Cholestasis underlies one of the major manifestations of drug-induced liver injury. We report a case of a 37-year-old man with type 1 diabetes mellitus and sickle cell trait presenting with jaundice and acute liver cholestasis. We describe a case of a 23-year-old woman with a history of Crohn's disease (CD), who initially presented with sepsis-like symptoms, subsequently developed severe cholestasis and following extensive inpatient workup was found to have non-caseating granulomas on her liver biopsy. Infectious aetiologies were excluded and the patient was treated with oral corticosteroids, which ameliorated but did not completely reverse the cholestasis. We review the differential diagnosis of hepatic granulomas and discuss the potential difficulties in establishing their exact aetiology in patients with CD.

This study aims to analyse the risk factors affecting prognosis of cholestasis in newborns. A four-year prospective cohort study was carried out. Neonates with cholestasis were enrolled. The diagnosis of neonatal cholestasis was based on jaundice in the newborn period, direct bilirubin > 2 mg/dl, discoloured stool and elevated liver enzymes. Liver function tests were consecutively monitored weekly during the first month and then monthly until the disease was under control. Neonatal conjugated jaundice is a common presentation of hereditary liver diseases, which, although rare, are important to recognize early. Developments in molecular genetic techniques have enabled the identification of causative genes, which has improved diagnostic accuracy for patients and has led to a greater understanding of the molecular pathways involved in liver biology and pathogenesis of liver diseases. This review provides an update of the current understanding of clinical and molecular features of the inherited liver diseases that cause neonatal conjugated jaundice.
