OBJECTIVES: Retrospective cholecystography studies in adults with hereditary spherocytosis (HS) suggested detectable gallstones in 37% to 43% of patients. Since longitudinal studies using biliary ultrasonography are unavailable, the aim of the present study was to determine the incidence of gallstone disease, as detected by biliary ultrasonography, in children and young adults with HS. As individuals with HS who co-inherit Gilbert syndrome have a greater risk of developing gallstones, uridine diphosphate-glucuronyl transferase (UGT-1A) gene polymorphism was also determined.

Fourteen patients with a cystic duct syndrome (CDS) underwent cholecystokinin (CCK) cholescintigraphy. All patients presented with persistent postprandial right upper quadrant pain and biliary colic. A wandering gallbladder typically is attached to surrounding structures only by the cystic duct and its mesentery. Its characteristic propensity for torsion places it at risk for necrosis. A 70-year-old woman admitted to our hospital was found to have a wandering gallbladder. The possible clinical implications of this finding are discussed.

Two cases of hypoplasia of the right hepatic lobe with gallbladder ectopy, which were diagnosed by computed tomography, are reported here. In each patient, the left hepatic lobe was enlarged. The gallbladder was located in the inferior portion of the liver in one patient and immediately below the right hemidiaphragm in the other. No other anomalies complicated these two cases. It should be kept in mind that hepatic lobe anomalies may have associated ectopy of the gallbladder.

Extracorporeal shock wave lithotripsy has become an important mode of therapy for renal lithiasis in patients who cannot withstand surgery. Recently, this method has been successful in fragmenting biliary stones in selected healthy patients. We report the successful fragmentation of biliary stones after the administration of local anesthesia in two patients who were poor surgical risks.
