[Sonographic findings for synovial fluid].

OBJECTIVE: The aim of this pictorial essay was to evaluate the sonographic features of synovial fluid in patients with arthritis. METHODS: Sixty-nine patients with active synovitis (rheumatoid arthritis, psoriatic arthritis, osteoarthritis, septic arthritis, crystal arthropathies, post-traumatic arthritis) were studied. Sonographic evaluation was performed with a AU-5 Harmonic, Esaote Biomedica (Genoa, Italy) equipped with a 10-14 MHz broadband linear transducer and a Diasus Dynamic Imaging Ltd.(Livingston, Scotland UK) equipped with a 8-16 MHz broadband linear transducer. RESULTS: Six main

Arthritis as a presenting feature of non-Hodgkin's lymphoma.

Leukaemia can present with joint swelling in the absence of abnormal haematological findings. Arthritis as a presenting sign of lymphoma, however, is extremely rare. Three children with non-Hodgkin's lymphoma who had joint swelling at the onset of their disease are reported. Two cases showed histological features of anaplastic large cell lymphoma (Ki-1/CD30 positive), and one of angioimmunoblastic T cell lymphoma. In all patients the unusual presentation delayed correct diagnosis.

Contrast-enhanced MRI features in the early diagnosis of Juvenile Idiopathic Arthritis.

OBJECTIVES: To determine whether clinical, laboratory or Magnetic Resonance Imaging (MRI) measures differentiate Juvenile Idiopathic Arthritis (JIA) from other forms of active childhood arthritis. MATERIALS AND METHODS: We prospectively collected data of 80 treatment-naïve patients clinically suspected of JIA with active non-infectious arthritis of (at least) one knee for <12 months duration. Upon presentation patients underwent clinical and laboratory assessments and contrast-enhanced MRI. MRI was not used as a diagnostic criterion. RESULTS: Forty-four (55%) patients were clinically diagnos

[Differential diagnosis of monoarthritis].

Making the diagnosis of monoarthritis can be quite difficult, because in contrast with polyarthritis just a few clinical data is available and wide diagnostic spectrum is present. The diagnosis of inflammatory rheumatic disease is usually in responsibility of a rheumatologist, but we know from experience that is necessary to carry out the basal differential diagnostic assessment as soon as possible to begin the optimal therapy. General practitioners and orthopaedists are usually first to face this problem.Monoarthritis can be divided into non-inflammatory arthritis where activated osteoarthriti

Reactive arthritis following COVID-19: cause for concern.

Low-quality evidence suggests that COVID-19 may trigger reactive arthritis one to four weeks after the infection. Post COVID-19 reactive arthritis resolves within a few days, and no additional treatment is required. Established diagnostic or classification criteria for reactive arthritis are missing, and a deeper understanding of the immune mechanism related to COVID-19 prompt us to further investigate the immunopathogenic mechanisms capable of promoting or contrasting the development of specific rheumatic diseases. Caution should be exerted when managing post-infectious COVID-19 patient with

Arthropod-borne arthritides.

Infections with several types of viral and bacterial pathogens are able to cause arthritic disease. Arthropod vectors such as ticks and mosquitoes transmit a number of these arthritis-causing pathogens, and as these vectors increase their global distribution, so too do the diseases they spread. The typical clinical manifestations of infectious arthritis are often similar in presentation to rheumatoid arthritis. Hence, care needs to be taken in the diagnoses and management of these conditions. Additionally, clinical reports suggest that prolonged arthropathies may result from infection, highlig

Lesson of the month 1: Septic arthritis with normal acute phase reactants and white cell count in a patient receiving tocilizumab.

Septic arthritis represents 8-27% of cases of monoarthritis presenting to the emergency department. Tocilizumab is an interleukin-6 blocking monoclonal antibody with the mechanistic potential to interfere with the body's normal inflammatory response to an infectious insult. We present a case of septic arthritis with a normal white cell count, C-reactive protein, erythrocyte sedimentation rate and fibrinogen in a patient treated with tocilizumab.

Current standardized therapeutic approach for uveitis in Japan.

Uveitis is an ocular disease associated with systemic immune-mediated diseases such as rheumatoid arthritis, inflammatory bowel disease and ankylosing spondylitis; and infectious diseases. Infectious uveitis occasionally shows symptoms similar to those of non-infectious uveitis. Therefore, distinguishing between non-infectious and infectious uveitis is critical for definitive diagnosis and appropriate choice of treatment. Once the cause of infection is known, treatment can be promptly initiated. However, in contrast to infectious uveitis, non-infectious uveitis is more difficult to diagnose cl

Monarthric gonococcal arthritis involving the calcaneocuboid joint.

Gonococcal arthritis is a frequently occurring clinical entity that should be included routinely in a differential diagnosis of pedal joint pain. Unfortunately, the lack of specificity in the presentation makes gonococcal arthritis difficult to diagnose. Indices of suspicion should rise with any sexually active patient, particularly when septic arthritis is suspected without a detectable portal of entry. The authors emphasize again the importance of carefully choosing empiric antibiotic coverage for gonococcal arthritis. Three factors that should be considered are regional epidemiology, the an

Neglected tropical rheumatic diseases.

The complexities of dealing with rheumatic diseases in tropical countries are diverse and likely due to limited health care infrastructure, lack of diagnostic and therapeutic facilities, impact of dominant prevailing diseases, and the challenges of differentiating from infectious and non-infectious disease mimics. Several tropical diseases present with musculoskeletal and rheumatic manifestations and often pose a diagnostic dilemma to rheumatologists. The diagnosis is often delayed or the disease is misdiagnosed, leading to poor patient outcomes. Endemic tropical diseases like tuberculosis and

Rheumatologic Perspective on Persistent Right-Hand Tenosynovitis Secondary to Mycobacterium marinum Infection.

BACKGROUND: Rheumatologic conditions share many symptoms with infectious processes. The immunosuppressive therapies used in rheumatology unfavorably impact any underlying infection. Nontuberculous mycobacteria (NTM) are difficult to grow in culture media and may affect the musculoskeletal system, developing manifestations that may imitate rheumatic inflammatory arthritis. For this reason, surgical debridement and biopsy culture are essential in cases where suspicion remains high. CASE PRESENTATION: We present the case of a patient with progressively worsening right-hand tenosynovitis who was

[Lyme arthritis].

Lyme disease is a multi-system inflammatory disease caused by spirochetes of the Borrelia burgdorferi. Among multiplicity manifestations of Lyme disease, musculoskeletal symptoms also occur. In the following article authors present epidemiology of Lyme disease, pathogenesis of antibiotic-refractory Lyme arthritis, symptoms, diagnosis and management of Lyme arthritis based on recent guidelines prepared by an expert panel of the infectious Diseases Society of America.

Molecular detection of bacterial DNA in venereal-associated arthritis.

OBJECTIVE: To evaluate the utility of polymerase chain reaction (PCR) amplification in detecting DNA from venereal-associated microorganisms in the synovial fluid of patients with inflammatory arthritis. METHODS: Oligonucleotide primers were developed for nested PCR based on Chlamydia, Ureaplasma, and Neisseria DNA sequences. PCR products were detected by gel electrophoresis and dot-blot hybridization. Primers specific for the target bacterial DNA were used to search for bacterial DNA in 61 synovial fluid specimens from patients with inflammatory arthritis, including several clinically associ

[Diagnosis of arthritis in black Africa. Apropos of 473 cases in Congo].

A cross-sectional study of arthritis was conducted in the Rheumatology Department of the Brazzaville Teaching Hospital, Congo. A total of 473 patients with arthritis seen between 1989 and 1991 were subjected to the limited tests available. Gout was the leading diagnosis (n = 83). Septic arthritis (n = 82) and infectious discitis (n = 55) were the most common reasons for admission. Tests often failed to identify the causative organism; Staphylococcus was the most commonly recovered organism. Tuberculous discitis was less common than discitis due to pyogenic bacteria. HIV-related arthritis (n =

Mycoplasmal arthritis in man.

Naturally occurring mycoplasmal arthritis in various animals species and the ability to induce mycoplasmal arthritis experimentally have been the main reasons for searching for evidence of mycoplasmal infection in the joints of patients with rheumatoid arthritis. However, reliable reports of the isolation of mycoplasmas from synovial fluids and tissue of such patients do not exist, and measurements of specific antibodies have not proved rewarding. There is some evidence that infection by Mycoplasma pneumoniae occasionally has an arthritic sequela, although the microorganism has not been isolat

Mycobacterium tuberculosis joint infections: A case series.

Tuberculosis is a chronic infectious disease, caused by Mycobacterium tuberculosis, that seriously endangers human health. Skeletal tuberculosis is the most common type of extrapulmonary tuberculosis and tuberculous arthritis is the second most common type of skeletal tuberculosis. We report a case series of patients with tuberculous arthritis, two of whom had no joint disease in the past and presented as monoarthritis. The final patient had a history of rheumatoid arthritis, with polyarthritis that was aggravated during treatment with glucocorticoids and immunosuppressive drugs. This series o

[Diagnostic Work-Up for Monoarthritis - Step by Step].

The work-up of acute monoarthritis is challenging due to the abundance of differential diagnoses. In addition to a bacterial septic arthritis, which can, if not treated promptly, cause rapid irreversible joint damage, many diseases have to be considered: inflammatory rheumatic diseases, activated osteoarthritis, other infectious arthritis, cristal induced arthritis, and rare tumorous diseases. In cases with high urgency, and/or when medical history, physical examination and laboratory parameters remain without a specific etiologic clue, septic arthritis has to be excluded by immediate diagnost

Lyme disease: musculoskeletal manifestations.

A previously unrecognized musculoskeletal syndrome led to the recognition of this "new" infectious disease. Several distinct patterns of musculoskeletal involvement can be seen throughout the course of untreated Lyme disease. Diffuse, nonspecific muscle achiness and stiffness can be seen early to be followed by characteristically brief, recurrent episodes of LIMP and transient arthritis that help to differentiate LD from other arthropathies. Chronic oligoarticular arthritis is seen in a few. Lyme arthropathy most closely resembles the reactive arthropathies. In a few cases, LD has mimicked oth

Rheumatological manifestations of Lyme disease and related tick-borne diseases in north America.

PURPOSE OF REVIEW: Lyme disease is the most common vector-borne infection in North America, and the incidence continues to rise as the geographic range of Ixodes ticks expands. While most patients can be cured with early antibiotic therapy, Lyme arthritis persists as a challenging late manifestation, with a subset developing postantibiotic inflammatory disease that mimics autoimmune arthritis. RECENT FINDINGS: Recent studies have advanced our understanding of Lyme arthritis. Even after bacterial eradication, in some patients, residual antigens in the synovial fluid can sustain immune activati
