Traumatic Floating Clavicle: A Case Report and Updated Literature Review.

Traumatic floating clavicle or bipolar dislocation is a rare injury. Herein, we present a case of ipsilateral sternoclavicular and acromioclavicular joint dislocation after a motorcycle accident. The patient was a 43-year-old man who presented with right shoulder pain and limited range of motion. The radiograph revealed superior displacement of the acromioclavicular joint dislocation and suspected ipsilateral sternoclavicular joint dislocation and a CT scan confirmed injuries to both the medial and lateral ends of the clavicle. Due to the patient being active and young, we considered operative…

Clavicle Shaft Non-Unions-Do We Even Need Bone Grafts?

Background: The surgical treatment of bony non-unions is traditionally performed with additional bone grafts when atrophic and/or stronger implants when hypertrophic. In the case of the clavicle shaft, however, in our experience, a more controversial method where no additional bone graft is needed leads to equally good consolidation rates, independent of the non-union morphology. This method requires the meticulous anatomical reconstruction of the initial fracture and fixation according to the AO principle of relative stability. Methods: A retrospective review following the STROBE guidelines w…

Do We Ever Need to Fix Clavicle Fractures in Adolescents?

INTRODUCTION: Clavicle fractures in adults are increasingly being treated by surgical fixation following reports of symptomatic non-union, malunion and poor functional outcome with conservative treatment. This has led to a similar trend in the management of clavicle fractures in adolescents. This study aims to evaluate the outcome and complications of non-operatively treated clavicle fractures in adolescents. MATERIALS AND METHODS: This is a retrospective, single institution study on adolescents aged 13-17 years who sustained a closed, isolated clavicle fracture, between 19972015. Clinical re…

Operative Versus Nonoperative Treatment for Closed Displaced Midshaft Clavicle Fractures.

BACKGROUND: Nonoperative treatment for clavicle fractures has historically been standard of care, but with a concomitant rise in recent operative fixation for displaced midshaft clavicle fractures (MCF), a re-evaluation of treatment modalities is necessary. The purpose of this study was to compare nonunion rates among operative and nonoperative treatment of closed displaced MCF. Specifically, we assessed the following between operative and nonoperative management: (1) 90-day to 1-year nonunion and malunion incidence; (2) 90-day medical complications; and (3) 90-day to 1-year total costs of car…

Closed midshaft clavicle fractures : an evidence-based triage management algorithm.

AIMS: The management of mid-shaft clavicle fractures (MSCFs) has evolved over the last three decades. Controversy exists over which specific fracture patterns to treat and when. This review aims to synthesize the literature in order to formulate an appropriate management algorithm for these injuries in both adolescents and adults. METHODS: This is a systematic review of clinical studies comparing the outcomes of operative and nonoperative treatments for MSCFs in the past 15 years. The literature was searched using, PubMed, Google scholar, OVID Medline, and Embase. All databases were searched …

Morphologic Alterations of the Sternoclavicular Joint following Ipsilateral Clavicle Fractures.

RATIONALE: To our knowledge, no study has investigated concomitant injuries of the sternoclavicular joint (SCJ) in acute clavicle fractures. The purpose of this study was to determine the effect of an ipsilateral clavicle fracture on the SCJ in a systematic computer tomography (CT) morphologic evaluation. METHODS: CT scans in the axial and coronal plane of 45 consecutive patients with clavicle fractures were retrospectively analyzed. The scans were assessed regarding anatomic congruence of bilateral SCJs-joint space width (JSW); the position of bilateral medial clavicles (PC); and the non-fus…

Medial Clavicle Fracture Fixation Including the Sternum: A Case Report.

CASE: A 29-year-old man presented with a displaced medial clavicle fracture. Surgical repair was performed using a precontoured plate designed for the contralateral distal clavicle, and medial fixation was accomplished at the sternum. The patient had no complications and demonstrated full strength and range of motion at the 8-month follow-up. CONCLUSION: Medial clavicle fractures with a small medial fragment can be immobilized using a plate designed for the contralateral distal clavicle that crosses the sternoclavicular joint to obtain medial fixation in the sternum. This technique may provid…

Long-term outcomes of initially conservatively treated midshaft clavicle fractures.

BACKGROUND: Recent studies about completely displaced midshaft clavicle fractures have reported that their nonunion/malunion rates were significantly higher in conservatively treated patients compared to surgically treated patients. The purpose of this study was to evaluate the factors associated with treatment decisions for midshaft clavicle fractures and also the factors that affect patient satisfaction with their treatment choice. METHODS: We retrospectively reviewed the records of 75 patients who had been diagnosed with a midshaft clavicle fracture and were treated conservatively at a sin…

Complications After Plate Fixation of Displaced Pediatric Midshaft Clavicle Fractures.

BACKGROUND: Operative treatment of displaced pediatric midshaft clavicle fractures has become increasingly popular, despite lack of evidence that surgical management leads to superior outcomes. Complications, such as plate irritation necessitating removal and wound infection, have been reported in adults. The purpose of this study was to evaluate complications after plate fixation of midshaft clavicle fractures in the pediatric population. METHODS: We retrospectively identified patients 10 to 18 years old who had undergone plate fixation of a displaced midshaft clavicle fracture between 2009 …

Lateral Clavicle Fracture Fixation Using a Superiorly Placed Locking Plate.

Clavicle fractures are common injuries that account for 4% of all fractures, and approximately 21% of clavicle fractures are lateral. Studies have demonstrated that displaced lateral clavicle fractures with disruption of the coracoclavicular ligaments have nonunion rates as high as 28%. Many surgical techniques for fixation of lateral-end clavicle fractures have been proposed. More recently, locking plate technology has led to the development of superiorly placed locking plates, which are used when the distal fragment is large enough and which offer greater biomechanical stability in osteoporo…

Association between screw prominence and vascular complications after clavicle fixation.

CONTEXT: Fixation of clavicle fractures has become more common to prevent symptomatic malunion and nonunion. The subclavian and axillary vessels are in close proximity to the medial two-thirds of the clavicle, placing them at risk from prominent metalware. Injury to these major vessels has the potential to be life or limb-threatening. Despite this anatomical risk, iatrogenic vascular injury associated with clavicle fixation is rare. AIMS: The aim of this study was to identify risk factors associated with modern fixation techniques in reported cases of vascular injury after clavicle fixation. …

Double-plate fixation in lateral clavicle fractures-a new strategy.

BACKGROUND: Lateral clavicle fractures Neer type II often lead to the dislocation of the medial fragment and an increased rate of nonunion after conservative treatment. Therefore, most authors recommend an open reduction and internal fixation. Double-plate fixation is a common technique in the treatment of complex radius fractures. The authors use it as a routine procedure in the treatment of lateral clavicle fractures Neer type II. In this article, we present our experiences. METHODS: From January 2006 to June 2008, 11 patients with lateral clavicle fractures of Neer type II were treated wit…

Preliminary analysis of Nacholapithecus scapula and clavicle from Nachola, Kenya.

The Miocene ape Nacholapithecus is known from rather complete skeletons; some of them preserve the shoulder joint, identified by three scapulae and one clavicle. Comparisons made with other Miocene and living apes ( Proconsul, Equatorius, Ugandapithecus) suggest that the mobility of the scapulohumeral joint was important, and scapular features such as the morphology and position of the spine and the morphology of the acromion and axillary border resemble those of climbing arboreal primates except for chimpanzees, gorillas, or orang-utans. From the size of the scapula (male Nasalis size), it is…
