OBJECTIVE: To describe the beneficial effects of low-dose dopamine infusion (2-5 micrograms/kg/min) in a patient with severe cor pulmonale complicating chronic obstructive pulmonary disease (COPD).
CASE SUMMARY: A 53-year-old woman with severe cor pulmonale and generalized edema complicating COPD received low-dose dopamine to stabilize blood pressure and, perhaps, improve cardiac output. Low-dose dopamine also improved her renal function and enhanced the diuretic response to furosemide therapy.
DISCUSSION: Previous studies of dopamine in such patients were reviewed. Both dopamine infusion (4

Intravenous inotropic therapy can be used in patients with advanced heart failure, as palliative therapy or as a bridge to cardiac transplantation or mechanical circulatory support, as well as in cardiogenic shock. Their use is limited to increasing cardiac output in low cardiac output states and reducing ventricular filling pressures to alleviate patient symptoms and improve functional class. Many advanced heart failure patients have sinus tachycardia as a compensatory mechanism to maintain cardiac output. However, excessive sinus tachycardia caused by intravenous inotropes can increase myoca

BACKGROUND AND PURPOSE: Early goal-directed hemodynamic therapy is of particular importance for adequate cerebral circulation of patients with vasospasm after subarachnoid hemorrhage but is often precluded by the invasiveness of established cardiac output determination using a pulmonary artery catheter. This study was undertaken to validate the usefulness of less invasive goal-directed hemodynamic monitoring by transpulmonary thermodilution technique in patients after subarachnoid hemorrhage.
METHODS: One hundred sixteen patients with subarachnoid hemorrhage who underwent surgical clipping wi

BACKGROUND: Replacement of the aortic root with a composite graft containing a prosthetic mechanic valve is the preferred surgical procedure for tailoring the aortic root. The aim of this study is to determine the 5-year experience with the composite root replacement using our new modification of the Bentall technique.
METHODS: Between January 1996 and June 2001, 96 patients underwent aortic root replacement using a flanged composite graft. Eighty patients (83.3%) were male, and 16 patients (16.7%) were female with a mean age of 48.7 +/- 14.4 years. Indications for operation were a true or fa

A progressive myocardial dysfunction is known to occur in prolonged septic shock. The cases of two patients with acute meningococcal infection and Waterhouse-Friderichsen syndrome are reported. Hemodynamic measurements showed, in the early stage, the classical picture of hyperdynamic septic shock, with increased cardiac output and low systemic vascular resistances. However, rapidly progressive cardiac failure then occurred, with high filling pressures and low cardiac output. One patient went into irreversible shock. In the other the administration of inotropic agents was beneficial, with a slo

Sodium and water retention is characteristic of edematous disorders including cardiac failure, cirrhosis, nephrotic syndrome, and pregnancy. In recent years, the use of a sensitive radioimmunoassay for plasma vasopressin has implicated the role of nonosmotic vasopressin release in the water retention of these edematous disorders. In experimental studies and studies in man, it has been found that the nonosmotic release of vasopressin is consistently associated with the activation of the sympathetic nervous and renin-angiotensin-aldosterone systems. Moreover, the sympathetic nervous system has b

The effects of superior mesenteric arterial and intravenous infusions of vasopressin and low and high dose intravenous infusions of vasopressin on splanchnic and systemic hemodynamics were compared in 20 anesthetized dogs. The following parameters were evaluated: flow in the superior mesenteric artery and portal vein, portal and systemic blood pressure, and cardiac output. In the comparison of selective arterial and intravenous infusions, no statistically significant difference was found between the degree of changes in portal flow, portal and systemic blood pressure, and cardiac output. The occurrence of unanticipated and seemingly unexplicable major complications of hepatic, pulmonary, and cardiac dysfunction after palliative operation for obstructive hypertrophic cardiomyopathy prompted a review of 71 sequential patients. Fifty-five patients had been treated preoperatively with beta-blockers, calcium-channel inhibitors, or both, and 16 had received amiodarone for six to 566 days (mean time, 210 days) at total doses ranging from 8 to 175 g (mean dose, 82 g) and had drug-free intervals prior to operation of zero to 457 days (mean time, 91 days). Comparisons were made between

In this paper we describe clinical and hemodynamic data in ten patients with right ventricular infarction and low output syndrome. Atrioventricular block and supraventricular arrhythmias were a common finding. All of them had a right atrial pressure disproportionately increased (average 16.3 +/- 5.2 mm. Hg) in relation to left ventricular filling pressure (average 15 +/- 4.5 mm. Hg), and a very low cardiac index (average 1.42 +/- 0.45 liters/min./m.2). The right atrial pulse tracings were similar to those of constrictive pericarditis, showing a deep "y" descent in every patient. We made the di

A young man with previously unrecognized Graves' disease presented with atrial fibrillation and severe low-output heart failure due to dilated cardiomyopathy. The patient's cardiomyopathy resolved and cardiac function recovered shortly after hyperthyroidism and tachycardia were treated during hospitalization. The temporal relationship between heart rate and cardiac function during the recovery period suggests that chronic tachycardia may have been an important cause of his cardiac dysfunction. Thyrotoxicosis seemed to be directly responsible for the development of sustained supraventricular ta

Simultaneous Fick and duplicate dye cardiac outputs were done in 105 patients with various cardiovascular diseases during routine cardiac catheterization. Dye was injected into the pulmonary artery and sampled from the brachial artery. Nineteen patients had mitral and/or aortic valvular regurgitation. Eighty-four per cent of the duplicate dye cardiac outputs agreed within 10 per cent variation from the line of identity, and 98 per cent were within 25 per cent. There was no systematic difference between the Fick and dye methods. Seventy-five per cent agree within 20 per cent variation from the

Cardiogenic shock is a clinical syndrome characterized by cardiac pump failure, resulting in low cardiac output and subsequent tissue hypoperfusion. Current guidelines recommend diagnosing cardiogenic shock based on the presence of hypotension and signs of hypoperfusion. However, recent randomized clinical trials have demonstrated that these criteria are imperfect and may not reliably distinguish cardiogenic shock from other forms of shock, such as hypovolemic, distributive, or mixed types. Therefore, new clinical diagnostic criteria are urgently needed. A revised definition, grounded in the u

In order to evaluate the effect which the cronic, severe, and constant decrease of the preload of the left ventricle has on the contractile function of this chamber, 15 cases, with the strict technical conditions and permitted the planned measurements, were taken from 86 studies of ventricular function in patients with tight mitral stenosis. The following parameters were calculated; the ejection fraction, the maximum DP/dt, the zero point diastolic pressure, the final diastolic pressure, the diastolic AP, the mid-diastolic pressure, the diastolic compliance index, and the passive elastic modul

With the use of peripheral and pulmonary arterial catheters, the peripartum hemodynamic patterns of 10 patients with severe preeclampsia were studied. At the time of line placement, our results demonstrated elevated cardiac outputs (9.3 +/- 1.3 L/min), high systemic vascular resistances (1,042 +/- 160 dynes . sec . cm-5), and hyperdynamic ventricular function. Immediately post partum (less than 30 minutes), a transient decline in left ventricular function in association with a rise in central venous and wedge pressures was observed. By 1 hour after delivery, ventricular function had returned t

Primary pulmonary hypertension (PPH), in which no cause for a rise in pulmonary artery pressure can be found, is characterized by a "restriction" of the peripheral pulmonary vascular bed. Treatment is directed at the consequences of this, namely, the low cardiac output. Oral anticoagulants appear to improve prognosis, as in other low cardiac output states, by preventing intravascular coagulation. Oral vasodilators are not uniformly successful in improving cardiac output. Acute intravenous infusion of prostacyclin (PGI2) lowers both pulmonary and systemic vascular resistance in normal subjects

A 34-year-old man presented with heart failure (HF). He suffered opportunistic infections and was shown to be human immunodeficiency virus (HIV)-positive (viral load: 156,013 copies/mL) and have low CD4 lymphocytes (3/mm3), so he was initially treated for the opportunistic infections. Initial investigations showed high elevation of brain natriuretic peptide (BNP: 969 pg/mL). Transthoracic echocardiography showed an enlarged left ventricle (LV: 70 mm), a reduced LV ejection fraction (EF: 19%), but no LV hypertrophy or significant valvular diseases. After treatments for the infections, we starte

Cardiolipin (CL) is a structurally unique dimeric phospholipid localized in the inner mitochondrial membrane where it is required for optimal mitochondrial function. In addition to its role in maintaining membrane potential and architecture, CL is known to provide essential structural and functional support to several proteins involved in mitochondrial bioenergetics. A loss of CL content, alterations in its acyl chain composition, and/or CL peroxidation have been associated with mitochondrial dysfunction in multiple tissues in a variety of pathological conditions, including ischemia, hypothyro

We describe a rare case of Becker's muscular dystrophy (BMD) in a 28-year-old man complicated by rapidly progressing heart failure without apparent clinical signs of neuromuscular disease. He showed rhabdomyolysis, which repeatedly occurred causing acute renal failure as heart failure worsened. His serum creatine kinase (CK) level was generally below 300 IU/l. However, it exceeded more than 10,000 IU/l at the time of myoglobinuria. This suggests that the worsening of heart failure could induce rhabdomyolysis in a BMD patient. Gene analysis for BMD should be considered when the elevation of ser
