A strategy to protect off-the-shelf cell therapy products using virus-specific T-cells engineered to eliminate alloreactive T-cells.

BACKGROUND: The use of "off-the-shelf" cellular therapy products derived from healthy donors addresses many of the challenges associated with customized cell products. However, the potential of allogeneic cell products to produce graft-versus-host disease (GVHD), and their likely rejection by host alloreactive T-cells are major barriers to their clinical safety and efficacy. We have developed a molecule that when expressed in T-cells, can eliminate alloreactive T-cells and hence can be used to protect cell therapy products from allospecific rejection. Further, expression of this molecule in vi

Differences in Pediatric Continuous Renal Replacement Therapy Initiation and Prescription Practices between the jpCRRT Registry and the WE-ROCK Cohort.

INTRODUCTION: Pediatric continuous renal replacement therapy (CRRT) practice varies across regions, but contemporary registry-based comparisons are limited. We aimed to compare the patient characteristics, pre-CRRT status, and initial CRRT prescription patterns between the published Worldwide Exploration of Renal Replacement Outcomes Collaborative in Kidney Disease (WE-ROCK) cohort and a WE-ROCK-aligned cohort from the Japanese Pediatric Continuous Renal Replacement Therapy (jpCRRT) Registry. METHODS: This descriptive comparison used jpCRRT data collected from January 2023 to March 2026 from

Fulminant Mantle Cell Lymphoma Presenting With Hepatic Infiltration and Spontaneous Tumor Lysis Syndrome.

Mantle cell lymphoma (MCL) is an uncommon, aggressive B-cell non-Hodgkin lymphoma that typically presents with lymphadenopathy, splenomegaly, and gastrointestinal (GI) tract involvement, most often as multiple lymphomatous polyps. While hepatic involvement by MCL is rare and usually asymptomatic, hepatic lesions in clinical practice more often represent metastases from GI, breast, lung, or pancreatic primaries. We report a case of MCL initially presenting with tumor lysis syndrome (TLS) and cholestatic liver injury, mimicking hepatobiliary malignancy. A 90-year-old male with coronary artery di

Alloantigen-pulsed host dendritic cells induce T-cell regulation and prolong allograft survival in a rat model of hindlimb allotransplantation.

BACKGROUND: Composite tissue allotransplantation is restricted due to the risks presented by long-term therapeutic immunosuppression. This study is conducted to investigate whether treatment with recipient immature dendritic cells (DCs) pulsed with donor alloantigens can prolong allograft survival and induce T-cell regulation in a rodent model. MATERIALS AND METHODS: Orthotopic hindlimb transplants from Brown-Norway (RT1(n)) to Lewis (RT1(1)) rats were performed (day 0). DCs were propagated from the recipient bone marrow and pulsed with the donor alloantigen lysate. Group 1 (control group) di

Antibody Secretion Capacity in CVID Patients: Immunoglobulin Isotypes and Antigen Specificities After T-Cell-Dependent In Vitro Stimulation.

Background: Common variable immunodeficiency (CVID), the most prevalent symptomatic inborn error of immunity, involves impaired B-cell differentiation and antibody production, causing recurrent infections and the need for life-long immunoglobulin replacement therapy. Methods: This study evaluated the in vitro differentiation of memory B-cells (MBCs) into antibody-secreting cells (ASCs) in CVID patients. Peripheral blood mononuclear cells from 13 CVID patients and 10 healthy controls were stimulated using two protocols: (I) Staphylococcus aureus Cowan Strain I, Pokeweed mitogen, and CpG, or (II

Acute respiratory distress syndrome caused by respiratory syncytial virus.

Acute respiratory distress syndrome (ARDS) complicating severe respiratory syncytial virus (RSV) infection has been described in only a few infants. In contrast to the low mortality rates usually associated with RSV infections (< 5%), mortality rates in the range of 40-70% have been reported in pediatric patients with ARDS. However, studies on patients with ARDS are usually lumped with respect to causation, and the disease course of RSV-induced ARDS has not been previously studied. We examined the pulmonary function abnormalities of 37 infants with RSV-induced respiratory failure who were admi

Case Report: Primary clear cell adenocarcinoma of the urethra-imaging features and literature review.

BACKGROUND: Primary clear cell adenocarcinoma of the urethra (PCCAU) is an extremely rare malignant tumor. Its clinical manifestations are nonspecific, and preoperative diagnosis relies primarily on imaging studies. To date, most reports on this disease are isolated case presentations, and systematic studies correlating imaging findings with pathological results remain scarce. CASE PRESENTATION: A 58-year-old woman was admitted due to voiding dysfunction for 14 months, worsened with gross hematuria for 2 months. Preoperative imaging evaluation included ultrasonography (US), computed tomograph

Mechanochemical endothelial-astrocyte signalling via Piezo1-Epac1 drives neurovascular injury after stroke.

Limited therapies exist to preserve tissue function in ischemia-reperfusion injury, particularly for ischemic stroke, where intravenous thrombolysis remains a primary but risky treatment option. During stroke reperfusion, mechanical forces including hemodynamic shear stress and tissue stiffness change rapidly. However, how the neurovascular endothelium senses and responds to these physical cues to drive pathological injury remains unclear. Using a transient middle cerebral artery occlusion and reperfusion mouse model, we mapped acute shear stress and stiffness remodeling via near-infrared II i

Bridging Evidence Gaps in Severe Carotid Stenosis: A Systematic Review of Pre-Revascularization Risk Stratification, Clinical Predictors, and High-Risk Subgroups.

OBJECTIVE: Severe carotid artery stenosis (CAS) impairs cerebral blood flow and cognitive function. Revascularization may improve outcomes, but patient selection and timing remain challenging. This review synthesizes evidence on pre-revascularization risk factors. This systematic review synthesizes pre-revascularization evidence on CAS-related cognitive decline and evaluates timing, restenosis, high-risk subgroups (dialysis, radiation-induced, and sex), bilateral stenosis, hyperperfusion, and risk for stroke/death/MI outcomes. METHOD: A systematic search of PubMed, Scopus, EBSCO, Web of Scien

The value of multimodal MRI in the clinical grading of acute cerebral infarction.

INTRODUCTION: Acute ischemic stroke (AIS) is one of the prevalent types of stroke, characterized by high mortality and disability rates. For reperfusion therapy in acute cerebral infarction, early and accurate identification of ischemic penumbra, collateral circulation, and clinical grading is crucial for clinicians in devising effective treatment strategies and assessing prognosis. METHODS: Patients diagnosed with AIS were prospectively examined. In this study, the patients were divided into two groups based on National Institutes of Health Stroke Scale score, including the severe (score ≥ 6

DNA Repair Capacity for Personalizing Risk and Treatment Response - Assay Development and Optimization in Human Peripheral Blood Mononuclear Cells (PBMCs).

DNA repair capacity (DRC) is the ability of a cell to repair DNA damage. Differential DRC plays an important role in human disease, including lung and other cancers. Measuring DRC could aid in translational disease research and in personalizing treatment. We developed and optimized a flow cytometry-based assay to measure individual DRC using GFP-expressing plasmids modified by ultraviolet (UV) light for nucleotide excision repair (NER) and restriction enzyme digestion to induce a blunt double-strand cut between promoter and GFP expression regions for nonhomologous end joining (NHEJ). Cryoprese

Surface acoustic wave patch therapy affects tissue oxygenation in ischemic feet.

INTRODUCTION: Transcutaneous oxygen pressure (TcPO2) less than 30 mm Hg at the toe leads to local tissue hypoxia and nonhealing wounds. Studies regularly illustrate that TcPO2 values are strong predictors of healing and can accurately demonstrate altered levels when extremities have restricted blood flow. The objective of this study was to evaluate the effectiveness of surface acoustic wave (SAW) in ischemic feet on local tissue oxygenation. METHODS: Ten patients, ranging from 40-75 years of age and suffering from critical limb ischemia (CLI) were selected from a vascular surgery clinic to un

ESRD in Australia and New Zealand at the end of the millennium: a report from the ANZDATA registry.

BACKGROUND: The incidence and prevalence of treated end-stage renal disease (ESRD) continues to grow throughout the world; however, there is substantial variation between countries in ESRD incidence and outcomes of renal replacement therapy (RRT). The manner of addressing this challenge varies around the world as seen in different patterns of RRT. We present here a summary of the incidence and prevalence of treated ESRD in Australia and New Zealand in 2000. METHODS: Data are derived from the Australia and New Zealand Dialysis and Transplant Registry (ANZDATA), which collects data about all pa

Hypothesis: selective phosphodiesterase-5 inhibition improves outcome in preeclampsia.

The pathogenesis of preeclampsia stems from aberrant changes at the placental interface. The trophoblastic endovascular invasion of tonic spiral arteries that converts them to passive conduits falters. Uteroplacental insufficiency and fetoplacental hypoxemia result. Secondary maternal oxidative stress and an excessive inflammatory response to pregnancy generate the clinical syndrome of preeclampsia. Current treatment focuses on preventing seizures, controlling hypertension, preserving renal function and delivering the baby. We propose that the pathophysiological changes induced by preeclampsia

Cerebrovascular resuscitation after polytrauma and fluid restriction.

BACKGROUND: There are few reproducible models of blast injury, so it is difficult to evaluate new or existing therapies. We developed a clinically relevant polytrauma model to test the hypothesis that cerebrovascular resuscitation is optimized when intravenous fluid is restricted. STUDY DESIGN: Anesthetized swine (42+/-5 kg, n=35) received blasts to the head and bilateral chests with captive bolt guns, followed by hypoventilation (4 breaths/min; FiO(2)=0.21). After 30 minutes, resuscitation was divided into phases to simulate typical prehospital, emergency room, and ICU care. For 30 to 45 min

Extracorporeal CO2 removal.

The extracorporeal carbon dioxide removal (ECCO(2)R) concept, used as an integrated tool with conventional ventilation, plays a role in adjusting respiratory acidosis consequent to tidal volume (Vt) reduction in a protective ventilation setting. This concept arises from the extracorporeal membrane oxygenation (ECMO) experience. Kolobow and Gattinoni were the first to introduce extracorporeal support, with the intent to separate carbon dioxide removal from oxygen uptake; they hypothesized that to allow the lung to 'rest' oxygenation via mechanical ventilation could be dissociated from decarboxy

Protective Effects of Adipose-derived Stem Cell Exosomes vs Dexamethasone on Neuromotor Deficits in Focal Cerebral Ischemia.

INTRODUCTION: Cerebral ischemia is a leading cause of mortality and long-term disability worldwide, characterized by restricted blood flow to the brain, resulting in inflammation, oxidative stress, and neuronal death. Current therapies, such as thrombolytics and corticosteroids, offer limited efficacy in reversing neuronal damage. Recent advances suggest that exosomes derived from mesenchymal stem cells (MSCs) may provide neuroprotective benefits through anti-inflammatory and regenerative mechanisms. This study aimed to evaluate the therapeutic effects of exosomes derived from adipose tissue-M

Improved outcomes of acute lymphoblastic leukemia after allogeneic blood or marrow transplantation with highdose post-transplantation cyclophosphamide in the era of more effective pre-transplant therapy.

The therapeutic landscape in acute lymphoblastic leukemia (ALL) has changed dramatically over the last decade. Allogeneic blood or marrow transplantation (alloBMT) has also evolved and remains an important option for consolidation. We assessed the interplay between these factors by analyzing the outcomes of 251 adult ALL (214 B- and 37 T-ALL) patients undergoing alloBMT with post-transplantation cyclophosphamide across two eras: 2008-2014 (ERA1) and 2015-2022 (ERA2). ERA1 patients were younger (median age 45.5 years vs. 50 years, P=0.03), less likely to have a Hematopoietic Stem Cell Transplan

Secondary prevention of preeclampsia.

Preventing preeclampsia (PE) is crucial for the wellbeing of the mother, fetus, and the neonate with three levels: primary, secondary, and tertiary. Secondary prevention involves pharmacological therapies aimed at stopping the disease's progression before clinical signs. The predominant approach currently employed is the daily administration of low dose Aspirin and calcium. PE is a multifaceted illness characterized by syncytiotrophoblast (STB) stress, leading to endothelial dysfunction and systemic inflammation. Various subtypes of PE, in particular early-onset PE (EOP) and late-onset PE (LOP

Effect of short-term fasting on the cisplatin activity in human oral squamous cell carcinoma cell line HN5 and chemotherapy side effects.

BACKGROUND: Ketogenic interventions like short-term fasting show potential as complementary therapies to enhance the effectiveness of chemotherapy for cancer. However, the specific effects of fasting on head and neck squamous cell carcinoma (HNSCC) cells and healthy oral mucosa cells during these treatments are not well understood. This study investigates whether short-term fasting can differentially impact HNSCC cell survival and viability compared to healthy keratinocytes while undergoing standard chemotherapy regimens. METHODS: This study investigated the effects of fasting on cell viabili
