BACKGROUND: This study compared the clinical and laboratory characteristics of patients with anti-glomerular basement membrane (GBM) disease and normal renal function, with those of patients with anti-GBM disease where there was renal impairment.
METHODS: The medical records of the 14 patients who had presented with anti-GBM disease to our hospital in the past 20 years were reviewed.

A patient with Alport's syndrome and chronic renal failure received a cadaver kidney graft. Four months after the transplantation, he developed glomerulonephritis induced by anti-glomerular basement membrane (GBM) antibody. The graft failed, and the patient returned to haemodialysis. Twenty-two months later the patient received a second cadaver kidney graft. Ten months after the second transplant, he developed a recurrent anti-GBM glomerulonephritis. This observation suggests that anti-GBM disease can recur in a second transplanted kidney in a patient with Alport's syndrome.

This report describes the development of anti-glomerular basement membrane (GBM) glomerulonephritis after kidney transplantation in related males with Alport syndrome. Antibodies in sera from one of these patients stained normal GBM, Bowman's capsule, tubular basement membranes, and epidermal basement membranes but did not stain tissues from an unrelated Alport male. The anionic sites of the basement membrane of rat seminiferous tubules were demonstrated ultrastructurally in the lamina densa by using cationic polyethyleneimine (PEI). The sites were largely digested out after incubation with heparitinase, indicating a large proportion of heparan sulfates. Characteristic ultrastructural alterations of the glomerular basement membrane (GBM) have been reported in hereditary nephritis. However, these GBM changes are not present in all patients with hereditary nephritis. In a retrospective study of 42 children with hereditary nephritis characteristic GBM changes were found in 28. A heparan sulfate proteoglycan (HSPG) synthesized by murine parietal yolk sac (PYS-2) cells has been characterized and purified from culture supernatants. The chest radiographs of 25 patients with proven antiglomerular basement membrane antibody disease (Goodpasture's syndrome) were analysed. All except two of the patients had pulmonary haemorrhage at some stage of their disease. Altogether there were 39 episodes of pulmonary haemorrhage, 25 being relapses.
