Thirty-Day Outcomes Following Surgical Decompression of Thoracic Outlet Syndrome.

BACKGROUND: Surgical thoracic outlet syndrome (TOS) management involves decompression of the neurovascular structures by releasing the anterior and/or middle scalene muscles, resection of the first and/or cervical ribs, or a combination. Various surgical approaches (transaxillary, supraclavicular, infraclavicular, and transthoracic) have been used with varying rates of complications. The purpose of this study was to evaluate early postoperative outcomes following surgical decompression for TOS. We hypothesized that first and/or cervical rib resection would be associated with increased 30-day c…

A "hard" neck lump.

We discuss a case of an asymptomatic cervical rib presenting as a hard neck lump in a 56-year-old woman. Cervical rib is a common condition associated with thoracic outlet syndrome. The diagnosis, investigation and management of this condition is an important differential diagnosis that an ENT surgeon should know in a patient presenting with a neck mass.

Regionalization of axial skeleton in the lungfish Neoceratodus forsteri (Dipnoi).

Differentiation of the axial skeleton into distinct regions, once thought to be characteristic of the Tetrapoda, also occurs in the actinopterygian Danio rerio. In these taxa, the boundary between the cervical-thoracic regions correlates with Hoxc6 expression and morphological features such as position of the pectoral fin and associated nerves, and the absence of ribs. In the lungfish Neoceratodus, a member of the extant sister taxon to the Tetrapoda, the first vertebral element to chondrify is situated well posterior to the skull, developing from somites 6 and 7 (6/7) and associated with an e

Histology shows that elongated neck ribs in sauropod dinosaurs are ossified tendons.

The histology of cervical ribs of Sauropoda reveals a primary bone tissue, which largely consists of longitudinally oriented mineralized collagen fibres, essentially the same tissue as found in ossified tendons. The absence of regular periosteal bone and the dominance of longitudinal fibres contradict the ventral bracing hypothesis (VBH) postulated for sauropod necks. The VBH predicts histologically primary periosteal bone with fibres oriented perpendicular to the rib long axis, indicative of connective tissue between overlapping hyperelongated cervical ribs. The transformation of the cervical…

The failed operation for thoracic outlet syndrome: the difficulty of diagnosis and management.

In 225 patients requiring reoperation for recurrent thoracic outlet syndrome, "pseudorecurrences" were noted in 43 patients not relieved of symptoms after the initial operation. Such recurrences were associated with technical errors at the initial operation including resection of the second rib instead of the first, resection of the first with a cervical rib left in place, or resection of a cervical rib with an abnormal first rib left. True recurrences occurred in 182 patients, 154 of whom had a substantial piece of rib remaining from the initial procedure. Indications for reoperation included…

[Thoracic outlet syndrome in a child: a rare pediatric diagnosis].

Thoracic outlet syndrome (TOS) is a rare pediatric syndrome and few cases have been reported in the literature. The authors report the case of a 10-year-old boy with generalized arm pain and neck stiffness. The angiocomputed tomography revealed the presence of a TOS caused by an additional cervical rib and stenosis of the sub-clavian artery. The resection of this rib relieved the patient of his pain during a full year following surgery. This case reminds us that children can also be affected by TOS. When a cervical rib causes TOS, a resection is often necessary.

Treatment of Thoracic Outlet Syndrome Complications Assisted with a Cerebral Embolism Protection Device.

We report a case of a stroke and upper limb ischemia in a 27-year-old female secondary to a right cervical rib and retrograde thromboembolization. Follow-up showed complete patency of the vessels after thrombectomy and internal carotid artery stenting followed by transbrachial embolectomy performed with a cerebral protection device. The cervical rib was surgically removed to prevent additional events.

Arterial complications of thoracic outlet syndrome.

Arterial complications of thoracic outlet compression have serious potential implications; however, these complications rarely appear. Between 1990 and 2006, prospectively collected data on 27 patients with arterial complications of thoracic outlet syndrome were analyzed. The causes of arterial compression were cervical rib (20 [74.1%]), abnormalities of the first thoracic rib (three [11.1%]), soft tissue anomalies (two [7.4%]), and hypertrophic callus after clavicle fracture (two [7.4%]). In all cases, a combined supraclavicular and infraclavicular approach was used. Decompression was achieve…

Thoracic outlet syndrome: influence of personal history and surgical technique on long-term results.

OBJECTIVE: Long-term results after surgery for thoracic outlet syndrome (TOS) are reviewed in terms of personal histories and surgical techniques. METHODS: Forty-eight operations were performed in 37 patients. In 21 instances, the picture was one of ordinary TOS, in eight TOS was traumatic and in nine the picture was sub-acute. Cervical ribs were excised through a supraclavicular approach (in seven cases), and first ribs through transthoracic, transaxillary or supraclavicular approaches (in 25, 15 or one, respectively). Long-term follow-up was obtained in 41 cases and averaged 11.7 years. RE…

SUPRACLAVICULAR FIRST RIB RESECTION FOR TREATMENT OF THORACIC OUTLET SYNDROME.

Twelve patients with thoracic syndrome were operated during a 15 month period. Eleven patients had features of neurogenic thoracic outlet syndrome, while one presented with arterial ischemia due to distal embolism. The diagnosis was based on the characteristic history and positive stress tests. Cervical rib was present in 7 patients. Abnormal nerve conduction studies were present in 7 out of 8 cases. Supraclavicular first rib resection was done in all patients in view of the severity of symptoms. If present, the cervical rib was also excised. There was no major operative complication. Eleven o…

Evaluation of patients with traumatic vertebral fractures and accompanying rib fractures in terms of complication development and need for surgery.

BACKGROUND: The aim of this study was to evaluate patients with traumatic vertebral fractures and accompanying rib fractures in terms of complication development and need for surgery. METHODS: Between January 2019 and September 2022, a total of 173 patients (138 males, 35 females; mean age: 53.0±18.5 years; range, 17 to 95 years) who had vertebral and accompanying rib fractures due to blunt and penetrating trauma were retrospectively examined. The patients were divided into three groups: upper cervical (n=8), lower cervical (n=31), and thoracolumbar (n=134). Among the groups, trauma mechanism

Reflections upon the aetiology of congenital pseudarthrosis of the clavicle. With a note on cranio-cleido dysostosis.

The cause of pseudarthrosis of the clavicle is obscure. Right-sidedness is an almost constant feature. We have proposed that the lesion is sometimes due to pressure upon the developing clavicle by the subclavian artery which is normally at a higher level on the right side. This may be accentuated in the presence of cervical ribs or unduly elevated first ribs, both of which we have observed in association with pseudarthrosis. We have also noted pseudarthrosis on the left side in association with dextrocardia (when the relative positions of the subclavian arteries are reversed) and in the presen…

Thoracic outlet syndrome (TOS) mimicking Takayasu's arteritis--case report.

Thoracic outlet syndrome (TOS) is defined as a set of symptoms caused by the compression of the brachial plexus and subclavian vessels in the thoracic outlet region. Anomalies in musculoskeletal structures may be responsible for TOS, including prolonged transverse process of the seventh cervical vertebra, cervical rib, and first anomalous rib and clavicle fractures. The authors describe a case of a young woman with pain in the left forearm, accompanied by intermittent claudication, weigh loss, myalgias and ischemic lesions in the fingers, with no pulses and no measurable blood pressure in the …

[Thoracic outlet syndrome].

Results of surgical treatment for thoracic outlet syndrome in 7 patients are reviewed. The symptom has been caused by cervical rib in 5 cases, in 1 case by the first rib anomaly and in 1 case by disturbed attachment of m.scalenus anterior. A case of misdiagnosed cervical rib is described in details; the patient has been operated on peripherally and serious iatrogenic complications followed. Ineffectiveness of pharmacotherapy and physiotherapy is pointed out. Surgical treatment resulted in remission of symptoms in 6 cases; in case of patient with iatrogenic complications marked improvement has …

Thoracic outlet syndrome in an adolescent with bilateral bifid ribs.

Reported here is a 13-year-old girl suffering from pain in her right lower neck region and the shoulder for the preceding 2-3 years. She was diagnosed to have right thoracic outlet syndrome due to bifid ribs--caused by the bony fusion of the variant cervical rib. She was operated via supraclavicular approach, whereby the variant cervical rib and the fibrous bands were excised. She was found to be quite improved 1 month after surgery. Thoracic outlet syndrome, a rare disease of childhood, is discussed in the light of the relevant literature.

Thoracic outlet syndrome in children.

Thoracic outlet syndrome, although uncommon in children, may be precipitated during rapid growth in adolescence, especially in association with a cervical rib. Four children exhibited the aching, tiredness in the limb, and occasional paresthesia in adolescence. Two had a cervical rib requiring removal, after which the symptoms disappeared. The other two were treated nonoperatively with shoulder-strengthening exercises and avoidance of posturing that elicited the symptoms. It is proposed that in a child, the anatomy of the thoracic outlet is constantly changing with growth and development of th…

[Thoracic outlet syndrome. A study of 45 cases treated between 1975 and 1993].

Since 1970 we have experience of more than 100 cases of the thoracic outlet syndrome. We have rewied 45 patients operated on between 1975 and 1993.The cause, in agreement with the literature, was in 30% a road accident (cervical spine and clavicular disease), in 54% malformations (cervical rib) with a similar frequency of involvement with neurological pathology of the upper limb.We always found a vascular symptomatology, wich increased in shoulder abduction. In 82% of the cases we found an associated neurological deficit. The diagnosis was confirmed with electromyography and arteriography. The …
