Perspectives on Living With Fibromyalgia.

Perceptions of people living with chronic illness change over time, contributing to health-related stress that necessitates coping skills. Paterson's shifting perspectives model provides an explanation of chronically ill people's variations in attention to their symptoms. In this qualitative study, 20 people with fibromyalgia living in a rural setting were interviewed in 2013 with the aim of gaining insight into their experiences and the meaning-making associated with their chronic condition. Analysis of the interview data categorized five recurrent, or common, themes: experiences of loss, fee

Is deinstitutionalization working in our community?

This exploratory study examined the impact of deinstitutionalization on consumers admitted to a regional community care unit (CCU) between 1996 and 2007, and looked at lengths of stay and re-admissions to acute psychiatric care units and the impact this might have on quality of life. The results showed that the original and current residents of CCU have improved quality of life through friendships, a home-like environment, and reduced re-admissions to acute psychiatric care units; however, further improvements can be made with more emphasis on employment/vocational services and social inclusio

Identifying common barriers and facilitators to linkage and retention in chronic disease care in western Kenya.

BACKGROUND: Sub-Saharan Africa is increasingly being challenged in providing care and treatment for chronic diseases, both communicable and non-communicable. In order to address the challenges of linkage to and retention in chronic disease management, there is the need to understand the factors that can influence engagement in care. We conducted a qualitative study to identify barriers and facilitators to linkage and retention in chronic care for HIV, tuberculosis (TB) and Hypertension (HTN) as part of the Academic Model Providing Access to Healthcare (AMPATH) program in western Kenya. METHOD

[Integrated care of hidradenitis suppurativa using the example of the EsmAiL project].

The care of patients with hidradenitis suppurativa (HS) is often characterized by delayed diagnosis, fragmented treatment pathways, and limited access to specialized care, allowing the disease to progress to irreversible stages. As the disease progresses, increasing pain, chronic inflammation, and psychosocial burden lead to a substantial reduction in quality of life. Studies indicate that the central problem is rather in the structural deficits in healthcare provision than in a lack of therapeutic options. To address these shortcomings, the EsmAiL project was developed to establish a structur

The quality of life of people who have chronic wounds and who self-treat.

AIMS AND OBJECTIVES: The aim of this study was to investigate the effect of chronic wounds on the physical, emotional, social, lifestyle and financial domains of quality of life among people who self-treat their wounds. BACKGROUND: Patient-centred models of care have received increased attention over recent decades because of the potential to contribute positively to the patient's health and well-being. A contemporary understanding of the effect of chronic wounds on quality of life may assist care providers and healthcare systems to respond to patient needs and improve patient outcomes. METH

A qualitative study of GPs' attitudes to self-management of chronic disease.

BACKGROUND: Improving the quality of care for patients living with a chronic illness is a key policy goal. Alongside systems to ensure care is delivered according to evidence-based guidelines, an essential component of these new models of care is the facilitation of self-management. However, changes to the way professionals deliver care is complex, and it is important to understand the key drivers and barriers that may operate in the primary care setting. AIM: To explore GPs' perspectives on their involvement in the facilitation of chronic disease self-management. DESIGN OF STUDY: Qualitativ

Pathological personality traits intensity variation according to ICD-11 within general population and clinical groups.

PURPOSE: Although rural and remote residents face general challenges accessing health care in comparison to urban dwellers, care for musculoskeletal conditions like chronic back disorders (CBD) is particularly challenging for rural and remote residents due to lack of access to physical yherapists. Telerehabilitation such as secure videoconferencing offers one solution to this disparity in rural care delivery, but incorporating the perspectives of health practitioners and patients is important when developing new sustainable care models. PATIENTS AND METHODS: This study investigated the experi

Interdisciplinary community care: emerging developments across European healthcare systems.

An ageing population, increasingly frail and affected by multiple chronic conditions and the impact of COVID-19 on overall health, has necessitated the acceleration of international territorial healthcare reorganisation. Responses have been adapted to several care settings highlighting the central role of nurses, both in terms of direct care and in organisational and clinical views. Urgent community response initiatives are based on key principles of multidisciplinarity and interprofessional collaboration, which are significant in the context of European healthcare, and could serve as a model

Integrating Primary and Secondary Care to Enhance Chronic Disease Management: A Scoping Review.

BACKGROUND: In Ireland, as in many healthcare systems, health policy has committed to delivering an integrated model of care to address the increasing burden of chronic disease. Integrated care is an approach to healthcare systems delivery that aims to minimise fragmentation of patient services and improve care continuity. To this end, how best to integrate primary and secondary care is a challenge. This paper aims to undertake a scoping review of empirical work on the integration of primary and secondary care in relation to chronic disease management. METHODS: A search was conducted of 'PubM

Community based Primary Care for Adolescents and Young Adults Transitioning From Pediatric Specialty Care: Results from a Scoping Review.

BACKGROUND: Ongoing primary care during adolescence is recommended by best practice guidelines for adolescents and young adults (AYAs; ages 12-25) with chronic conditions. A synthesis of the evidence on the roles of Primary Care Physicians (PCPs) and benefits of primary care is needed to support existing guidelines. METHODS: We used Arksey and O'Malley's scoping review framework, and searched databases (MEDLINE, EMBASE, PsychINFO, CINAHL) for studies that (i) were published in English between 2004 and 2019, (ii) focused on AYAs with a chronic condition(s) who had received specialist pediatric

Development and validation of a screening tool to predict the risk of chronic low back pain in patients presenting with acute low back pain: a study protocol.

INTRODUCTION: Around 40% of people presenting to primary care with an episode of acute low back pain develop chronic low back pain. In order to reduce the risk of developing chronic low back pain, effective secondary prevention strategies are needed. Early identification of at-risk patients allows clinicians to make informed decisions based on prognostic profile, and researchers to select appropriate participants for secondary prevention trials. The aim of this study is to develop and validate a prognostic screening tool that identifies patients with acute low back pain in primary care who are

ISSLS prize winner: early predictors of chronic work disability: a prospective, population-based study of workers with back injuries.

STUDY DESIGN: Prospective population-based cohort study. OBJECTIVE: To identify early predictors of chronic work disability after work-related back injury. SUMMARY OF BACKGROUND DATA: Identification of early predictors of prolonged disability after back injury could increase understanding concerning the development of chronic, disabling pain, and aid in secondary prevention. Few studies have examined predictors across multiple domains in a large, population-based sample. METHODS: Workers (N = 1885) were interviewed 3 weeks (average) after submitting a lost work-time claim for a back injury.

Fear avoidance and illness beliefs in post-traumatic neck pain.

STUDY DESIGN: A descriptive overview of the relevant literature and the introduction of a new psychological model. OBJECTIVE: The fear-avoidance (FA) model and the potential importance of illness beliefs in post-traumatic neck pain are discussed. The causal beliefs-anxiety model is introduced as an adaptation of the FA model, emphasizing the critical role of illness beliefs. SUMMARY OF BACKGROUND DATA: Although the FA model is most thoroughly used to investigate chronic low back pain, it seems also highly relevant as a starting point for other chronic pain conditions like whiplash. Impact of a Primary Care Value-Based Compensation Model.

BACKGROUND: Primary care is essential for achieving high quality population health; however, its future remains uncertain with pervasive provider burnout, compensation structures based on service volume, and systemic undervaluation relative to specialty care. Value-based payment models aim to improve health outcomes and reduce costs, yet their implementation in academic medical centers remains complex. This study examined primary care physicians' perceived impacts of changing to a value-based compensation model in a large academic medical center. METHODS: A mixed methods study was conducted u

A risk prediction score for acute kidney injury in the intensive care unit.

BACKGROUND: Acute kidney injury (AKI) is common in critically ill patients and is associated with high morbidity and mortality. Early identification of high-risk patients provides an opportunity to develop strategies for prevention, early diagnosis and treatment of AKI. METHODS: We undertook this multicenter prospective cohort study to develop and validate a risk score for predicting AKI in patients admitted to an intensive care unit (ICU). Patients were screened for predictor variables within 48 h of ICU admission. Baseline and acute risk factors were recorded at the time of screening and se

The Hepatitis C virus treatment cascade at an urban postincarceration transitions clinic.

Transitions clinics, which provide medical care to individuals who have been released from incarceration, reach a population at high risk for hepatitis C Virus (HCV) infection. We used the HCV treatment cascade to describe HCV care at an urban postincarceration transitions clinic, identifying gaps in care and determining reasons for lapses in care. In this retrospective cohort study, we reviewed electronic health records for all formerly incarcerated individuals receiving care at the Bronx Transitions Clinic. HCV treatment cascade measures included the following: detection of HCV antibodies, c

'Kids need to talk too': inclusive practices for children's healthcare education and participation.

AIMS AND OBJECTIVES: To examine how children with chronic medical conditions view healthcare education and decision-making and to propose the application of the universal design for learning in paediatric settings. BACKGROUND: Children and adolescents with chronic medical conditions tend to be excluded from healthcare decision-making. In schools, the universal design for learning promotes access to education and participation in school communities for all children, regardless of their disabilities or medical needs, rendering it an appropriate model for children's participation in healthcare d

A pragmatic implementation and outcomes evaluation of the older persons emergency network acute outreach service (OPEN AOS) model utilising the integrated PRISM and RE-AIM framework: the OPEN AOS study protocol.

BACKGROUND: Globally, the population aged 65 years and over is increasing and with this growth comes a rising risk of chronic diseases that often require emergency care. Older adults living in Residential Aged Care Facilities (RACFs) have increased healthcare utilisation including visits to Emergency Departments (EDs). EDs are busy, noisy environments poorly suited to meeting the care needs of older adults. Older adults attending EDs often receive delayed and fragmented care, unnecessary procedures and tests and are at greater risk of iatrogenic complications. The Older Person’s Emergency Netw

Preclinical in vitro and in vivo evaluation of omadacycline against multidrug-resistant Acinetobacter baumannii.

Acinetobacter baumannii is an increasing cause of chronic wound infections, especially in military operational settings, and is resistant to many currently available antimicrobial therapies, making treatment options limited. We evaluated the in vitro activity of omadacycline against A. baumannii clinical isolates as well as the in vivo efficacy of omadacycline in mice against the multi-drug resistant (MDR) A. baumannii strain 5075 using both a neutropenic thigh infection model and a dorsal wound model of chronic infection relative to the standard of care drugs tigecycline and doxycycline, resp
