Early results with annular support in reconstruction of the bicuspid aortic valve.

OBJECTIVE: Repair of the bicuspid aortic valve may be performed in aortic regurgitation and aneurysm. Dilatation of the atrioventricular junction has been identified as a risk factor for repair failure, and we have used suture annuloplasty to correct atrioventricular junction enlargement. The objective was to compare the early results of aortic repair with and without annuloplasty. METHODS: Between November 1995 and January 12, a total of 559 patients were treated with bicuspid aortic valve repair for predominant regurgitation (n = 389), aortic aneurysm (n = 158), or acute dissection (n = 12)

Right subclavian artery aneurysm in an adolescent with a bicuspid aortic valve.

Bicuspic aortic valve is the most common congenital cardiac anomaly (Fedak et al. 106:900-904, 2002), and it is associated with other structural anomalies of the aorta, such as coarctation, suggesting a common embryologic developmental mechanism (Mergan et al. 104:118-119, 2004). In more than a half of patients, it is associated with progressive dilation and aneurysm formation of the aorta (Warnes 89:965-966, 2003) despite normally functioning bicuspid aortic valves. In this context, aneurysms of the right subclavian artery are extremely rare, and even more so when associated with a right-side

Bicuspid pulmonary valve in transposition of the great arteries: impact on outcome.

OBJECTIVE: Long-term evaluation of the impact of bicuspid pulmonary valve on neoaortic valve regurgitation and aortic root dilatation (ARD) after arterial switch operation (ASO) for transposition of the great arteries (TGA). METHODS: Between January 1987 and March 2010, 980 neonates underwent ASO for TGA. A total of 40 patients (4.0%) had a pulmonary bicuspid valve with no significant left ventricular outflow tract obstruction. In this group, 11 patients (28%) had associated ventricular septal defect, three hypoplastic aortic arch, and three had a right ventricular hypoplasia. No pulmonary va

Repair of the bicuspid aortic valve: a viable alternative to replacement with a bioprosthesis.

OBJECTIVE: We sought to compare the safety and durability of bicuspid aortic valve repair versus replacement with a bioprosthesis. METHODS: We reviewed medical records of patients aged 18 years or older undergoing bicuspid aortic valve repair for aortic regurgitation from 1984 through 2007. We analyzed early outcomes and predictors of aortic valve replacement after initial repair. Patients with repair were compared with an age- and sex-matched cohort who had replacement with a bioprosthesis. Overall survival and survival free from reoperations were compared between groups. RESULTS: The mean

Modification of the Cabrol as a bailout procedure in complicated bicuspid valve aortopathy.

The Cabrol technique is reserved where the conventional 'button' or Bentall techniques fail to maintain a tension-free anastomosis between the coronary arteries and aortic conduit. However, the side-to-side anastomosis of the interposition graft that connects the coronary ostia with the aorta in the Cabrol, may lead to kinking or tension, and subsequent occlusion. We present a case of successful Cabrol modification in a patient with bicuspid aortopathy where the graft to the right coronary artery was anastomosed directly onto the valved conduit and the graft to the left main stem onto the prev

Performance of bicuspidized pulmonary allografts compared with standard trileaflet allografts.

BACKGROUND: Pulmonary allografts are commonly used in the reconstruction of the right ventricular outflow tract. However, in pediatric patients, allografts that are small enough are often unavailable. To downsize larger, more readily available conduits, one leaflet can be excised, forming a bicuspid valve. The aim of this study was to assess the intermediate-term durability of bicuspidized allografts compared with standard allografts. METHODS: Forty-seven patients who received a bicuspidized allograft from 1998 to 2008 were compared with 51 patients who received a standard allograft during th

Fate of nonreplaced sinuses of Valsalva in bicuspid aortic valve disease.

OBJECTIVE: There is growing consensus that the ascending aorta should be replaced at the time of aortic valve replacement for bicuspid aortic valve even if it is only moderately dilated; the natural history of nonreplaced sinuses of Valsalva is less clear. METHODS: We identified patients without defined connective tissue disorder undergoing primary aortic valve replacement for bicuspid aortic valve and separate repair of the ascending aorta without root replacement at the Mayo Clinic between January 1, 1988, and December 31, 2007. RESULTS: Among 218 patients, 65 underwent ascending aortoplas

Modification of the David procedure for reconstruction of incompetent bicuspid aortic valves.

The David procedure was described primarily to treat tricuspid valves. The asymmetry of the bicuspid root asks for modifications to achieve a competent bicuspid valve. The most common feature of the bicuspid valve is the presence of left and right coronary rudimentary cusps. In this case usually the base of the noncoronary cusp is displaced toward the left ventricular outflow tract. The uneven plane of this type of bicuspid aortic annulus has to be compensated for when a rigid prosthesis is wrapped around the aortic root. We describe the modification of the David technique in 14 patients who u

Acute myocardial infarction in a young patient with bicuspid aortic valve.

Bicuspid aortic valve is one of the most common congenital heart valve disorders. We present the development of acute myocardial infarction (AMI) in an 18-year-old male patient with unrecognized bicuspid aortic valve and moderate aortic regurgitation. He presented with chest pain. The electrocardiogram showed ST-segment elevation in leads V2 to V6. Creatine kinase-MB level was elevated to 97 U/l and troponin I was very high (45,000 ng/ml). The diagnosis was made as anterior wall AMI. Following treatment with intravenous rt-PA, ST-segment elevation completely returned to normal. Transthoracic e

Management of the ascending aorta in patients with bicuspid aortic valve disease.

Bicuspid aortic valve (BAV) disease is the most common form of congenital heart disease, affecting 1-2% of the population. Only 20% of patients will maintain normal valve function throughout their life and more than 30% of patients will develop serious morbidity. It is a highly heritable condition, with transmission likely to be autosomal dominant. Patients with BAV have a 10-fold risk of aortic dissection when compared to the normal population. Management of BAV associated aortopathy represents a significant clinical challenge.

Proteomics of ascending aortic aneurysm with bicuspid or tricuspid aortic valve.

Bicuspid aortic valve is often associated with lesions of the ascending aorta, which differ histologically from those in tricuspid valve patients. We undertook proteomic analyses to assess differences at the proteome level. Aortic samples were collected from 20 patients undergoing aortic valve and/or ascending aortic replacement; 9 had a bicuspid valve: 5 with aortic aneurysm (diameter > 50 mm) and 4 without dilation; 11 had a tricuspid valve: 6 with aortic aneurysm and 5 without dilation. Patients with histologically proven connective tissue disorders were excluded. Samples were dissected, so

Recurrent endocarditis of a bicuspid aortic valve due to Q fever.

A 46-year-old man was referred to our institution for a recurrent endocarditis with negative blood culture. Clinical examination and complementary investigations confirmed the diagnosis of aortic valve endocarditis with left ventricular fistula. Blood culture was negative but serological tests were positive for Coxiella burnetti. Aortic valve replacement and fistula repair were done. A combination of Doxycycline and Chloroquine antibiotics was given postoperatively with a clinical improvement. Coxiella burnetti should be systemically searched for in all cases of endocarditis even with negative

Frequency of bicuspid aortic valve in young male conscripts by echocardiogram.

The database of echocardiographic examinations performed during the military screening of 20,946 young men in northeastern Italy was systematically reviewed to assess the frequency, hemodynamic characteristics, and aortic sizes of subjects with bicuspid aortic valves (BAVs). One hundred sixty-seven patients with BAVs were identified (0.8%), of whom 80 (48%) were diagnosed de novo during military screening. Aortic insufficiency was the predominant hemodynamic lesion in 110 subjects (66%), mild in 90, and moderate to severe in 20. Patients with BAVs displayed larger aortic sizes than controls at

Results of valve preservation and repair for bicuspid aortic valve insufficiency.

BACKGROUND AND AIM OF THE STUDY: There is increasing interest in aortic valve-sparing and repair techniques for the treatment of aortic insufficiency (AI) and/or root aneurysm. The results of bicuspid aortic valve (BAV) repair at the authors' institution were evaluated. An attempt was made to assess the mode of failure and to identify surgical methods that provide durable repair results. METHODS: Aortic valve repair for BAV was performed in 71 patients (62 men, nine women; mean age 41.5 +/- 13.2 years) between 1993 and 2005. Repair techniques included cusp free margin plication (n = 47), subc

Midterm results of conservative repair of the incompetent bicuspid aortic valve.

AIM: Repair of the bicuspid aortic valve is not generally considered the treatment of choice. Our success with this procedure leads us to report our immediate and mid-term METHODS: From August 1993 to December 2000, 19 patients with aortic regurgitation due to congenital aortic valve underwent aortic valve repair (17 men and 2 women with a mean age of 42+/-17 years; range, 16 to 70 years). The mean preoperative aortic regurgitation grade was 3.1+/-0.8 on a scale of 1 to 4. Mean preoperative New York Heart Association functional class was 1.9+/-0.8. Fourteen patients had pure aortic regurgitat

Options for repair of a bicuspid aortic valve and ascending aortic aneurysm.

BACKGROUND: Intrinsic abnormality of the aortic wall may explain the association of bicuspid aortic valves with ascending aortic aneurysms. Separate valve and graft repair of such lesions, rather than composite valve graft replacement, is more straightforward but leaves potentially abnormal sinuses behind. METHODS: Between January 1985 and January 1998, 45 patients underwent separate valve and graft (n = 27) or composite valve graft (n = 18) for an ascending aortic aneurysm and bicuspid aortic valve. Perioperative events and late results were compared. RESULTS: Patients undergoing separate v

[A case of bicuspid aortic valve stenosis with single coronary artery].

A 62-year-old female who had a complaint of palpitation was diagnosed as bicuspid aortic stenosis and left single coronary artery by the echocardiographic examination, cardiac catheterization study and coronary artery angiography. Aortic valve replacement was performed and the patient was discharged 22 days after surgery with good post-operative course. Preoperative coronary artery angiography was important in the case of aortic valvular disease.
