis].

A case is reported of central retinal artery obstruction appearing during the course of a Liebow lymphomatoid granulomatosis. Extrapulmonary manifestations of this necrotizing angiitis, and the differential diagnosis with Wegener's granulomatosis are discussed.

Rheumatoid arthritis with systemic necrotizing arteritis.

An autopsy case of rheumatoid arthritis with active polyarthritis, systemic necrotizing arteritis, pleuritis, pericarditis, rheumatoid nodules in a few organs and a healing gastric ulcer was reported. Histologically, systemic necrotizing arteritis was characterized by vascular changes of the following three types: Granulomatous arteritis with a characteristic arrangement of mesenchymal cells forming a palisade around coagulation necrosis of media and some of them formed a rheumatoid nodule-like lesion in the wall (RA type); Fibrinoid arteritis very similar to the Kussmaul-Maier type periarteri…

Cerebral arteritis associated with heroin abuse.

A twenty-year-old man suffered a left homonymous hemianopia after self-administration of heroin. Investigations revealed right occipital infarction with angiographic changes of arteritis in the posterior cerebral arteries.

Ophthalmic complications of giant cell arteritis.

Temporal arteritis, or giant cell arteritis, is a common cause of blindness among older age groups. Classical and occult forms are described with particular reference to ocular symptomatology. Although the disease is self-limiting and usually subsides spontaneously within a year, prompt diagnosis and treatment are required if second eye involvement and blindness are to be prevented. Erythrocyte sedimentation rate is of major significance to diagnosis, but biopsy is necessary to prove or disprove the existence of temporal arteritis. Pathology, etiology, association with other disorders and labo…

Giant cell arteritis with visceral angiitis.

We describe a case in which giant cell arteritis coexisted with microscopic polyarteritis nodosa and focal-local glomerulonephritis. We also review previous cases of renal abnormalities in giant cell arteritis. We believe that this association of inflammatory renal disease and giant cell arteritis has not been documented in the past.

[Takayasu-Onishi arteritis. Current etiopathogenetic aspects].

By Takayasu-Onishi's arteritis is meant an arteritic process with unknown aetiology which electively affects young women, seemingly of prevalently Asiatic stock. The disease concerns almost exclusively large elastic arteries and presents clinically with early preocclusive symptomatology followed, after a varying period, by a picture of obstructive angiopathy. Here, the most recent aetiopathogenetic findings are considered. The disease's predilection for the young female, together with certain clinical and experimental observations, suggest that a dysendocrine condition may have some pathogenet…

Carpal tunnel syndrome in temporal arteritis.

An 88 year old man presented with pain in the hands and wrists, associated with electrodiagnostic studies compatible with a carpal tunnel syndrome in the right wrist. Approximately seven months later he developed manifestations of bilateral temporal arteritis resulting in monocular blindness. Electrodiagnostic studies at this time confirmed the presence of a more severe carpal tunnel syndrome, now present bilaterally. We have been unable to find other case reports of carpal tunnel syndrome related to temporal arteritis. This association may be recognized more frequently if electrodiagnostic st…
