[Cavernous angiomas of the brain stem: auditory and vestibular manifestations].

AIMS: To establish the correlation between the clinical manifestations of hearing and balance disturbance and the anatomical site within the pons of cavernous angiomas, and to describe their clinical features, and the findings on ABR and MRI. MATERIALS AND METHODS: Two clinical cases of cavernous angioma with cochlear and vestibular manifestations underwent audiometric evaluation, with VNG and ABR as well as CT and MRI scans. CONCLUSION: Cavernous angiomas are rare (less than 2% of intra-cranial space-occupying lesions). Their most common topographical site is midline in the pons, and may gi

Systematic approach to intradural tumors ventral to the brain stem.

Intradural tumors that are situated anterior to the midbrain, pons, and medulla have historically been among the most inaccessible of all intracranial lesions. The classic approaches to the posterior fossa (e.g., suboccipital, retrosigmoid) provide only limited access to the anterior midline, primarily due to interposition of the cerebellum, brain stem, and numerous cranial nerves between the tumor and the viewpoint of the surgeon. A variety of techniques have been developed in recent years that create a craniotomy by removal of a portion of the lateral skull base. These procedures enhance exp

[Vascular stroke of the brain stem. Study of 21 cases].

A prospective study was designed to follow patients with syndromes of brain stem during 12 months (in 1991-1992). The aim was to correlate clinical and radiologic findings and comparing them with the classical descriptions of brain stem syndromes. Twenty one consecutive patients were admitted at the Hospital de Base do Distrito Federal. The diagnosis and the follow up were carried out by neurological examination and neuroradiologic images: CT (100%), angiography (24%, 5) and MRI (5%, 1). Hypertension and old age were the most important risk factors and hemorrhage at pons with extension to midb

Cavernous malformations of the brain stem. A review of 139 cases.

A retrospective analysis of 139 patients with brain stem cavernous malformations is presented. The material consists of 41 cases from Bern and Phoenix and 98 further well-documented cases from the literature. Sixty-eight patients were male, 70 were female. The average age was 31.8 + 11.8 years. Sixty-two percent of the cavernous malformations were in the pons, 14% were in the mesencephalon, 12% were in the pontomesencephalic and in the pontomedullary junction, and 5% were in the medulla. Eighty-eight percent of the patients showed evidence of recent or previous hemorrhage, 55% had one hemorrha

Brain stem and motor evoked responses in "locked-in" syndrome.

A patient suffering for an ischemic "locked-in" syndrome following a subarachnoid hemorrhage, was evaluated with brain stem acoustic evoked potentials (BAEPs), short latency somatosensory evoked potentials (SEPs) and motor evoked potentials (MEPs). Neuroradiological findings failed to reveal any lesion in the brain stem, while a transcranial Doppler showed increased flow velocity upon the basilar artery, suggesting vascular spasm. BAEPs were normal and SEPs showed to be slightly impaired while MEPs upon magnetic cortical stimulation were heavily deranged. Neurophysiological investigations appe

The MRI and CT evidence of primary brain stem injury.

A 61-year-old man made a good recovery from posttraumatic right hemiparesis and prolonged disturbance of consciousness. Computed tomography (CT) showed a hematoma in the interpeduncular cistern. Magnetic resonance imaging (MRI) pinpointed an injury between the left cerebral peduncle and the reticular formation. Apart from contusion, the mechanism of injury may have been disruption of the paramedian branches of the basilar artery as the brain stem impacted on the clivus during the traffic accident. In contrast with reported autopsy findings that brain stem injury must be associated with severe

Magnetic resonance imaging in ischaemic accidents of the brain stem.

Seven cases of ischaemic accident affecting the brain stem are reported. There were two lacunar infarctions and five infarctions due to occlusion of major arteries (two lateral medullary infarctions, one pontine infarction and two thalamo-mesencephalic infarctions). In every case the abnormalities detected at MRI were concordant with the clinical findings. The authors underline the value of MRI in establishing clinico-topographic correlations in vivo and in differentiating lacunar infarctions from infarctions due to stenosis of major arteries in the vertebro-basilar system.

Distribution and morphology of brain stem plaques in Alzheimer's disease.

The morphology, incidence and distribution of senile plaques in the brain stem were examined in 15 cases of Alzheimer's disease, using mainly the Methenamine-Bodian method. The plaques were found in all cases and were grouped into three types according to their morphology. They were not randomly scattered in the brain stem, but had a distribution common to all cases. There were numerous plaques in the periaqueductal gray, superior colliculus, fourth-ventricle floor and superior central nucleus. The plaques were also found less abundantly in the reticular formation, substantia nigra, pontine nu

Primary pulmonary adenocarcinoma with brain stem metastasis in a dog.

Primary pulmonary adenocarcinoma with metastatic spread to the brain stem was diagnosed at necropsy in an aged Bulldog. Initial clinical signs included coughing, decreased exercise tolerance, and personality changes. Primary differential diagnoses after thoracic radiography were multicentric metastatic neoplasia or fungal disease. Later clinical signs (inappetence, unilateral facial paralysis and decreased sensation, and hemiparesis) were referable to the effect of the tumor on cranial nerves V and VII and other brain stem structures. Primary lung tumors are uncommon, as are metastatic tumors

An anomaly in the auditory brain stem projections of hypopigmented ferrets.

The number of cochlear nucleus (CN) neurones projecting to the ipsi- and contralateral inferior colliculus (IC) was studied in darkly pigmented and hypopigmented ferrets by the retrograde transport of wheat germ agglutinin-horseradish peroxidase. The contralateral projection in all divisions of the CN of both albino and 'red eyed' ferrets was normal. However, the ipsilateral projection from the anteroventral CN was significantly smaller in albino than in pigmented ferrets. The volume of each division of the CN was also measured and was found to be the same in albinos as in pigmented animals. T

Brain stem mucormycosis in a narcotic addict with eventual recovery.

In addition to the well-known infectious complications of intravenous narcotic abuse, a much rarer and more recently recognized association between intravenous narcotic addiction and mucormycotic abscesses of the central nervous system has been described. Only four cases have been cited in the literature, with a mortality rate of 100 percent in this group. This report describes a narcotic abuser who presented with obstructive hydrocephalus and a mucormycotic abscess of the brain stem, and recovered. Central nervous system mucormycosis should be included in the differential diagnosis of drug ab

[Secondary hematoma of the brain stem. Apropos of 5 cases].

Five cases of brain-stem hematoma are described. The cause of these hematomas was identified as "cryptic angioma" (1 cavernous angioma, 1 telangiectasia, 3 arteriovenous malformations). So, they are so-called "secondary hematoma", as opposed to brain-stem hematoma in relation with hypertension. Such secondary hematomas are reported in the literature: 37 operated on cases and 22 untreated cases were found. The clinical picture does not seem to be typical. The presentation appears to be either with the acute onset of a stroke, or with a subacute onset including relapsing symptoms. A progressive

Brain stem infarction as a complication of giant-cell arteritis.

Two cases of brain stem infarction as an early and fatal complication of giant-cell arteritis are reported. These complications occurred despite adequate treatment with corticosteroids. The findings at autopsy are compared with those of the literature. The possible pathogenetic mechanisms of vertebro basilar occlusion and the therapeutical implications are discussed.

The approximation of audiometric thresholds by auditory brain stem responses.

Auditory brain stem response (ABR) thresholds for tone pip stimuli were compared with audiometric thresholds for 63 ears to assess the feasibility of deriving audiograms with ABR techniques. Correlations between the two measures were highly significant, indicating a high degree of accuracy in assessing degree and configuration of hearing loss with ABR. Patients with conductive losses showed a simple one-to-one relationship between ABR and audiometric thresholds, while patients with cochlear losses did not. Possibly this is due to the effects of abnormal temporal integration or poor frequency s

Topological analysis of the brain stem of the lungfish Lepidosiren paradoxa.

The ventricular sulcal pattern and the cellular structure of the brain stem of the lungfish Lepidosiren paradoxa have been studied in transversely cut Nissl and Bodian stained sections. Five longitudinal sulci, the sulcus medianus inferior, the sulcus intermedius ventralis, the sulcus limitans, the sulcus intermedius dorsalis and the sulcus medianus superior could be distinguished. In the isthmus region a number of obliquely oriented sulci is present. One of these, designated here as sulcus d, continues as a longitudinal groove into the mesencephalon. In Lepidosiren most neuronal perikarya are

[Disorders of the coordination of masticatory muscles in lesions of the brain stem].

Observations of 10 patients were reported. In addition to other neurologic deficits, coordination of the trigeminus, a motor nerve, may also be disturbed with brain lesions. Disturbance of bilateral synergism can be demonstrated with the EMG. The muscles closing the jaw on the affected side are paradoxically innervated when the jaw is opened, i.e., they function like antagonists to the muscles responsible for jaw closure on the opposite side. This condition may produce trismus. Similar central disturbances in bilateral coordination of the ocular muscles have also been established. Bilateral sy

[Syndromology of a vascular lesion of the oral regions of the brain stem].

Nineteen patients with a history of circulatory brain disorders in the oral areas of the brain stem showed, along with known neurological syndromes, psychopathological conditions (peduncular hallucinosis, oneiroid and deliriously changed consciousness) and other more rare syndromes (hypersomnia, akinetical states with mutism and intellectual and memory impairment). Apart from describing the clinical picture of these conditions, the pathogenetical mechanisms underlying the syndromes are suggested.

[Intracranial cerebral herniations and secondary lesions in the brain stem].

Intracranial cerebral herniations with secondary lesions in the brain stem are reported. The secondary lesions founded on the upper brain stem of 23 encephala with cerebral herniations are described. Hemorrhages in arteries were seen in all the brains. The lesions points to a mechanical etiology, the caudal displacement of the brain stem being the most important factor. Areas of ischemic infarction were found in the brain stem in all the cases; it is suggested that they were produced by factors other than the causative of the hemorrhagic lesions.
