Microchimerism, the stable persistence of an allogeneic cell population, can result from allogeneic exposures including blood transfusion. Transfusion-associated microchimerism (TA-MC) appears to be a common but newly recognized complication of blood transfusion. Thus far TA-MC has been detected when severely injured patients are transfused. Injury induces an immunosuppressive and inflammatory milieu in which fresh blood products with replication-competent leukocytes can sometimes cause TA-MC. TA-MC is present in approximately half of transfused severely injured patients at hospital discharge …

A 51-year-old male patient with living, unrelated kidney transplantation in Iran in June 2001, developed Plasmodium falciparum P. falciparum … ral smear. He was treated with intravenous quinidine gluconate and oral doxycycline, in addition to blood transfusion, and he responded well to the treatment. An investigation was carried out to try to find the source of malaria infection, which is believed to be the blood or blood products that he received during his initial acute illness. Measures to minimize tra …

BACKGROUND: Preoperative transfusions are frequently given to prevent morbidity in nonpregnant patients with sickle cell disease. We describe a case of a life-threatening delayed hemolytic transfusion reaction with hyperhemolysis syndrome in pregnancy.
CASE: A multigravida with sickle cell disease underwent prophylactic blood transfusion before repeat cesarean delivery. Her immediate postpartum course was uneventful, but on postoperative day number 6 she presented in grave condition with what was thought initially to be an infection versus crisis. Delayed hemolytic transfusion reaction with h…

Sickle cell disease and homozygous β-thalassaemia are common haemoglobinopathies in Oman, with many implications for local healthcare services. The transfusions of such patients take place in many hospitals throughout the country. Indications for blood transfusions require local recommendations and guidelines to ensure standardised levels of care. This article summarises existing transfusion guidelines for this group of patients and provides recommendations for blood banks and transfusion services in Oman. This information is especially pertinent to medical professionals and policy-makers deve…

OBJECTIVES: Blood and its derivatives are one of the most lifesaving products in the modern medicine practice. However, it is not an absolutely safe prescription. Many adverse effects such as infection, transfusion-related acute lung injury, immunosuppression, multi-organ dysfunction, acute respiratory syndrome, transfusion errors, transmission of infectious agents such as HIV, HBV, HCV are attributable to blood transfusion. The aim of this study was to describe how and when blood products were transfused in a referral burn center.
PATIENTS AND METHODS: This cross-sectional study was performe…

INTRODUCTION: Toxicologists are often called upon to interpret results of drug testing. Phosphatidylethanol is a biomarker sensitive for ethanol consumption within the previous one to four weeks and is commonly used in monitoring for alcohol abstinence in organ transplant candidates. Phosphatidylethanol is stable in stored units of packed red blood cells and has been shown to create "false positive" or iatrogenically elevated phosphatidylethanol concentrations after blood transfusion.
METHODS: In this prospective observational study, 50 subjects receiving one unit of packed red blood cells tr…

Patients with sickle cell disease (SCD) can present with numerous complications, primarily associated with red blood cell (RBC) turnover, hemolysis, and associated immune pathway activation. While RBC transfusions can alleviate some of the most severe complications, the survival of transfused RBCs in chronically transfused patients varies significantly. To address this, novel protocols combining RBC biotinylation with flow cytometric detection have been developed, enabling more precise measurements of post-transfusion RBC survival. In an effort to elucidate the half-life of transfused RBCs in

IMPORTANCE: Babesiosis is a worldwide emerging tick-borne disease with an expanding geographic range in the US, Europe, and Asia. Red blood cell exchange transfusion (ET) is often used as an adjunctive treatment for severe illness from babesiosis, particularly in patients with high parasitemia, acute organ injury, or severe hemolytic anemia. Data supporting its clinical effectiveness, however, are lacking.
OBJECTIVE: To test whether ET improves clinical outcomes among hospitalized adult patients with severe babesiosis.
DESIGN, SETTINGS, AND PARTICIPANTS: This target trial emulation used data

OBJECTIVES: Phosphatidylethanol (PEth) is a long-term marker of alcohol consumption used clinically for evaluating abstinence in patients including transplant candidates. Packed red blood cell (pRBC) transfusion can introduce exogenous PEth to recipients, complicating interpretation. This study evaluated the kinetics and duration of PEth 16:0/18:1 positivity post-transfusion.
DESIGN & METHODS: This study evaluated liver transplant recipients (n = 76) who received ≥ 1 pRBC during transplantation surgery. PEth 16:0/18:1 concentrations were monitored up to 2 weeks post-transfusion to determine c

"Bombay" and "Para-Bombay" phenotypes are uncommon blood groups with absence or deficiency of H antigen. These individuals possess anti-H antibodies and crossmatching with ABO blood group packed red blood cells (PRBC) becomes incompatible. In this report, we find out alternative strategy (patient blood management) for the management of anemia in Bombay blood group patient. Case: A 49-year-old woman referred to our hospital with severe anemia (Hb = 6 gm/dl). On history patient visited to peripheral hospital with complains of fatigue and weakness. Patient was transfused one unit of group O PRBC

Premature infants commonly receive adult packed red blood cells (pRBCs) during their hospital stay. As adult erythrocytes differ substantially from those of preterm infants, transfusion of adult pRBCs into preterm infants can be considered inappropriate for the physiology of a preterm infant. An absence of standardisation of transfusion protocols makes it difficult to compare and interpret pertinent clinical data, as reflected by unclear associations between pRBC transfusion and complications related to prematurity, such as bronchopulmonary dysplasia, neurodevelopmental impairment, retinopathy

BACKGROUND: Current American and Canadian guidelines recommend delaying live vaccine immunization by 5 to 6 months after transfusion of red blood cells (RBCs) due to potential interference by serum antibodies. Due to lack of data, the recommendations may be unfounded and prevent valuable vaccination opportunities for children with frequent blood transfusions. The primary aim of this study was to determine measles, mumps, rubella (MMR) vaccine immunogenicity in patients chronically transfused with RBCs. A secondary aim was to quantify vaccine antibodies in RBC donations.

BACKGROUND: Bartonella spp. are neglected fastidious Gram-negative bacilli. We isolated Bartonella henselae from 1.2% of 500 studied blood donors and demonstrated that the bacteria remain viable in red blood cell units after 35 days of experimental infection. Now, we aim to evaluate the possibility of B. henselae transmission by blood transfusion in a mouse model.
STUDY DESIGN AND METHODS: Eight BALB/c mice were intraperitoneal inoculated with a 30 µL of suspension with 10(4) CFU/mL of B. henselae and a second group of eight mice were inoculated with saline solution and used as control. After…
