Stable injuries of the cervical spine can be treated with a cervical collar. Unstable injuries that do not require repositioning can be immobilized with a Minerva plaster cast. Crutchfield's extension is used for extension of short duration during repositioning before setting a cast or performing surgery and for long-term extension in cases of transverse lesions of the spinal cord. The Halo apparatus is especially suitable for treatment requiring extension of long duration when there are no symptoms of transverse lesions of the spinal cord.

OBJECTIVE: Ponticulus posticus is an anomaly of the atlas. Odontoid fracture type 2 fracture occurs by hyperflexion, hyperextension, lateral flexion, and a combination of these movements. The presence of Ponticulus Posticus anomaly may affect the occurrence of odontoid fractures of the C2 vertebra. In this study, the relationship between the presence of PP anomaly and occurring of the odontoid type 2 fracture was investigated.
PATIENTS AND METHODS: Cervical CTs of a total of 14 patients with odontoid type 2 fracture were retrospectively evaluated for PP, and compared with 13 patients with cer

BACKGROUND: To introduce a novel transoral instrumentation in the treatment of unstable fractures of the atlas.
METHODS: From January 2008 to May 2018, 22 patients with unstable C1 fractures who received Jefferson-fracture reduction plate (JeRP) via transoral approach were retrospectively analyzed. The case history and the radiographs before and after surgery were noted. The type of fracture, the reduction of the fracture, and position of the internal fixation were assessed through preoperative and postoperative CT scans.
RESULTS: All 22 patients successfully underwent anterior C1-ring osteo

UNLABELLED: Compression of the caudal medulla oblongata and ventral portions of the spinal cord is the most dangerous complication of atlanto-axial dislocation (AAD).
AIM: The study objective was to improve surgical management of patients with ventral compression of the spinal cord in the setting of AAD of various genesis.
MATERIAL AND METHODS: We analyzed treatment outcomes in 250 patients with C1 and C2 injuries and diseases for the period between 2002 and 2016. Persistent ventral compression of the neural structures in the setting of AAD was detected in 34 (13.6%) patients. Anterior or po

We present a unique variant of C1 fracture, which, at the best of our knowledge, has never been previously reported. This lesion consists in a lateral mass atlas fracture with a longitudinal separation of the transverse ligament that remains functionally preserved. We considered this lesion to be stable and treated the patient conservatively with good recovery. Even if many classifications of atlas fractures have been proposed, none of them ever described this uncommon variant. We debate about biomechanical aspects and therapeutic implication of the reported case.

A 45 year old male was admitted to the hospital for unsteady gait and dizziness. The brain CT showed bilateral cerebellar ischemic stroke. The MRI corroborated the stroke in the cerebellum with a third acute ischemic stroke in the pons. The laboratory work up showed a positive anti beta2-glycoprotein I antibodies. The cerebral angiogram showed the territories occluded and also a rare malformation of the cranium vertebral junction called assimilation of the atlas. We concluded that due to the turbulence created in the V3 segment of the vertebral artery surrounding the atlas, the presence of the

An unusual fracture of the axis was best displayed using three-dimensional reconstruction. The fracture was noted to differ significantly from the usual "hangman's fracture" as it involved the body of C2 with subluxation of the anterior part of C2 forward on C3 and without "decompression" of the spinal canal at the C2 level. Better understanding made feasible by three-dimensional reconstruction had important treatment implications in this case.

Unilateral posterior arch fractures of the atlas are discussed with two clinical examples and an experimental study of their mechanism. Laboratory fracturing of posterior arches of atlas specimens with a specially adapted universal testing machine produced nonsimultaneous fractures of the two sides in four of six specimens. In three of these specimens, a complete fracture on one side was temporarily displaced because the orientation of the leverage acting on the other side changed from sagittal to oblique. The consequent increase in the effective length of the lever arm reduced the angular def

Jefferson fractures in children are exceedingly rare. A case in a 2-year-old is reported, and the other only case in the literature is reprinted. The mechanism of fracture is probably separation of the lateral masses by a force transmitted from the vertex. Because these fractures are not usually associated with neurological deficits, and because of the poor detail obtained with conventional radiography in a child, these fractures may be overlooked. Computerized tomography provides superb definition of this lesion. The preferred mode of treatment is immobilization.

PURPOSE OF THE STUDY: Injuries of atlas account for 1-2% of all injures of the spine and for 7% of the injuries of the cervical spine. Fractures of atlas occur either as isolated fractures or in combination with the injury of the axis or occipital condyles. The aim of the work is to evaluate a group of patients with the injury of the atlas treated both conservatively and surgically.
MATERIAL: Between January 1996 and October 2001 we treated at the Orthopaedic Department of 3rd Medical Faculty, Carles University in Prague 10 patients with the injury of atlas. Between November 2001 and December

BACKGROUND: Ponticulus posticus (PP) occurs frequently and may cause symptom series, including vertebrobasilar insufficiency, migraine, hearing loss, and Barré-Liéou syndrome. However, few studies to date have described surgical treatment of PP. We report a rare case of a patient who suffered from torticollis, facial asymmetry, localized pain, and Barré-Liéou syndrome in connection with PP. We also review the pertinent literature, focusing on surgical treatment for symptoms due to PP.
CASE PRESENTATION: A 23-year-old male presented with the chief complaint of continuous significant dizziness

This paper is Part II of a two-part report. Part I of the report covered atlanto-occipital dislocation or dissociation, and isolated condylar fractures. This part of the report covers isolated and combination fractures of the atlas and axis.
