Exposure to household furry pets influences the gut microbiota of infant at 3-4 months following various birth scenarios.

BACKGROUND: Early-life exposure to household pets has the capacity to reduce risk for overweight and allergic disease, especially following caesarean delivery. Since there is some evidence that pets also alter the gut microbial composition of infants, changes to the gut microbiome are putative pathways by which pet exposure can reduce these risks to health. To investigate the impact of pre- and postnatal pet exposure on infant gut microbiota following various birth scenarios, this study employed a large subsample of 746 infants from the Canadian Healthy Infant Longitudinal Development Study (C

EQUIP emergency: can interventions to reduce racism, discrimination and stigma in EDs improve outcomes?

BACKGROUND: Despite a publicly funded system, health care in Canada has been shown to be deeply inequitable, particularly toward Indigenous people. Based on research identifying key dimensions of equity-oriented health care as being cultural safety, harm reduction and trauma- and violence-informed care, an intervention to promote equity at the organizational level was tested in primary health care, refined and adapted, and tested in Emergency Departments (EDs). METHODS: In partnership with clinical, community and Indigenous leaders in three diverse EDs in one Canadian province, we supported d

Clinical characteristics of patients from Quebec, Canada, with Morquio A syndrome: a longitudinal observational study.

BACKGROUND: Morquio A syndrome is a rare, autosomal recessive, progressively debilitating disorder, with multi-system impairments and high medical burden. Quebec, Canada has a large Morquio A population, which is considered unique due to the presence of founder pathogenic variants. The objectives of this study were to document the genetic and clinical heterogeneity of patients with Morquio A in Quebec, to better characterize the phenotype of those with the French Canadian founder pathogenic variant (NM_000512.5: c.1171A>G, p.Met391Val), and to describe the natural history of the patients treat

Rates of minor adverse events and health resource utilization postcolonoscopy.

BACKGROUND: Little is known about minor adverse events (MAEs) following outpatient colonoscopies and associated health care resource utilization. OBJECTIVE: To estimate the rates of incident MAE at two, 14 and 30 days postcolonoscopy, and associated health care resource utilization. A secondary aim was to identify factors associated with cumulative 30-day MAE incidence. METHODS: A longitudinal cohort study was conducted among individuals undergoing an outpatient colonoscopy at the Montreal General Hospital (Montreal, Quebec). Before colonoscopy, consecutive individuals were enrolled and inte

[Prostate-specific antigen in the diagnosis of organ-confined treatable prostate carcinoma].

Prostate cancer is now the most common cancer and the second most common cause of death from cancer among men. Several studies have shown a frequency of autopsy-detected cancer of 40% in men over 50 years of age. In contrast, the lifetime probability of prostate cancer being diagnosed clinically is only 8%. Thus histologically documented prostate cancer only becomes clinically relevant if the tumors are > 0.5 cm3 and the life expectancy exceeds 10 years. Therapy with curative intention is only possible for organ confined disease. Because disease specific survival is about 80-90% after 10 years

Health-related quality of life in Myalgic Encephalomyelitis/Chronic Fatigue Syndrome and Post COVID-19 Condition: a systematic review.

PURPOSE: Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Post COVID-19 Condition (PCC) are debilitating, chronic multi-systemic illnesses that require multidisciplinary care. However, people with ME/CFS (pwME/CFS) and people with PCC (pwPCC) are often precluded from accessing necessary disability and social support services. These unmet care needs exacerbate the existing illness burdens experienced by pwME/CFS and pwPCC. To deliver appropriate care and optimise health outcomes for pwME/CFS and pwPCC, the development of evidence-based healthcare policies that recognise the disab

A population-based study to develop juvenile arthritis case definitions for administrative health data using model-based dynamic classification.

BACKGROUND: Previous research has shown that chronic disease case definitions constructed using population-based administrative health data may have low accuracy for ascertaining cases of episodic diseases such as rheumatoid arthritis, which are characterized by periods of good health followed by periods of illness. No studies have considered a dynamic approach that uses statistical (i.e., probability) models for repeated measures data to classify individuals into disease, non-disease, and indeterminate categories as an alternative to deterministic (i.e., non-probability) methods that use summ

In Utero Exposure to Maternal Injury and the Associated Risk of Cerebral Palsy.

IMPORTANCE: Although maternal unintentional injury during pregnancy has shown negative impacts on the mother and fetus, the evidence on its long-term associations with children's neurodevelopment is limited. OBJECTIVE: To examine the association between maternal unintentional injury and cerebral palsy (CP) in offspring. DESIGN, SETTING, AND PARTICIPANTS: This was a population-based, longitudinal, cohort study of all in-hospital live births born between April 1, 2002, and March 31, 2017, in a publicly funded health care system setting of Ontario, Canada. Infants born more than 20 weeks' gesta

School attendance matters: co-occurring trajectories of school attendance and academic achievement from elementary to secondary school in a Canadian context.

INTRODUCTION: School attendance and academic achievement are central indicators of student engagement and success, and relations between constructs are typically framed as predictors and outcomes, not as co-developing intertwined trajectories across multiple years of schooling. In Canada, where national attendance data are scarce, little is known about how absences and grades co-evolve through critical educational transitions. We examined heterogeneous joint trajectories of school absences and grade point averages (GPAs) from mid-elementary through secondary school (Grade 5 to Grade 12) and ex

The epidemiology of trauma and post-traumatic stress disorder in a representative cohort of young people in England and Wales.

BACKGROUND: Despite the emphasis placed on childhood trauma in psychiatry, comparatively little is known about the epidemiology of trauma and trauma-related psychopathology in young people. We therefore aimed to evaluate the prevalence, clinical features, and risk factors associated with trauma exposure and post-traumatic stress disorder (PTSD) in young people. METHODS: We carried out a comprehensive epidemiological study based on participants from the Environmental Risk Longitudinal Twin Study, a population-representative birth-cohort of 2232 children born in England and Wales in 1994-95. Impact of Canadian human immunodeficiency virus non-disclosure case law on experiences of violence from sexual partners among women living with human immunodeficiency virus in Canada: Implications for sexual rights.

OBJECTIVES: People living with human immunodeficiency virus in Canada can face criminal charges for human immunodeficiency virus non-disclosure before sex, unless a condom is used and their viral load is <1500 copies/mL. We measured the reported impact of human immunodeficiency virus non-disclosure case law on violence from sexual partners among women living with human immunodeficiency virus in Canada. METHODS: We used cross-sectional survey data from wave 3 participant visits (2017-2018) within Canadian HIV Women's Sexual and Reproductive Health Cohort Study; a longitudinal, community-based

Spanish translation, cultural, and content validation of the Mouth Impairment and Disability Assessment in a prospective, longitudinal evaluation of pediatric patients with facial burn injuries.

UNLABELLED: The Mouth Impairment and Disability Assessment (MIDA) was developed to provide clinicians with an outcome measure that thoroughly and objectively evaluates the impairment and disabilities associated with burn injuries of the face involving the mouth. However, thus far, it is only available in English and French-Canadian and has not been used with pediatric burn survivors. Additionally, there is limited longitudinal outcome data describing the recovery trajectory associated with burn injuries of the mouth. Thus, the purpose of this study was to translate the MIDA into Spanish, valid

Utility of diaphragm dome height as a marker of operational lung volume changes, disease burden and exacerbations in patients with mild-to-moderate COPD: an observational study within the CanCOLD cohort.

BACKGROUND: Dynamic hyperinflation is central to dyspnoea, exercise limitation and exacerbations in COPD. While studied previously in moderate-to-severe COPD, the relevance of diaphragm dome height (DDH) on clinically important outcomes has been under-investigated in mild-to-moderate COPD. METHODS: Canadian Cohort Obstructive Lung Disease (CanCOLD) participants with spirometry-confirmed COPD, symptom-limited incremental cardiopulmonary exercise testing and computed tomography image data were included. Base-to-apex left DDH (LDDH) and right DDH (RDDH) were automatically segmented, with increas

Risk Factors for Perinatal Arterial Ischemic Stroke: A Machine Learning Approach.

BACKGROUND AND OBJECTIVES: Perinatal arterial ischemic stroke (PAIS) is a focal vascular brain injury presumed to occur between the fetal period and the first 28 days of life. It is the leading cause of hemiparetic cerebral palsy. Multiple maternal, intrapartum, delivery, and fetal factors have been associated with PAIS, but studies are limited by modest sample sizes and complex interactions between factors. Machine learning approaches use large and complex data sets to enable unbiased identification of clinical predictors but have not yet been applied to PAIS. We combined large PAIS data sets

Association of Psychological Resilience, Cognitive Reserve, and Brain Reserve with Post-Concussive Symptoms in Children with Mild Traumatic Brain Injury and Orthopedic Injury: An A-CAP Study.

Protective factors, including psychological resilience, cognitive reserve, and brain reserve, may be positively associated with recovery after pediatric mild traumatic brain injury (mTBI) but are yet to be studied concurrently. We sought to examine these factors as moderators of post-concussive symptoms (PCS) in pediatric mTBI compared with mild orthopedic injury (OI). Participants included 967 children (633 mTBI, 334 OI) aged 8-16.99 years, recruited from 5 Canadian pediatric emergency departments as part of a prospective longitudinal cohort study. At 10 days post-injury, psychological resili
