How do we improve maternal and child health outcomes in Ghana?

Maternal and infant mortality includes a number of health challenges in Ghana, with outcomes among the worst in the subregion and the world. … al care services in particular. However, a number of challenges still hinder access to maternal and child health outcomes. Poor infrastructure, human resource challenges, poor access to essential medicines, poor quality of care, and superstitious and cultural beliefs have been noted in the literature. We suggest that while providing the necessary human and fin …

Health Professionals: Identifying and Reporting Child Physical Abuse-a Scoping Review.

Health professionals play a significant role in identifying and reporting child physical abuse (CPA). However, several studies have pointed out non-reporting behavior among these professionals, evidencing difficulties identifying and reporting suspected cases. This review aimed to explore the frequency and possible barriers in identifying and reporting CPA by health professionals worldwide and to identify associated factors. This scoping review was conducted in the Pubmed, Web Of Science, Scopus, and SciELO databases between July 2019 and December 2020. Analytical and qualitative observational

Climate Change Imperils Pediatric Health: Child Advocacy Through Fossil Fuel Divestment.

Climate change poses an existential threat to children's health. Divestment of ownership stakes in fossil fuel companies is one tool available to pediatricians to address climate change. Pediatricians are trusted messengers regarding children's health and therefore bear a unique responsibility to advocate for climate and health policies that affect children. Among the impacts of climate change on pediatric patients are allergic rhinitis and asthma; heat-related illnesses; premature birth; injuries from severe storms and fires; vector-borne diseases; and mental illnesses. Children are dispropor

Child incarceration and long-term adult health outcomes: a longitudinal study.

Purpose Although incarceration may have life-long negative health effects, little is known about associations between child incarceration and subsequent adult health outcomes. The paper aims to discuss this issue. Design/methodology/approach The authors analyzed data from 14,689 adult participants in the National Longitudinal Study of Adolescent to Adult Health (Add Health) to compare adult health outcomes among those first incarcerated between 7 and 13 years of age (child incarceration); first incarcerated at>or=14 years of age; and never incarcerated. Findings Compared to the other two group

Child maltreatment and mental health problems in adulthood: birth cohort study.

BACKGROUND: Retrospective studies have shown a high association between child abuse and subsequent psychiatric morbidity. Prospective studies are rarer.AimsTo examine, using a prospective record-linkage analysis, whether substantiated child maltreatment is associated with adverse psychological outcomes in early adulthood. METHOD: The participants were 3778 mother and child pairs enrolled in a population-based birth cohort study in Brisbane, Australia. Exposure to suspected child maltreatment was measured by linkage with state child protection agency data. The primary outcomes were the interna

Child abuse: an underestimated health problem? A case report from Cameroon.

Physical abuse of children can be of varying nature. Children can be shaken, beaten, burnt, or cut by their parents or guardians. The incidence of trauma inflicted on children is underestimated in many countries, probably because clinical signs are misjudged, as this case report shows. This three-year-old boy was seen in several health facilities of Yaounde, Cameroon. He was treated erroneously for severe malaria and then for meningitis because he presented with neurologic signs, before the diagnosis of child abuse was made. This was confirmed after an imaging work-up revealed fractures of the

Maternal nutrition interventions to improve maternal, newborn, and child health outcomes.

Maternal undernutrition affects a large proportion of women in many developing countries, but has received little attention as an important determinant of poor maternal, newborn, and child health (MNCH) outcomes such as intrauterine growth restriction, preterm birth (PTB), and maternal and infant morbidity and mortality. We recently evaluated the scientific evidence on the effects of maternal nutrition interventions on MNCH outcomes as part of a project funded by the Gates Foundation to identify critical knowledge gaps and priority research needs. A standardized tool was used for study data ab…

Vitamin D and child health: part 2 (extraskeletal and other aspects).

The first part of this review focused on the skeletal aspects of vitamin D. This second part reviews some of the available evidence that vitamin D may have a physiological extraskeletal role beyond its traditional effect on the skeleton. This aspect has influenced the definition of vitamin D deficiency and what level of vitamin D should be regarded as optimal. The recognition of the prevalence of vitamin D deficiency and insufficiency has led to debate as to whether and how we should be treating asymptomatic individuals. This review discusses the potential extraskeletal effects of vitamin D, t

Blood sample volumes in child health research: review of safe limits.

OBJECTIVE: To determine paediatric blood sample volume limits that are consistent with physiological "minimal risk." METHODS: A literature review was performed to search for evidence concerning the adverse effects of blood sampling in children and for guidelines on sampling volume in paediatric research. The search included Medline, EMBASE, other web-based and non-web-based sources and the bibliographies of the sources identified. Experts were also consulted. FINDINGS: Five studies and nine guidelines were identified. Existing guidelines specify paediatric blood sample volume limits ranging

Effectively addressing mental health issues in permanency-focused child welfare practice.

The purpose of this paper is two-fold: to highlight to the professional community the ongoing and shifting mental health needs of children exiting the child welfare system into permanent plans, dismissed from public caseloads yet still much in need of services, and, to present a model of one agency's integrated, outpatient intervention with promising preliminary data. The model offers a specialized; effective approach to the field to address the needs of families built by adoption and relative caregiving. The increasing number of adoptions, and relative and near-kin placements, mandate that co

Relationship between entry into child welfare and mental health service use.

OBJECTIVE: This study examined the relationship between initiation of outpatient mental health service use and level of child welfare involvement. METHODS: Three levels of child welfare involvement were examined: in-home care and no child welfare services beyond an initial investigation, in-home care and additional child welfare services, and placement in out-of-home care (foster care). Longitudinal data were collected for a subsample of children (N=3,592) aged two through 14 years who were enrolled in the National Survey of Child and Adolescent Well-Being, a nationally representative sample

Restricting legal abortion: Some maternal and child health effects in Romania.

The dramatic rise of the Romanian birth rate in 1967 following the restrictive abortion decree of late 1966 is well known. Less well known are significant changes in key maternal and child health indicators in the period after 1966. Possible reasons for these changes are discussed as are the implications of the Romanian experience for population policy and abortion research in the United States.

A community-based child health and parenting intervention to improve child HIV testing, health, and development in rural Lesotho (Early Morning Star): a cluster-randomised, controlled trial.

BACKGROUND: When caregivers live in remote settings characterised by extreme poverty, poor access to health services, and high rates of HIV/AIDS, their caregiving ability and children's development might be compromised. We aimed to test the effectiveness of a community-based child health and parenting intervention to improve child HIV testing, health, and development in rural Lesotho. METHODS: We implemented a matched cluster-randomised, controlled trial in the Mokhotlong district in northeastern Lesotho with 34 community clusters randomly assigned to intervention or wait-list control groups …
