Healthcare professionals' knowledge and attitudes toward the microbiome and fecal microbiotherapy or microbiota transfer in hematology and oncology: an international survey.

The gut microbiome has emerged as a critical factor influencing cancer progression, treatment response, and complications in hematology and oncology. Fecal microbiota transfer (FMT) is being explored as a novel biotherapeutic strategy, yet the knowledge and attitudes of healthcare professionals towards its clinical use remain poorly defined. We conducted an international survey of 152 healthcare professionals, including hematologists, oncologists, transplant specialists, and others. Most respondents were affiliated with academic centers (66.0%) and practiced primarily in hematology (48.0%) or

The Transitional Liver Clinic: Study protocol for a stepped-wedge cluster randomized trial.

BACKGROUND: Patients with complications of advanced liver disease experience high rates of hospital readmission after discharge. Transitional care models have improved outcomes in other chronic conditions, but their efficacy in liver disease care remains uncertain. This randomized trial evaluates the effectiveness of a tailored transitional care model-the Transitional Liver Clinic (TLC)-in reducing readmissions, improving quality of life, and enhancing patient satisfaction for patients with advanced liver disease. METHODS: This is a 45-month, stepped-wedge cluster randomized trial enrolling u

Healthcare impact of electroencephalography performed during clinical neurophysiology on-call duty in a third-level hospital.

INTRODUCTION: The electroencephalogram (EEG) is a useful tool in the diagnosis of pathologies such as non-convulsive status epilepticus (NCSE) or brain death (BD), cardiac arrest (CA), and status epilepticus (SE) treatment monitoring. In addition, it provides irreplaceable information depending on the time it is performed, as is the case with the diagnosis of epilepsy after a first epileptic seizure (ES) or to differentiate these from non-epileptic paroxysmal events (NEPE). Its usefulness is maintained outside the usual working day, but it is not available in many centers. SUBJECTS AND METHOD

Frozen section evaluation of deceased donor kidney biopsies: A field guide.

The demand for donated kidneys is steadily increasing, making it critical that the transplant community maximizes the use of organs procured from deceased donors. When a donated kidney becomes available, a transplant program evaluates the suitability of the organ for their patients and will choose whether to decline or accept. A pre-implantation biopsy may be performed if the transplant center is unsure about the suitability of the organ. More than half of deceased donor kidneys are biopsied prior to implantation, and "biopsy findings" is the most commonly cited reason for a kidney not being a

Rationale for the Development of a Traumatic Brain Injury Case Definition for the Pilot National Concussion Surveillance System.

BACKGROUND: Current methods of traumatic brain injury (TBI) morbidity surveillance in the United States have primarily relied on hospital-based data sets. However, these methods undercount TBIs as they do not include TBIs seen in outpatient settings and those that are untreated and undiagnosed. A 2014 National Academy of Science Engineering and Medicine report recommended that the Centers for Disease Control and Prevention (CDC) establish and manage a national surveillance system to better describe the burden of sports- and recreation-related TBI, including concussion, among youth. Given the l

Necropolitics as accumulation: Enforcement and enclosure in Brisbane during COVID-19.

This article explores the uneven impacts that Indigenous and detained migrant populations have endured in Australia in the context of the COVID-19 pandemic. Australia has one of the most restrictive immigration enforcement systems in the world. Along with imposing practices of mandatory detention in rural and remote regions, the Australian government finances the carceral systems of nearby countries and island nations. These logics of enforcement are embedded within histories and techniques of Indigenous quarantine, incarceration, and colonial erasure. Following Achille Mbembe (2019), I advanc

Functional outcomes of Fournier's gangrene: a multi-institutional experience.

INTRODUCTION: Fournier's gangrene (FG), is a progressive, necrotizing soft tissue infection of the external genitalia, perineum, and/or anorectal region. How treatment and recovery from FG impacts quality of life related to sexual and general health is poorly characterized. Our purpose is to evaluate the long term impact of FG on overall and sexual quality of life using standardized questionnaires through a multi-institutional observational study. MATERIALS AND METHODS: Multi-institutional retrospective data were collected by standardized questionnaires on patient-reported outcome measures in

Real-world data on cannabidiol treatment of various epilepsy subtypes: A retrospective, multicenter study.

OBJECTIVE: Cannabidiol (CBD) is approved for treatment of Dravet syndrome (DS), Lennox-Gastaut syndrome (LGS), and tuberous sclerosis complex (TSC). Several studies suggest antiseizure effects also beyond these three epilepsy syndromes. METHODS: In a retrospective multicenter study, we analyzed the efficacy and tolerability of CBD in patients with epilepsy at 16 epilepsy centers. RESULTS: The study cohort comprised 311 patients with epilepsy with a median age of 11.3 (0-72) years (235 children and adolescents, 76 adults). Therapy with CBD was off-label in 91.3% of cases due to age, epilepsy

Disparities in pancreatic cancer care and research in Native Americans: Righting a history of wrongs.

Disparities in pancreatic cancer incidence and outcomes exist in Native American populations. These disparities are multifactorial, difficult to quantify, and are influenced by historical, socioeconomic, and health care structural factors. The objective of this article was to assess these factors and offer a call to action to overcome them. The authors reviewed published data on pancreatic cancer in Native American populations with a focus on disparities in incidence, outcomes, and research efforts. The historical context of the interactions between Native Americans and the United States healt

Aging in the downtown corridor: Mapping the neighborhood experiences of Seattle's unhoused adults over age 50.

Research has established the importance of understanding the dynamic relationship between older adults and the environments in which they are embedded. However, the meaning of place for unhoused older adults amidst an increasingly contested urban landscape is largely unknown. This exploratory study aims to further include unhoused older adults' experiences in the scholarship on aging and place by asking how unhoused adults over age 50 (1) describe their spatial patterns and experiences and (2) negotiate their relationship with common urban places. Through iterative mapping conducted in focus g

Laboratory tests of headache disorders - dawn of a new era?

Context The classification of headache disorders has improved over the years, but further work is needed to develop and improve headache diagnosis within headache subtypes. The present review is a call for action to implement laboratory tests in the classification and management of primary and some secondary headaches. Background In this narrative review we present and discuss published tests that might be useful in phenotyping and/or diagnosis of long-lasting headache disorders such as migraine, tension-type headache, trigeminal autonomic cephalalgias, trigeminal neuralgia and persisting seco

The cost of surveillance after urethroplasty.

OBJECTIVE: To determine variability in urethral stricture surveillance. Urethral strictures impact quality of life and exact a large economic burden. Although urethroplasty is the gold standard for durable treatment, strictures recur in 8%-18%. There are no universally accepted guidelines for posturethroplasty surveillance. We performed a literature search to evaluate variability in surveillance protocols, analyzed costs, and reviewed performance of each commonly used modality. METHODS: MEDLINE search was performed using the keywords "urethroplasty," "urethral stricture," and "stricture recur

Factors associated with breast MRI use: a population-based analysis.

INTRODUCTION: Although controversial, the use of breast magnetic resonance imaging (MRI) is widespread. We sought to determine factors that influenced its use in a population-based sample. METHODS: The National Health Interview Survey is conducted annually by the Centers for Disease Control and is designed to be representative of the American population. Data from 2010 were queried for the use of breast MRI and associated sociodemographic and risk characteristics. RESULTS: Of the 11,222 women aged ≥30 years who were surveyed, 4.7 % reported ever having a breast MRI. Nearly a quarter were don

Artificial muscle for reanimation of the paralyzed face: durability and biocompatibility in a gerbil model.

BACKGROUND: Current management of permanent facial paralysis centers on nerve grafting and muscle transfer; however, limitations of those procedures call for other options. OBJECTIVES: To determine the durability and biocompatibility of implanted artificial muscle in a gerbil model and the degree of inflammation and fibrosis at the host tissue-artificial muscle interface. METHODS: Electroactive polymer artificial muscle (EPAM) devices engineered in medical-grade silicone were implanted subcutaneously in 13 gerbils. The implanted units were stimulated with 1 kV at 1 Hz, 24 h/d via a function

Modeling the Triggering of Saccades, Microsaccades, and Saccadic Intrusions.

When we explore a static visual scene, our eyes move in a sequence of fast eye movements called saccades, which are separated by fixation periods of relative eye stability. Based on uncertain sensory and cognitive inputs, the oculomotor system must decide, at every moment, whether to initiate a saccade or to remain in the fixation state. Even when we attempt to maintain our gaze on a small spot, small saccades, called microsaccades, intrude on fixation once or twice per second. Because microsaccades occur at the boundary of the decision to maintain fixation versus starting a saccade, they offe

Contaminant identification: the importance of thorough interviewing techniques.

The exact name of a contaminant is of extreme importance to the poison information specialist (SPI) in order to make a correct assessment of the poisoning exposure and to recommend proper treatment modalities. Incorrect contaminant information could lead to a serious and possibly fatal mistake on the part of the SPI. Thorough and effective interviewing techniques must be employed to avoid errors. We conducted a prospective study to determine how often misinformation is provided to the SPI at the time of the initial contact with the poison center. Over a 6-mo period 159 calls were identified in
