The incidence of unilateral blindness and ophthalmoplegia after aneurysm surgery is very rare, but if it occurs, it is mainly caused by intra-operative nerve injury. We experienced 6 cases of unilateral blindness immediately after surgery for 3 recent years. These patients were classified into Hunt-Hess grade I to II except for one patient with III. All patients complained of visual loss with varying degree of lid oedema and ophthalmoplegia ipsilateral to the site of surgery. Angiographic examination of these patients revealed that the aneurysm was located at the internal carotid artery bifurc…

Diabetes, the leading cause of new blindness among U.S. adults aged 20-74 years, accounts for approximately 8% of cases of legal blindness and 12% of all new cases of blindness in the United States each year. One of the national health objectives for the year 2000 is to decrease by 50% the incidence of blindness caused by diabetes (objective 17.10). However, surveillance for blindness among persons with diabetes has not been conducted nationally, and national prevalence estimates of blindness caused by diabetes have been based on state data from the register of the Massachusetts Commission for…

Ocular trauma is a major cause of monocular blindness in both the developed and developing world, but is not seen as a significant cause of bilateral blindness. Trauma is therefore generally thought of as a major cause of blind eyes without being a major cause of blind people. Because of the difficulties of surgical treatment of ocular trauma in many developing countries, long-term management is usually aimed at prevention, e.g. by improving safety standards in the workplace. The situation in countries such as Cambodia, which have high landmine densities, is different. Cambodia has an estimate…

Inflammatory exophthalmos with visual loss due to optic atrophy is reported in a 45-year-old man. Computed tomography showed an ethmoido-frontal mucocel. This single case stresses the severe complication of a mucocel like optic nerve head atrophy. The different tools for diagnosis and treatment are also emphasized.

The authors are investigating cases of corneal blindness in Tunisia. In the rural areas of the southern part of Tunisia, the corneal blindness is the second cause for sight loss and is responsible for almost 25% of the cases of blindness. In areas where preventive blindness programs have been set up, the prevalence rate of blindness caused by corneal disease has risen from 25% to 8% within the past few years. The most frequent etiology is composed by trachoma, traumatism and corneal ulcers. The authors insist on better prevention and better treatment of corneal diseases.

The ERG of 9 cases (18 eyes) of congenital stationary night blindness with normal fundi or myopia were tested. All eyes showed absence of rod ERG and cone ERG with normal a-wave. Scotopic mixed ERG were of the negative type in 7 eyes and the subnormal type in 11 eyes. The b/a ratio was reduced in all eyes. The ERG characteristics are useful for the classification, estimation of the probable location of the lesion, and differential diagnosis of the disease.

Rayleigh matches performed by 13 patients with Schubert-Bornschein type congenital stationary night blindness with normal color vision, revealed that they use consistently slightly more red light primary in order to achieve a brighter yellow match than a control group with normal color vision and visual acuity. The matching differences between the two groups were statistically significant.

A 7-year-old boy with glycogen storage disease type III developed transient acute cortical blindness associated with hypoglycemia on two separate occasions; the patient also demonstrated transient high-voltage slowing on the electroencephalogram over both occipital areas. This patient is the second examined in our pediatric department during a 5 year period with acute cortical blindness associated with hypoglycemia.

We reported a case of congenital stationary night blindness (CSNB) who showed Schubert-Bornschein type electroretinogram (ERG) in the right eye and nearly normal ERG in the left eye with absolute glaucoma. From the electrophysiological findings of both eyes and histological findings of the enucleated eye, we concluded that the mechanism of night blindness in CSNB is an abnormal inhibition in the bipolar cell layer. The experiments in rabbits by vincristine intravenous and intravitreous administrations did not give rise to a negative type ERG, but to a decrease of the amplitude of all ERG compo…

Blindness is a rare complication of acute glomerulonephritis. During three years (1980-82) 32 cases of acute glomerulonephritis are admitted in a Pediatric Department. Five children had neurologic complications among whom three developed complete, bilateral and transient blindness. In all cases blindness is associated with hypertension and intracranial pressure. The mechanism suggested in this kind of amaurosis is cerebral oedema, and the adequate treatment of this oedema permit a complete recovery of the vision.

Various noninvasive test procedures were used to evaluate retinal function in a patient who had become night blind following vincristine chemotherapy. The results obtained were strikingly similar to those reported previously in subjects with recessively inherited stationary night blindness; the dark-adaptation curve was monophasic (ie, no evidence of a scotopic branch), rhodopsin kinetics were entirely normal, and spectral threshold data revealed the presence of residual rod-mediated vision. Also like the heritable condition, the b-wave of the ERG was depressed grossly despite normal a-wave po…

Tennessee has the highest histoplasmosis infection incidence in the United States. Over 60% of the population reacts positively to skin tests and more than 60 new patients require hospitalization for the treatment of this disorder each year. However, the presumed ocular histoplasmosis syndrome (POHS) remains an uncommon cause of blindness in this state. A retrospective analysis performed with the assistance of 173 Tennessee Ophthalmologists identified 98 new POHS cases in 1980. A similar study, performed in a prospective manner, found 44 new POHS cases in the first six months of 1981. An analy…
