BACKGROUND: The array of symptoms that characterize thoracic outlet syndrome (TOS) often lead to a failure or delay in diagnosing this condition in persons who are physically active.
METHODS: Using the key words and phrases "thoracic outlet syndrome," "sport," "exercise," and "athlete," the MEDLINE files from 1991 to April 1996 were searched. Articles dating before 1991 were accessed by cross-referencing the more recent articles.
RESULTS AND CONCLUSIONS: TOS results from compression of the neural or vascular structures of the upper extremity at the thoracic outlet. Clinical manifestations ca

Mycoplasma pneumoniae infection has been associated with a variety of neurologic complications involving the central nervous system, the peripheral nervous system and muscle. We present a patient who developed a previously unreported complication: mononeuritis multiplex. This consisted of a severe brachial plexus neuropathy with contralateral cervical monoradiculopathy.

Variations regarding cervical musculature have been well documented in the literature, including the presence of a levator claviculae muscle within the lateral neck. This case report presents an example of levator claviculae that was discovered during the prosection of a 98-year-old male anatomical donor. This accessory muscle was located in the left cervical region and originated from the posterior belly of the levator scapulae, traversed along the anterior border of the trapezius, and inserted onto the inferior clavicle. Branches of the cervical plexus were noted to innervate both the levato…

Basic studies were carried out to apply frozen allogeneic nerve grafts in dogs after wide-ranging defects of the brachial plexus due to surgical resection of tumor. In this study, morphological variations in branching patterns of the brachial plexus were examined in ten beagle dogs, to evaluate whether the brachial plexus might represent a useful source of allogeneic nerve grafts. Spatial relationships between the axillary lymph node, which had the possibility of carcinomatous metastasis, and the musculocutaneous (MC) nerve, which was important for the function of the forelimbs, were also inve

PURPOSE: This study demonstrates the physiological changes of the cerebral venous outflow routes in healthy humans in the recumbent and the sitting position employing positional MRI.
METHODS: In five volunteers, the internal jugular veins and the cervical vertebral plexus were analyzed in the supine and sitting position using an open MR system. Axial T2-weighted scans and axial T1-weighted flow sensitive gradient echo sequences were acquired. The findings were compared to previously published anatomic descriptions from cadaver preparations.
RESULTS: In the supine position, the internal jugul

Short-latency somatosensory evoked potentials (SSEPs) were obtained in a 61-year-old patient with a cervical spinal cord glioma extending from C3 to C7. SSEPs in response to tibial nerve stimulation were normal; however, all components of the median nerve responses were markedly abnormal bilaterally (except for normal brachial plexus responses). This selective abnormality of median nerve SSEPs has not been reported previously in a patient with cervical cord tumor associated with a syrinx and illustrates the use of the SSEP in the anatomical localization of the lesion.

Perioperative brachial plexus injury has become relatively rare, as preventive positioning strategies are now widely implemented. We report the case of a female patient who underwent bilateral breast reconstruction with bilateral deep inferior epigastric perforator flaps for bilateral breast cancer and developed left upper limb neuropathy, despite careful intraoperative positioning measures. General anesthesia was induced, and the operative time was 10 hours and 36 minutes. The patient was positioned supine, with both upper limbs abducted to approximately 90 degrees and secured to arm boards w

Experience with median nerve SEPs in the diagnosis of brachial plexus lesions is analysed in 49 patients selected from a total material of 264 cases with brachial plexus problems tested by SEP techniques. Median nerve SEPs were always compared with the results of SEPs after stimulation of at least one other nerve relevant to the site of the lesion as suspected clinically and electromyographically. All patients presented with unilateral brachial plexus problems and all root lesions were verified by clinical presentation, EMG studies, myelogram or surgery. There were 19 brachial plexus injuries,

BACKGROUND: Nerve reconstruction strategies for restoration of elbow flexion and shoulder function in patients with neonatal brachial plexus palsy with neurotmesis of C5 and avulsion of C6 are not well defined and the outcomes are unclear.
METHODS: From 1990 to 2008, nerve surgery was performed in 421 patients with neonatal brachial plexus palsy. This study focused on thirty-four infants who had a neurotmetic lesion of C5 and avulsion or intraforaminal neurotmesis of C6, irrespective of C7. The C8 and T1 functions were intact. Intraplexal transfer of C6 to C5 with direct coaptation was prefer

Administration of adequate volumes of local anaesthetic through a single injection into the interscalene space produces blockade of both the brachial and cervical plexuses. A report of 2 patients who underwent operative repair of recurrent dislocation of the shoulder under interscalene block is presented. The advantages and disadvantages of the block for surgery around the shoulder region, and the possible complications are discussed.

Schwannomas, arising from the brachial plexus, are rare nerve sheath tumors originating from Schwann cells, affecting peripheral nerves. Brachial plexus schwannomas present unique diagnostic and surgical challenges due to their rarity and intricate anatomy. This case report focuses on a significant case of a left brachial plexus schwannoma extending into the pleural cavity, emphasizing the complexities in diagnosis and the use of video-assisted thoracoscopic surgery (VATS) for intrathoracic extensions. A 71-year-old female with a two-year history of left arm pain and numbness presented, having

BACKGROUND CONTEXT: As recovery from brachial neuropathy after C5 or more extensive palsies after cervical spine surgery is generally favorable and may occur even beyond 2 years, early surgical interventions (eg, nerve transfers) may risk overtreatment. Estimates of recovery probability over time may help guide decisions about the timing and potential benefits of additional surgery. Bayesian analysis is helpful in estimating this probability, as it estimates the probability of recovery and updates it when new information becomes available, such as when nerve recovery does not occur over time.

BACKGROUND: Neuropathic pain after nerve root or plexus avulsion injury is disabling and often refractory to medical therapy. Dorsal root entry zone (DREZ) lesioning is a neurosurgical procedure that disrupts the pathological generation and transmission of nociceptive signaling through the selective lesioning of culprit neurons within the dorsal horn of the spinal cord.
OBJECTIVE: To present a case report and describe the operative technique for cervical spinal cord DREZ lesioning using radiofrequency thermocoagulation.
METHODS: We present the case of a 29-year-old man who experienced a trau

Brachial plexus is complex network of nerves, formed by joining and splitting of ventral rami of spinal nerves C5, C6, C7, C8, and T1 forming trunks, divisions, and cords. The nerves emerging from trunks and cords innervate the upper limb and to some extent pectoral region. Scanty literature describes the variations in the formation of cords and nerves emanating from them. Moreover, the variations of cords of brachial plexus and nerves emanating from them have iatrogenic implications in the upper limb and pectoral region. Hence study has been carried out. Twenty-eight upper limbs and posterior

In a five year period we have treated surgically eight patients with TOS in the Neurosurgical Clinic Sarajevo. We have concluded that there is not specific diagnostic procedure for the verification of TOS and as surgical method of choice we suggest supraclavicular scalenectomy with neurolysis of plexus brachialis and resection of cervical rib, if exists.

OBJECTIVE: The transaxillary approach to thoracic outlet decompression in the presence of cervical ribs offers the advantage of less manipulation of the brachial plexus and associated nerves. This may result in reduced incidence of perioperative complications, such as nerve injuries. Our objective was to report contemporary data for a series of patients with thoracic outlet syndrome (TOS) and cervical ribs managed through a transaxillary approach.
METHODS: We reviewed a prospectively maintained database for all consecutive patients who underwent surgery for TOS and who had a cervical rib. Sym
