Principles of Level Selection for Instrumentation in Adult Cervical Spinal Deformity Surgery: Guidelines for Treatment.

➢ For isolated cervical deformities in patients with good bone health, the upper instrumented vertebra can be C3 or C4.➢ For patients with global cervical deformity, poor bone health, planned 3-column osteotomies, or long fusion constructs, C2 may be an appropriate upper instrumented vertebra.➢ For patients with focal kyphosis, optimal sagittal alignment, and preserved bone stock, a lower instrumented vertebra cranial to the cervicothoracic junction is appropriate.➢ For patients with poor cervical sagittal alignment, a lower instrumented vertebra in the proximal thoracic spine, ranging from T1

Bone mineral density of human female cervical and lumbar spines from quantitative computed tomography.

STUDY DESIGN: This study determined bone mineral density (BMD) of cervical, thoracic, and lumbar vertebrae in healthy asymptomatic human sub … 2-L4) spines, mean BMD was 260.8 +/- 42.5, 206.9 +/- 33.5, and 179.7 +/- 23.4 mg/mL. Average BMD of cervical vertebrae was higher than (P < 0.0001) thoracic and lumbar spines. Correlations between BMD and level indicated the lowest r value for T1 (0.42); in general, the association was the strongest in the lumbar spine (r = 0.89-0.95). The cervical spine also respon …

Isolated C5 Vertebrae Dislocation with Trauma: An Extremely Rare Case of Isolated C5 Dislocation.

BACKGROUND Total spondylolisthesis, or dislocation of 1 cervical vertebrae, is only caused by high-energy trauma and is usually fatal. Cervical spine fractures and dislocations often cause 3-column structural damage to the cervical spine, injury to the spinal cord, and precipitating alignment of the cervical vertebrae, as well as cervical instability, which are detrimental, show poor prognosis, and are associated with high rates of mortality rate and disability. CASE REPORT We report an extremely rare case of isolated C5 dislocation caused by falling out of a tree, with sudden tetraplegia. CON…

Breaking evolutionary and pleiotropic constraints in mammals: On sloths, manatees and homeotic mutations.

BACKGROUND: Mammals as a rule have seven cervical vertebrae, except for sloths and manatees. Bateson proposed that the change in the number of cervical vertebrae in sloths is due to homeotic transformations. A recent hypothesis proposes that the number of cervical vertebrae in sloths is unchanged and that instead the derived pattern is due to abnormal primaxial/abaxial patterning. RESULTS: We test the detailed predictions derived from both hypotheses for the skeletal patterns in sloths and manatees for both hypotheses. We find strong support for Bateson's homeosis hypothesis. The observed ver…

Vertebral formula and congenital abnormalities of the vertebral column in rabbits.

The aim of this retrospective study of 330 rabbits (164 males, 166 females) was to determine different vertebral formulas and prevalence of congenital vertebral anomalies in rabbits from radiographs of the cervical (C), thoracic (Th), lumbar (L) and sacral (S) segments of the vertebral column. The number of vertebrae in each segment of vertebral column, position of anticlinal vertebra and localisation and type of congenital abnormalities were recorded. In 280/330 rabbits (84.8%) with normal vertebral morphology, seven vertebral formulas were identified: C7/Th12/L7/S4 (252/330, 76.4%), C7/Th12/

[Age-related changes in the cervical spine studied based on the results of a quantitative analysis of its structural-functional characteristics].

Quantitative parameters characterizing the shape and interlocation of the vertebrae were determined in roentgenograms of 60 healthy persons and of 61 patients suffering from chronic pain syndrome caused by cervical osteochondrosis. The calendar age of the persons examined has correlated mostly with a group of parameters characterizing the shape of vertebrae (the highest correlation was with the length of vertebrae). Using the multiple correlation method the authors have worked out highly reliable (p < 0.00005) regressive models of the calendar age of healthy persons and have confirmed the incr

Suicidal hanging resulting in complete decapitation--forensic, radiological, and anthropological studies: a case report.

The authors report a case of complete posthanging decapitation. The decapitated corpse lay against a pillar of a road bridge. The head had rolled 5 m from the trunk. The bridge was 7.2 m above the road level. The rope was 3.6 m long, its lower end was 3.6 m from the ground and its diameter was 10 mm. The noose used was a slip knot. Plain X-rays of the skull and cervical spine were obtained. The skull X-rays showed air in the meningeal spaces, in both lateral and third ventricles. The severance plane of the cervical spine was between the third and the fourth cervical vertebrae. No other cervica…

Vertebral body deformation causing Wobbler syndrome in a Great Dane.

This paper reports a case of cervical vertebral body deformation with gradually worsening posterior paresis in a Great Dane. The ventrocranial part of the body of the seventh cervical vertebra was severely eroded and deformed. This vertebra was tilted in relation to the sixth cervical vertebra producing a stenosis of the spinal canal which was demonstrated by myelography. Spondylolisthesis of the second type of G e a r y's classification was diagnosed. 99mTc-polyphosphate showed no concentration in the affected vertebra as would be found where bony tissues of different maturities are present,

Anatomical and Radiological Findings of Abnormal Calcified Structures within and around the Foramen Transversarium and Its Clinical Implications.

BACKGROUND: The foramen transversarium is a vital anatomical structure found in the cervical vertebrae of the spine. Typically, it serves as a passageway for important neurovascular structures, including the vertebral artery and vein, as well as the vertebral nerve. However, abnormal calcification or ossification of soft tissues in and around this area can lead to various clinical implications. Understanding the presence and implications of abnormal ossified structures in and around the foramen transversarium is crucial for clinicians involved in the diagnosis and management of cervical spine …

Computed tomography study of implantation corridors in canine vertebrae.

OBJECTIVES: To define the characteristics of optimum implantation corridors in vertebral bodies C2 to C7 and T10 to S1, using computed tomography examination. METHODS: Measurements were taken from 207 vertebrae from 35 different adult dogs. RESULTS: Implantation corridors of the cervical vertebrae are narrow. The width preserving the transverse hole is less than 2.5 mm in 68.6 per cent of the 86 vertebrae C2 to C6. Dorsal implantation corridors of the last four thoracic vertebrae are narrow, and major anatomical structures are very close to their emergence point. In 63 per cent of the 40 tho…
