Hypoxia-inducible factor-2α plays a role in mediating oesophagitis in GORD.

OBJECTIVE: In an earlier study wherein we induced acute reflux by interrupting proton pump inhibitor (PPI) therapy in patients with reflux oesophagitis (RO) healed by PPIs, we refuted the traditional concept that RO develops as an acid burn. The present study explored our alternative hypothesis that RO results from reflux-stimulated production of pro-inflammatory molecules mediated by hypoxia-inducible factors (HIFs). DESIGN: Using oesophageal biopsies taken from patients in our earlier study at baseline and at 1 and 2 weeks off PPIs, we immunostained for HIF-1α, HIF-2α and phospho-p65, and m

Quantitative analysis of diethyl-IDA reflux. Comparison of controls and dyspeptic, gastric ulcerated, and gastrectomized patients.

The administration of a radioactive tracer through the liver cell and the elimination of bile following an induced vesicular contraction permits duodenogastric reflux to be objectively diagnosed without intubation. This quantitative study of the reflux using 99mTC diethyl-IDA is based on 214 patients and demonstrates: 1) a frequent association with the gastric ulcer, 2) a considerable frequency in gastric resection surgery with duodenogastric anastomosis, 3) a constant occurrence in gastric resection surgery using jejunogastric anastomosis. These findings point to the involvement of biliopancr

A newly modified esophagogastrostomy with a reliable angle of His by placing a gastric tube in the lower mediastinum in laparoscopy-assisted proximal gastrectomy.

BACKGROUND: An optimal reconstruction method for proximal gastrectomy (PG) remains elusive. Esophagogastrostomy (EG) is technically simple b … 01 to 2005. RESULTS: Laparoscopic procedures were performed more frequently, and residual food and bile reflux were less common in the EG group than in the JI group. No significant differences in remnant gastritis or reflux esophagitis were observed between the two groups. However, the late complication of intestinal obstruction occurred only in the JI group …

Sphincter of Oddi Dysfunction and the Formation of Adult Choledochal Cyst Following Cholecystectomy: A Retrospective Cohort Study.

To determine the causes underlying the formation of adult choledochal cyst.Anomalous pancreaticobiliary junction is the most widely accepted theory regarding the etiology of choledochal cyst. However, choledochal cysts have been found in patients in the absence of this anomaly. Because the number of adult patients with choledochal cyst is increasing, it is important to address this controversy.Bile amylase levels in the cysts of 27 patients (8 males and 19 females) who had undergone cholecystectomy were retrospectively evaluated. The average age of the 27 patients was 45.8 ± 10.1 years and the

Disorders of gallbladder function related to duodenogastric reflux in technetium-99m DISIDA hepatobiliary scintigraphy.

Controversy exists over the relation between gallbladder dysfunction and the propensity for duodenogastric reflux. To evaluate this, Tc-99m DISIDA hepatobiliary imaging studies in 120 patients were reviewed, excluding patients who had had cholecystectomy or subtotal gastrectomy before scintigraphy. Serial images were obtained at 5, 10, 15, 30, 45, and 60 minutes and up to 24 hours, if indicated, after intravenous injection of 5-10 mCi of Tc-99m DISIDA. Normally, the liver, bile ducts, gallbladder, common bile duct, and bowel are visualized sequentially. Reversal of the normal sequence of gallb

[The effect of Helicobacter pylori infection and duodenogastric reflux on the histology of gastric mucosa in peptic ulcers].

In a group of 96 patients with peptic ulcer (36 patients with ulcer of the stomach, 60 patients with ulcer of the duodenum) we analysed the alterations of the mucosa of the stomach associated with the ulcer. A significantly frequent occurrence was shown of the athropic gastritis and of the dysplasia in patients with ulcer of the stomach in comparison with the group of patients with ulcer of the stomach in the comparison with the group of the patients with ulcer of the duodenum. Among the patients analysed we also appraised the alterations on the picture of the stomach mucosa of the Helicobacte

Use of the appendix to replace the choledochus.

Ten cases of choledochal cyst (CC) were treated by biliary-appendicoduodenostomy. The follow-up comprised a patient interview, ultrasonography (US), and single-proton ejected computerized tomography (SPECT) scanning. In all cases an anti-reflux submucosal tunnel was added to the distal appendico-duodenostomy; all showed an uneventful postoperative course. All the dilated intrahepatic bile ducts had normalized on B-US postoperatively. Four children under went SPECT examination; all of them had patent neo-bile ducts. In the authors' opinion: (1) Anastomosing the cecal end of the appendix to the

Occult pancreaticobiliary reflux is a pathogenic factor of some benign biliary diseases and gallbladder cancer.

BACKGROUND: Pancreaticobiliary maljunction (PBM) is a well-known high-risk factor for biliary malignant tumors because of constant pancreaticobiliary reflux (PBR). However, the impact of occult pancreaticobiliary reflux (OPR), which is characterized by high bile amylase levels in individuals with anatomically normal pancreaticobiliary junction, on biliary diseases remains unclear. The aim of this study was to assess the correlation between OPR and biliary diseases. METHODS: We enrolled 94 consecutive patients with normal pancreaticobiliary junction and primary biliary diseases confirmed by ma

Three hypotheses linking bile to carcinogenesis in the gastrointestinal tract: certain bile salts have properties that may be used to complement chemotherapy.

The literature for the proliferative effect of bile acids on the GI tract has been reviewed and three hypotheses developed regarding their carcinogenicity. Considered as a unit, the GI tract is protected from this carcinogenicity in different ways in the esophagus, stomach, small bowel and colon. Uncharged BAs can enter the esophageal epithelial cells by reflux of gastroduodenal juices in humans, and in animals by surgical alteration of the GI tract or by adding bile concentrate to the diets, and this can initiate GERD and eventually cancer. Acid suppressant therapy used to treat GERD patients

Many faces of pancreaticobiliary reflux.

Pancreaticobiliary reflux may occur either as a result of an anatomically abnormal pancreaticobiliary junction or because of a functionally impaired sphincter despite a normal radiological appearance. It is associated with a wide spectrum of biliary diseases, including gall bladder and bile duct carcinoma. Pancreaticobiliary maljunction and related biliary reflux have been studied extensively in Southeast Asian populations and associations with choledochal cyst and biliary malignancy defined. However, reflux in the absence of ductal malunion has only been described relatively recently and its

Alkaline reflux gastritis. Late results on a controlled trial of diagnosis and treatment.

In 1977, a controlled, prospective trial was initiated to test the hypothesis that excessive enterogastric (EG) reflux was responsible for a unique postgastrectomy syndrome, "alkaline reflux gastritis." Late (42 +/- 3 months) follow-up on all treated patients (N = 14; Rx = 45 cm Roux Y limb) is reported. The following parameters were assessed in symptomatic (N = 11 nonrefluxers, 15 refluxers) and asymptomatic postgastrectomy patients (N = 9): CCK-stimulated scintographically determined EG reflux (EGRI %), intragastric (IG) concentration of bile acids (BA, mM), net bile acid reflux/hr (microM),

Pancreatobiliary Maljunction: Clinical Features, Diagnosis, and Associated Conditions.

CONTEXT.—: Pancreatobiliary maljunction (PBM) is a congenital malformation characterized by the pancreatic and common bile ducts joining anatomically outside the duodenal wall, resulting in the formation of a long common channel. PBM-associated conditions include choledochal cysts and reflux-associated cholecystopathy. Precursor lesions, such as biliary intraepithelial neoplasias, intraductal papillary neoplasms of the bile duct, intracholecystic papillary neoplasms, and cancers associated with PBM, occur in the gallbladder, bile duct, and pancreas. Most PBM cases, along with their associated

[Pay attention to endoscopic diagnosis and treatment for confluence disease of pancreaticobiliary ducts].

The confluence of pancreaticobiliary ducts refers to the area between the end of common bile duct, the opening of main pancreatic duct and duodenal papilla. Those confluence diseases of pancreaticobiliary ducts will cause obstruction of biliary and pancreatic outflow tract, cholestasis, pancreatic juice deposition or pancreaticobiliary reflux, and thus induce gallstones, cholangitis, pancreatitis and other diseases. Endoscopic ampullary sphincterotomy is an important way to treat biliary and pancreatic diseases, but the length of incision is determined by the anatomy of Oddi sphincter. The end

[Duodenitis and its correlation to diseases of the neighbouring organs and to duodeno-gastric reflux (author's transl)].

In 26,4% of patients (n = 201) with disturbances in the upper abdomen a serious proximal duodenitis was found. Older persons were more often concerned. A duodeno-gastric reflux was more frequent in presence of a serious duodenitis than in normal mucous membrane. Duodenitis was significantly more frequent in patients with atropic gastritis, ventricular ulcers and cholecystectomy. In the patients with reflux of bile the cases of antrum gastritis, with diseases of the biliary tract and nonoperated biliary tract diseases were significantly correlated with duodenitis; in atrophic gastritis as well

Pancreaticobiliary maljunction and biliary cancer.

Pancreaticobiliary maljunction (PBM) is a congenital malformation in which the pancreatic and bile ducts join anatomically outside the duodenal wall. Japanese clinical practice guidelines on how to deal with PBM were made in 2012, representing a world first. According to the 2013 revision to the diagnostic criteria for PBM, in addition to direct cholangiography, diagnosis can be made by magnetic resonance cholangiopancreatography (MRCP), 3-dimensional drip infusion cholangiography computed tomography, endoscopic ultrasonography (US), or multiplanar reconstruction images by multidetector row co

Review article: the nature of oesophageal injury in gastro-oesophageal reflux disease.

The purpose of this review was to explore issues relating to the nature of oesophageal injury in gastro-oesophageal reflux disease. Several structural and functional elements of the oesophageal epithelium provide for an inherent resistance against refluxed material. It is only when this defence is overcome that reflux-induced damage ensues. The light microscopic changes in reflux oesophagitis are manifold. Early changes are confined to the epithelium and consist of reactive changes and inflammatory cell infiltration. When the epithelial integrity can no longer be maintained, erosions and ulcer
