The aim of the study was to prepare chloramphenicol (CAM)-loaded transfersomes to enhance drug permeability following topical administration, considering the final goal of treating conjunctivitis by applying the formulation onto the eyelid. The clinical history and treatment outcome of a twenty-year old female patient (EN) with typhoid fever is presented. Laboratory examination showed that the etiological agent was a strain of Salmonella typhi which was simultaneously resistant to chloramphenicol (C), streptomycin (S), sulphonamides (Su) and tetracycline (T). Genetic and molecular analysis of this strain demonstrated that resistance to CSSuT was encoded in a plasmid which showed a thermosensitive (28 degrees C) transfer to an Escherichia coli K12 recipient, at a frequency of 4.0 x 10(-2). Forty patients admitted with typhoid fever from 1-1-1990 to 31-3-1990 were studied. The clinical features differed from the classical clinical profile. Thirty cases (75%) did not respond to chloramphenicol. Gentamicin, nitrofurantoin, nalidixic acid, kanamycin, cephalexin and norfloxacin were tried according to the sensitivity pattern. But response was variable. Ciprofloxacin was used in 18 cases which failed to respond to one of the above drugs. The result was quite satisfactory with minimal side-effects.

Aztreonam treatment was successful in 4 cases of typhoid fever. The patients were seriously ill with growth of Salmonella typhi in blood cultures despite treatment with chloramphenicol. Aztreonam appears to be a useful alternative to standard therapy of typhoid fever.
