Dual atrioventricular nodal pathways, the substrate responsible for atrioventricular node reentry tachycardia (AVNRT), are thought to be randomly occurring congenital anomalies. This article describes 14 patients in six families, each with two or three first-degree relatives with paroxysmal supraventricular tachycardia. Electrophysiological evidence of dual atrioventricular nodal pathways was established in all 13 patients studied, AVNRT was induced in 12 (92%), and radiofrequency ablation of the slow pathway was curative in all cases. The data suggest a hereditary contribution to the developm…

Mitral isthmus block during left lateral accessory pathway ablation has been reported in the past. Here, we document for the first time an irregular atrioventricular reciprocating tachycardia due to alternating mitral isthmus block and mitral isthmus delay, resulting in different atrioventricular node conduction times and tachycardia cycle length.

Atrioventricular (AV) nodal conduction is decremental and very prone to alterations in autonomic tone. Conduction through the His-Purkinje s … n the HPS. Enhanced sympathetic tone and reduced vagal tone can facilitate induction of both AV and atrioventricular node reentry.

Much importance is attached lately to the use of lasers in surgical treatment of tachyarrhythmias. The advantages of the laser radiation are as follows: the possibility of using it on a contracting heart, its exact and selective effect, infliction of even damages in preservation of the structure, reduction of damage to the myocardium as a whole. At the Bakulev Institute of Cardiovascular Surgery, USSR AMS, 52 operations were carried out in the management of supraventricular tachyarrhythmias by laser radiation: 27 operations for laser isolation of the atrioventricular node, 15 operations for la…

We report a case of a 21-year-old female manifesting narrow QRS complex tachycardia and alternating R-R intervals. The fixed RP interval suggested ventriculoatrial conduction via an accessory pathway (AP). The alternating PR intervals indicated anterograde conduction through the fast and slow nodal pathways, respectively. The coexistence of AP and dual atrioventricular nodal pathways is not rare, with most of them exhibiting as atrioventricular reentry tachycardia (AVRT) using the fast or slow nodal pathway exclusively as the anterograde limb. We propose that alternating dual nodal pathway AVR

A 31-year-old woman reported dizziness in the early postpartum period after receiving dexmedetomidine. The ECG was misinterpreted as complete heart block; however, more careful analysis revealed an atypical Wenckebach pattern with dual AV nodal conduction and termination of nonconducted P waves with junctional escape beats. The patient's rhythm returned to sinus after stopping dexmedetomidine. Atypical Wenckebach patterns account for greater than 50% of patients with Mobitz Type I AV block and can be misinterpreted as high-grade AV block. This case highlights the causes of atypical Wenckebach

A 27-year-old white female with a history of paroxysmal supraventricular tachycardia presented to the emergency department complaining of intermittent palpitations. Although no tachydysrhythmia was present, she was noted to have two distinct PR intervals during normal sinus rhythm while in the emergency department. The patient was referred for electrophysiologic study. This study demonstrated dual AV nodal paths, and AV nodal reentrant tachycardia was induced and terminated. She was placed on flecainide for outpatient management of her dysrhythmia. Dual AV nodal pathways leading to AV nodal re

Patients with autoimmune diseases are at increased risk for developing cardiovascular diseases, and abnormal electrocardiographic findings are common. Voltage-gated calcium channels play a major role in the cardiovascular system and regulate cardiac excitability and contractility. Particularly, by virtue of their localization and expression in the heart, calcium channels modulate pace making at the sinus node, conduction at the atrioventricular node and cardiac repolarization in the working myocardium. Consequently, emerging evidence suggests that calcium channels are targets to autoantibodies…

We performed an electrophysiologic study in 40 children with an atrial septal defect and analyzed their pre- and postoperative electrocardio … nce of electrophysiologic abnormalities or arrhythmias. These results indicate that the sinus node, atrioventricular node and atrial myocardium show some degree of dysfunction in patients with an atrial septal defect. An early operation may prevent further progression of electrophysiologic abnormalities and the development of symptomatic arrhythmias.

Programmed transesophageal electrical stimulation was used to examine 733 rural inhabitants aged 16 to 70 years. Conduction dissociation along the atrioventricular node was revealed in 65 (8.8%) of the examinees, out of them 12 had induced paroxysms of atrioventricular nodal tachycardia. Atrioventricular tachycardia involving the accessory anomalous pathways was provoked in 10 subjects. Among them 2 had apparent, 1, latent, and 7, obscure forms of the Wolff-Parkinson-White syndrome. The induced paroxysms of atrioventricular tachycardias were sustained in 45.5% of the cases. Only 50% of the sub…

BACKGROUND: Since 1967, numerous case reports have described fibromuscular alterations of the sinus node artery and/or the atrioventricular node artery as a potential cause of death. However, the prevalence of these changes in a healthy population has only rarely been investigated systematically.
METHODS: The arteries of the cardiac conduction system were studied systematically, by means of routine histology, in 100 cases of victims aged 0 to 40 years with a nonnatural cause of death.
RESULTS: Microscopic alterations were seen in the walls of sinus node arteries in 52 out of 100 cases, in th…

Initial assessment of widecomplex tachycardias (WCTs) should begin with patient history and physical examination. If there is a history of r … ardia. Cardioversion-defibrillation is required in patients with WCT who are in unstable condition. Atrioventricular node blockers are the agents of choice for arrhythmias that are atrioventricular node-dependent, but they may cause hemodynamic collapse if administered to patients with ventricular tachycardia or with atrial fibrillation and preexcitation. Lidocaine hyd …

In humans, accessory pathways (APs) in an anteroseptal and midseptal position are often challenging to ablate because of their close proximity with the conduction pathways of the atrioventricular junction. The use of low-energy ablation techniques can be useful to reduce the risk of permanently damaging the atrioventricular node and the His bundle. This report describes the use of low-energy radiofrequency catheter ablation to successfully and permanently ablate anteroseptal APs in two dogs with orthodromic atrioventricular reciprocating tachycardia. In the first dog, a transient first degree …

Preterm infants often experience side effects from intubation. Desaturation and sinus bradycardia are frequent. Atropine as premedication mitigates these risks. We report the occurrence of severe bradycardia related to atrioventricular block during intubation. The infant experienced severe bradycardia not responsive to effective endotracheal ventilation. The electrocardiogram trace displayed an initial 2:1 Mobitz II block with a rapid progression to complete atrioventricular block. Congenital atrioventricular block is mostly related to atrioventricular node lesions by maternal anti-SSA/anti-SS…
