Neonatal candidiasis: results of an 8 year study.

BACKGROUND: The aim of the present study was to evaluate the risk factors, demographic features, treatment and clinical outcome associated with candidemia in a neonatal intensive care unit (NICU) within an 8 year period. METHODS: The data of infants who were diagnosed as having candidemia, were evaluated. RESULTS: Between January 2000 and December 2007, a total of 28 candidemia episodes were identified in 28 infants. A 1.1% candidemia incidence was documented in the neonatal intensive care unit (NICU). The species most frequently causing candidemia were Candida parapsilosis (57.1%), followed

Neonatal onychomadesis with candidiasis limited to affected nails.

A case of onychomadesis in one of twins with onset in the neonatal period is reported. We hypothesize that the nail deformities were induced by the stress of a breech presentation during preterm labor and that candidiasis limited to the bands of onychomadesis exaggerated the appearance of the nail deformity.

Effects of modified amphotericin in experimental systemic candidiasis.

Lysosomotropic composition of dialdehyde dextran and amphotericin B had a greater therapeutic effect in mice with systemic candidiasis compared to free amphotericin B. This composition normalized glucocorticoid function of the adrenal glands and decreased the severity of liver destruction at late terms of granulomatous inflammation.

Value of colposcopy in the diagnosis of candidiasis in patients with vulvodynia.

OBJECTIVE: To evaluate the value of colposcopy in the diagnosis of chronic vulvar candidiasis in patients presenting with vulvodynia. STUDY DESIGN: A prospective study of 460 patients presenting with vulvodynia over a 24-month period was undertaken. All patients underwent colposcopy of the vulva, which was regarded as positive if acetowhite lesions with central clearing with or without petechiae were present. Vulvar skin scrapings were taken for Candida culture on all patients. Those patients with positive colposcopic findings were treated with long-term oral antimycotic therapy irrespective …

Assessment of the paradoxical effect of caspofungin in therapy of candidiasis.

Paradoxical growth of some Candida albicans isolates in the presence of caspofungin (CAS) in vitro has been demonstrated previously. We sought to determine whether a similar phenomenon occurred in vivo. A systemic model of candidiasis was studied in CD-1 mice by intravenous inoculation of different isolates of C. albicans. Infected animals were treated with CAS at various dosages (0.01 to 20 mg/kg) and CFU remaining in the kidneys determined. Four clinical isolates that showed paradoxical growth in vitro and one that did not were tested. Recovery of CFU from the kidneys showed that dosages of …

The role of antifungal susceptibility testing in the therapy of candidiasis.

Prior to the introduction of azoles, no real need for antifungal susceptibility testing (AFST) existed, as amphotericin B was the only agent available to treat systemic candidiasis. Introduction of fluconazole and itraconazole provided alternate, less toxic antifungal therapies. Intrinsic resistance of Candida krusei, decreased susceptibility of Candida glabrata, and development of resistance by Candida albicans (in mucosal disease in AIDS) to azoles led to development of our current AFST methodologies. The goal of AFST, like that of antibacterial susceptibility testing, is to predict clinical…

Colonization and epithelial adhesion in the pathogenesis of neonatal candidiasis.

Candida species are important nosocomial pathogens in the newborn population, particularly among the premature very-low-birth-weight infants in neonatal intensive care units. Candida colonization of the neonatal skin and gastrointestinal tract is an important first step in the pathogenesis of invasive disease. C albicans is the most commonly isolated species in colonized or infected infants. Over the past decade the incidence of both colonization and infection with other Candida species, particularly C parapsilosis, has risen dramatically. Colonization of the infant occurs early in life and is

Prevalence of vulvovaginal candidiasis and susceptibility to fluconazole in women.

OBJECTIVE: The purpose of this study was to determine the prevalence of vaginal colonization by Candida with the use of a rapid detection method, to examine the determinants of vaginal candidiasis, and to evaluate susceptibility for fluconazole. STUDY DESIGN: Vaginal swabs were collected from unselected women at the outpatient clinic of the Department of Obstetrics and Gynecology. A differentiation was made between patients with a positive and a negative potassium hydroxide examination. RESULTS: Six hundred twelve women were recruited, of whom 39 women (6.3%) had clinical candidiasis. The ov…

Involvement of aspirin-sensitive oxylipins in vulvovaginal candidiasis.

3(R)-Hydroxyoxylipins are produced via an aspirin-sensitive pathway in Candida albicans, an abundant pathogen in vulvovaginal candidiasis. In the present study, we have investigated the effect of aspirin on vaginal isolates of C. albicans from patients with recurrent candidiasis. Aspirin alone and with clotrimazole, a commonly used drug, strongly suppressed growth of C. albicans. 3(R)-Hydroxyoxylipins, which were selectively located in hyphae and other filamentous structures, but not in free blastospores, were almost totally suppressed by aspirin. Moreover, C. albicans stimulated prostaglandin…

Systemic candidiasis with cataract formation in a premature infant.

Endogenous Candida endophthalmitis is a well-recognized complication in low-birth-weight premature infants. The incidence of systemic candidiasis in premature and low-birth-weight infants is approximately 3%. As many as half of these infants will develop Candida endophthalmitis. Eye involvement typically presents as one or more tiny white "fungal ball" chorioretinal lesions with overlying vitritis. Lens involvement is extremely rare. We report a case of cataract due to Candida albicans in a premature, low-birth-weight infant unresponsive to systemic and intracameral antifungal therapy and requ…

Liposomal amphotericin B in neonates with invasive candidiasis.

Liposomal amphotericin B (L-Amp B), a novel formulation of amphotericin B, is effective for the treatment of invasive fungal infections in c … blished experience on this topic. Liposomal amphotericin B may be an option for therapy of invasive candidiasis in neonates who are at high risk of nephrotoxicity and other amphotericin-related reactions, but clinical trials are necessary to document its safety and efficacy in this age group.

Is erythematous candidiasis associated with advanced HIV disease?

OBJECTIVES: A multicentre collaborative study by the London HIV and Dentistry Group failed to associate the presence of erythematous candidiasis (EC) with advanced HIV disease. Data from a study of the periodontal health of homosexual men attending a genito-urinary medicine clinic presented an opportunity to investigate social and medical factors related to the presence of EC and mucosal disease. SUBJECTS AND METHODS: 312 men with HIV were examined by a single examiner blinded to HIV status; 22.8% had an indicator condition for AIDS; 37.8% had CD4 lymphocyte counts less than 200 cells mm-3; a…

Oral candidiasis: the clinical challenge of resistance and management.

Oral candidiasis is a common fungal infection in patients with an impaired immune system, such as those undergoing chemotherapy for cancer and patients with AIDS. It has a high morbidity amongst the latter group with approximately 85% of patients being infected at some point during the course of their illness. A major predisposing factor in HIV-infected patients is a decreased CD4 T-cell count. The majority of infections are due to C. albicans although other species such as C. glabrata, C. tropicalis, C. krusei and C. parapsilosis are increasingly isolated. The systemic azoles, ketoconazole, f…

The epidemiology of vulvovaginal candidiasis among university students.

Vulvovaginal candidiasis affects approximately 20% of women annually, but it is not well characterized epidemiologically. Of 1027 respondents to two mailed cross-sectional surveys at a large university, 37.5% reported a prior clinical diagnosis of vulvovaginal candidiasis. The frequency of first diagnosis increased rapidly after age 17, with an estimated 54.7% of women experiencing the condition by age 25. In a proportional hazards model of age at first diagnosis, vulvovaginal candidiasis was associated with initiation of sexual activity (rate ratio [RR] = 2.9; 95% confidence interval [CI] = 2…

[The use of fluconazole in low-weight neonates with candidiasis].

Systemic Candida Albicans infections in premature newborns are characterized by high morbidity; early diagnosis and immediate treatment are therefore necessary. In this study we report the cases of fourteen newborns admitted to our Newborn Care Unit in a three-year period, in whom candidiasis was diagnosed. The mycetes was isolated from blood, liquor, peritoneal fluid, urine, oesophageal aspirate. Treatment was undertaken immediately with fluconazole, administered through the parenteral route or locally into peritoneal cavum and oesophageal stump. Duration of treatment varied from five to twen…

Invasive candidiasis in the ICU--pathogenesis and management strategy.

How should new and old antifungals be used in the management of invasive candidiasis in ICU patients? A pathogenesis-based management strategy for invasive candidiasis is outlined. Colonisation of the catheterised bladder by Candida species should be eliminated to prevent invasion from the urinary tract. Invasion from the peritoneum (in the patient with a perforated bowel) should be treated promptly to prevent local vascular invasion. Empirical therapy is recommended for treatment of suspected disseminated candidiasis, 'pulmonary' candidiasis and vascular-access candidiasis. Dose and duration …

Chronic candidal meningitis: an uncommon manifestation of candidiasis.

Chronic meningitis is an uncommon manifestation of candidiasis. We present the case of an elderly woman who had symptoms such as headache, malaise, and fever for 8 months and was found to have Candida albicans meningitis, and we review 17 similar cases. An underlying illness or risk factor for candidiasis was present in only 13 (72%) of the 18 patients. Headache, fever, and nuchal rigidity were the predominant clinical findings. Analysis of CSF showed either mononuclear or neutrophilic pleocytosis, an elevated protein level, and a decreased level of glucose. Only 17% of CSF smears were positiv…
