Improvement in electrocardiographic parameters of repolarization related to sudden death in patients with ventricular dysfunction and left bundle branch block after cardiac resynchronization through His bundle pacing.

BACKGROUND: Cardiac resynchronization therapy (CRT) through permanent His bundle pacing (p-HBP) normalizes interventricular conduction disorders and QRS. Similarly, there are immediate and long-term changes in repolarization, which could be prognostic of a lower risk of sudden death (SD) at follow-up. We aimed to compare the changes in different electrocardiographic (ECG) repolarization parameters related to the risk of SD before and after CRT through p-HBP. METHODS: In this prospective, descriptive single-center study (May 2019 to December 2021), we compared the ECG parameters of repolarizat

Feasibility and Efficacy of His Bundle Pacing or Left Bundle Pacing Combined With Atrioventricular Node Ablation in Patients With Persistent Atrial Fibrillation and Implantable Cardioverter-Defibrillator Therapy.

Background Persistent atrial fibrillation may lead to a higher probability of inappropriate shocks in heart failure patients with an implantable cardioverter-defibrillator (ICD). The aim of this study was to evaluate the impact of His-Purkinje conduction system pacing combined with atrioventricular node ablation in improving heart function and preventing inappropriate shock therapy in these patients. Methods and Results A total of 86 consecutive patients with persistent atrial fibrillation and heart failure who had indications for ICD implantation were enrolled from January 2010 to March 2018.

Successful catheter fulguration of the His bundle in a postoperative Mustard patient after unsuccessful fulguration of ectopic atrial foci.

Incessant supraventricular tachycardia (SVT) resistant to pharmacological agents may cause cardiac dysfunction requiring more aggressive therapy. We present the case of a 12-year-old postoperative Mustard patient who developed biventricular heart failure due to an atrial ectopic tachycardia resistant to amiodarone. Using endocavitary direct current fulguration techniques, catheter ablation of the His bundle was successfully performed after unsuccessful attempts at ablation of the ectopic atrial foci. After placement of a permanent transvenous rate responsive ventricular pacemaker, the patient'

Alternating bundle branch block and infra-Hisian Mobitz II block: an educational case of rapidly progressive His-Purkinje disease.

BACKGROUND: Disease of the cardiac conduction system may arise in the atrioventricular node or distal His-Purkinje network, with important differences in mechanism, prognosis, and management. Alternating bundle branch block (ABBB) is a rare, high-risk electrocardiographic finding reflecting advanced bilateral His-Purkinje involvement, particularly when accompanied by atrioventricular block. Full serial documentation of its progression with intracardiac confirmation during the same clinical course appears exceptionally uncommon. CASE SUMMARY: We present an educational case of rapidly progressi

A Case Report of a Wenckebach Phenomenon Occurring during a His-Bundle Pacing Procedure: Is It Atrioventricular Node Pacing?

A 70-year-old man with severe valvular cardiomyopathy, permanent atrial fibrillation (AF) with a slow ventricular response, and transient atrioventricular (AV) block, was admitted to our center for severe heart failure and recurrent presyncope. While hospitalized, the coronary computed tomography angiography (CTA) showed huge atriums. We tried His bundle pacing (HBP). HB potential was observed at site A, and the His-ventricular (HV) interval was 68 ms. The duration from the stimulus signal to the onset of paced QRS (S-QRSonset) at site A was 232 ms when pacing at 60 beats per minute (BPM) with

Impact of physiological pacing on functional mitral regurgitation in systolic dysfunction: Initial echocardiographic remodeling findings after His bundle pacing.

BACKGROUND: Although His bundle pacing (HBP) has been shown to improve left ventricular ejection fraction (LVEF), its impact on mitral regurgitation (MR) remains uncertain. OBJECTIVES: The aim of this study was to evaluate change in functional MR after HBP in patients with left ventricular (LV) systolic dysfunction. METHODS: Paired echocardiograms were retrospectively assessed in patients with reduced LVEF (<50%) undergoing HBP for pacing or resynchronization. The primary outcomes assessed were change in MR, LVEF, LV volumes, and valve geometry pre- and post-HBP. MR reduction was characteriz

Early improvement of left ventricular ejection fraction by cardiac resynchronization through His bundle pacing in patients with heart failure.

AIMS: Permanent His bundle pacing (p-HBP) can correct intraventricular conduction disorders and could be an alternative to traditional cardiac resynchronization therapy (CRT) via the coronary sinus. We describe the short-term impact of HBP on left ventricular ejection fraction (LVEF) and improvement of left intraventricular synchrony. METHODS AND RESULTS: This prospective descriptive study, performed from January 2018 to February 2019, included patients with left bundle branch block (LBBB) and an CRT indication who were resynchronized by p-HBP. We used the Medtronic C315 His catheter or a com

The Efficacy of His Bundle Pacing: Lessons Learned From Implementation for the First Time at an Experienced Electrophysiology Center.

OBJECTIVES: This study sought to evaluate the clinical and procedural characteristics impacting outcomes during implementation of a His bundle pacing (HBP) program in a real-world setting. BACKGROUND: Right ventricular pacing is associated with an elevated risk of heart failure, but device reprogramming and upgrades have significant challenges. HBP has emerged as an alternative and is reported to be highly successful in the hands of highly experienced centers. METHODS: All patients referred for permanent pacemaker implantation at the Valley Hospital (Ridgewood, New Jersey) between October 20

Cardiac resynchronization therapy in non-ischemic cardiomyopathy: a comparative non-randomized study of His Bundle pacing versus biventricular pacing.

BACKGROUND: Cardiac resynchronization therapy (CRT) via permanent His bundle pacing (pHBP) has gained acceptance globally, but robust studies comparing pHBP-CRT with classic CRT are lacking. In this study, we aimed to compare the improvement in left ventricular ejection fraction (LVEF) after pHBP-CRT versus classic CRT. METHODS: This was a single-center study comparing a prospective series of pHBP-CRT with a historical series of CRT via classic biventricular pacing (BVP). Patients with non-ischemic cardiomyopathy, baseline LVEF < 35%, left bundle branch block (LBBB), and CRT indications were

Comparison of single and double vein approaches for His bundle ablation and pacemaker placement for symptomatic rapid atrial fibrillation.

BACKGROUND: His Bundle ablation (HBA) with permanent pacemaker (PPM) implantation is an effective strategy for controlling heart rate in symptomatic patients with rapid atrial fibrillation (AF), resistant to pharmacologic therapy. The standard double vein (DV) approach involves mapping and HBA from a femoral approach to achieve complete heart block (CHB), while single chamber (SC), dual chamber (DC), or biventricular (BiV) PPM is then placed via a subclavian approach. METHODS: We compared 7 patients with drug-resistant rapid AF who underwent the standard DV approach to 8 patients who underwen

[Left bundle of his brunch block as a manifestation of the syndrome of dromotropic insufficiency in patients with chronic heart failure].

Left bundle brunch block (LBBB) is met in approximately 20% of hospitalized patients and in 1/3 of outpatients with chronic heart failure (CHF). Abnormalites of impulse conduction in LBBB promote dyssynchronization, which represent a pathophysiological process which not only worsens ventricular function but also facilitates remodeling of the myocardium and development of CHF. As a result of dyssynchrony systolic and diastolic functions of the heart are impaired. This manifests as shortening of diastole of the left ventricle and dyssynchronous movement of interventricular septum (IVS) with a te

[Diagnosis of myocardial infarction complicated by complete block of the left branch of His bundle as shown by computed electrocardiotopography].

AIM: To assess efficacy of computed electrocardiotopography (CECT) in diagnosis of myocardial infarction (MI) complicated by a complete block of His' bundle left branch (HBLB). MATERIAL AND METHODS: Twenty two patients with a complete block of HBLB have undergone CECT (hardware Rhythm-M, software "Rhythm" and "Heart"). RESULTS: Five patients had no scars. The rest 17 patients had MI. The diagnosis of MI was definite by ECG in 6 (35%) patients. In 11 (65%) patients MI diagnosis by ECG was doubtful. Only CECT detected typical for MI changes in the QRS changes. CONCLUSION: CECT can significant

[Choice of the mode of stimulation after ablation of the bundle of His. Experience based on a retrospective survey of 192 patients].

A retrospective study of 192 patients centered on the outcome of supraventricular arrhythmias after catheter ablation of the atrioventricular junction provided some useful information concerning the choice of pacing mode in these patients. With the exception of atrioventricular bloc after ablation of the rapid nodal pathway where simple DDD pacing is adequate, rate adaptive pacing would seem to be essential. The VVIR mode should be the mode of choice in atrial flutter, permanent atrial fibrillation, poorly controlled atrial fibrillation and paroxysmal atrial fibrillation of elderly subjects (o

An Unusual Electrogram Sequence with a Questionable Potential on the His-bundle Catheter During Sinus Rhythm: What Is the Mechanism?

The presence of 2 ventricular signals caused by structures near the His bundle region is rare. Some associative and dissociative maneuvers for dissociating a certain electrical signal from others of known origin are used to ascertain the source of an unknown potential.
