Catastrophic early drug eluting stents thrombosis and aspirin hypersensitivity.

Stent thrombosis is a feared complication of percutaneous coronary intervention. Promises and problems, late complications and early stent thrombosis have been reported after drug eluting stents implantation too. Moreover some patients with imperative cardiologic indications for combination therapy with aspirin and clopidogrel (stent placement and/or acute coronary syndrome) have a history of allergy to aspirin. We present a case of catastrophic early drug eluting stents thrombosis in a 79-year-old Italian woman with aspirin hypersensitivity.

Catastrophic intrauterine spinal cord injury caused by an arteriovenous malformation.

We report a baby born with flaccid quadraparesis that was considered possibly attributable to birth injury. Postmortem examination identified an injury that occurred well before delivery caused by hemorrhage from an arteriovenous malformation of the brainstem and spinal cord. We discuss imaging modalities to diagnose neonatal cord injuries, possible treatments, prognosis and end of life decision making for these unfortunate patients. We also emphasize the importance of the autopsy in cases of suspected birth injury.

Chronicle of a death foretold: a case of catastrophic vascular Behcet's disease.

A 20-year-old man with Behcet's disease characterized by recurrent arterial aneurysms presented with a new aortic root aneurysm. This patient previously had aneurysms of the coronary arteries and vein, as well as ruptured renal artery aneurysm. Chronic maintenance immunosuppressive therapy was recommended due to the catastrophic nature of the disease, which the patient refused to take. The patient died shortly after admission. This case demonstrates the unique catastrophic natural history of vascular Behcet's disease with recurrent life-threatening arterial events, and this case stresses the t

Testing factorial validity and gender invariance of the pain catastrophizing scale.

Recent research has indicated that the Pain Catastrophizing Scale (PCS) is a reliable measure that taps three dimensions of a singular construct. Gender differences have been found consistently in catastrophizing, with women reporting significantly higher scores than men on the PCS. This study was designed to cross-validate the factor structure of the PCS, independently for men and women, through second-order confirmatory factor analysis. Results indicate that the second-order models provide a good fit to the data. The conceptualization of catastrophizing as a singular latent construct, within

[Systemic lupus erythematosus and/or relapsing polychondritis with catastrophic antiphospholipid syndrome].

INTRODUCTION: Antiphospholipid syndrome is frequently associated with auto-immune disorders. EXEGESIS: We report the case of a 59-years-old woman with an antiphospholipid syndrome associated with systemic lupus erythematosus and/or relapsing polychondritis. CONCLUSION: This fatal evolution as a consequence of a delayed management shows that fast and invasive steps are required for diagnosis and treatment of systemic diseases.

Skew ocular deviation: a catastrophic sign on MRI of fetal glioblastoma.

CASE REPORT: We report the prenatal observation of fetal ocular skew deviation in a case of a glioblastoma found prenatally at 33 weeks' gestation by MRI. The fetal MRI, obtained by half-Fourier single-shot turbo spin echo (HASTE) pulse sequences, showed a tumor (80 mm in maximum diameter) in the right deep cerebral hemisphere with extension to the mid-cranial fossa and tentorial hiatus in addition to severe hydrocephalus. On coronal MR images a vertical dysconjugated gaze was recognized. One week after the MRI, frequent delayed deceleration prompted an emergency cesarean section. The baby was

[Catastrophic antiphospholipid syndrome and acute heart failure. Report of a case].

A 33 years old woman was admitted to the hospital after four days with cough, dyspnea, orthopnea and hemoptysis. Blood pressure was 170/90 mmHg, pulse was 112 and temperature was normal. She had cyanosis and a left ventricular gallop, without heart murmurs. A chest radiograph revealed pulmonary edema and echocardiogram showed a global left ventricular systolic disfunction. Oxygen and furosemide were started, but cardiopulmonary collapse ensued. The patient was supported with mechanical ventilation and treated with inotropic drugs. A right sided cardiac catheterization showed pulmonary wedge pr

Catastrophic reaction in acute stroke: a reflex behavior in aphasic patients.

Twelve patients with a catastrophic reaction (CR) (an outburst of frustration, depression, and anger when confronted with a task) were identified in a prospective cohort population (n = 326) with first-ever stroke admitted within 48 hours from onset. The authors' findings suggest that CR is a rare though not exceptional phenomenon in acute stroke and is associated with nonfluent aphasias and left opercular lesions. CR, poststroke depression, and emotionalism are distinct but related disorders.

Imaging findings in the rare catastrophic variant of the primary antiphospholipid syndrome.

We report imaging findings in a case of the rare catastrophic variant of antiphospholipid syndrome (CAPS) characterized by widespread microvascular occlusions, which may lead to multiple organ failure. We present a case of a 66-year-old woman with bone marrow necrosis, acute acalculous cholecystitis (AAC), focal liver necrosis, subtle patchy splenic infarctions, and bilateral adrenal infarction. The demonstration of multiple microvascular organ involvement (three or more) is crucial for the diagnosis of the catastrophic variant of APS. This can be performed radiologically intra-vitam. A catastrophic complication of systemic lupus erythematosus: massive mesenteric infarction.

The AA reports a case of a 64-year-old female patient affected for 3 years by systemic lupus erythematosus who developed a massive perforating mesenteric infarction. An immediate surgical treatment with resection of the small bowel and right colon followed by a side-to-side primary anastomosis between the remaining jejunum and transverse colon were performed. The patient died 15 days after surgery due to recurrence of a mesenteric and pulmonary thromboembolia.

Children's well-being 11 years after the Chornobyl catastrophe.

BACKGROUND: The psychological effects of technological disasters have rarely been studied in children. This study assessed the aftermath of the 1986 Chornobyl disaster in children evacuated to Kyiv from the contaminated zone surrounding the nuclear power facility. METHODS: In 1997, we evaluated three hundred 10- to 12-year-old children in Kyiv who were in utero or infants at the time of the disaster and who had resided near Chornobyl (evacuees) and 300 sex-matched homeroom classmates who had never lived in a radiation-contaminated area. Response rates were 92% (evacuees) and 85% (classmates).

Catastrophic familial rhabdomyolysis: compartment syndrome with muscle fiber regeneration.

Two reports of patients with rhabdomyolysis are described. Patient 1 was a 4-year-old-girl who had a 48-hour history of pyrexia and a 24-hour history of vomiting, drowsiness, polydipsia oliguria, and back pain. She could not walk easily because of tenderness in the calves. She was treated with furosemide and dopamine. On day 9, she was mobilized with the aid of physiotherapy. After 2 years, she tired easily but could walk normally. Patient 2 was a 3-year-old girl who had a 24-hour history of general malaise, peripheral aches and pain, and increasing drowsiness. She had severe swelling in the c

Aortoenteric fistulas. Recognizing a potentially catastrophic cause of gastrointestinal bleeding.

Aortoenteric fistulas are a relatively rare but serious cause of massive gastrointestinal hemorrhage. Most occur as a consequence of aortic reconstructive surgery and involve the proximal graft anastomosis. The distal duodenum is the site of bleeding in about three fourths of cases. Most patients have an initial episode of bleeding followed hours to weeks later by catastrophic hemorrhage. Patients with gastrointestinal bleeding who have undergone prior aortic reconstructive surgery should be approached with a great sense of urgency and a high index of suspicion. Endoscopic and radiographic stu

Hemorrhagic shock and encephalopathy. Clinical definition of a catastrophic syndrome in infants.

We treated nine infants who unexpectedly developed shock, seizures, and fever, followed by diarrhea, consumption coagulopathy, and hepatorenal dysfunction. Despite vigorous treatment, three infants died and all except one of the six survivors have severe residual neurologic abnormalities. Postmortem findings included cerebral edema, white matter petechial hemorrhages, gastrointestinal hemorrhages, and fatty liver. These clinicopathologic features are similar to those previously described in 10 infants as being due to hemorrhagic shock and encephalopathy, except for the presence of fatty liver

[Diagnostic and therapeutic measures in acute catastrophic bleeding esophageal varices].

Principles of management of bleeding esophageal varices are 1. fluid therapy of bleeding shock, 2. prevention of hepatic coma, 3. emergency endoscopy, 4. balloon tubes (Senkstaken-Blakemore, Linton-Nachlas), and 5. with some restriction, selective infusion of vasopressin into the a. mesenterica superior. If these procedures fail, sclerosing of esophageal varices stops bleeding in more than 90% of the cases. Bleeding from varices of the gastric fundus may be stopped by gastro-esophageal disconnection (Pettinari-Hassab). Both procedures have with 15% and 25% respectively, the lowest mortality. P

Idiopathic Spontaneous Cervical Epidural Hematoma: A Sudden Neurological Catastrophe.

Spontaneous cervical epidural hematomas (SCEHs) are unique in both their rarity and devastating associated complications. Through this report, the case of a male in his mid-50s with minimal past medical history who developed an acute onset of neck pain secondary to a SCEH with progressive left-sided paralysis is detailed. In doing so, a discussion about the underlying mechanisms of SCEH, or lack thereof, is provided. Emphasis is placed on the patient's clinical presentation, imaging, neurosurgical intervention, and neurological recovery. It is imperative to identify SCEH and provide emergent i
