Colonoscopic or Capsule Fecal Microbiota Transplantation for Recurrent Clostridioides difficile Infection: A Prospective Cohort.

BACKGROUND AND AIMS: Fecal microbiota transplant (FMT) is effective for preventing recurrent Clostridioides difficile infection (rCDI). Prior studies suggest that capsule and colonoscopic FMT administration are similarly effective, although the studies may be limited by small sample size or selection bias. The aim of this study was to compare the CDI recurrent rates following capsule or colonoscopic FMT. METHODS: Prospective cohort of patients with rCDI treated with capsule or colonoscopic FMT between 7/1/19 and 11/29/23. This study's primary outcome was 1-month CDI recurrence rates. Predicto

Targeting PPARγ-NF-κB: (+)-catechin reverses Clostridioides difficile toxin A-induced intestinal barrier dysfunction.

BACKGROUND: Clostridioides difficile toxin A (TcdA) is a key factor in antibiotic-associated intestinal epithelial injury. (+)-Catechin (CC) exhibits anti-inflammatory and antioxidant properties, but its role in protecting intestinal epithelial barrier function during TcdA exposure remains unclear. METHODS: Human colon epithelial Caco-2 cells were treated with TcdA to induce barrier dysfunction, followed by CC at varying concentrations. Barrier integrity was assessed using trans-epithelial electrical resistance (TEER), Lucifer yellow paracellular flux, and lactate dehydrogenase (LDH) release.

Natural History of Clostridioides difficile-Related Disease Progression in the 2-Step Testing Era.

BACKGROUND: The natural history of Clostridioides difficile progression in nucleic acid amplification test (NAAT) positive, toxin enzyme immunoassay-negative patients remains poorly described. Better understanding risk for subsequent disease may improve prevention strategies. We aimed to describe the natural history of C. difficile NAAT+/toxin- adults. METHODS: A cohort of adults (≥18 years) tested for C. difficile within the Duke University Health System between 15 March 2020 and 31 December 2023 were classified as NAAT-, NAAT+/toxin-, or NAAT+/toxin+ and followed up to 90 days. Antimicrobial susceptibility of Clostridioides difficile. An Argentinian multicenter study of isolates from human patients.

Clostridioides difficile is an etiological agent of diarrhea, and the use of antibiotics is one of the main risk factors for infection. Antimicrobials used for treatment are vancomycin (VAN), metronidazole (MET), and fidaxomicin. Resistant strains have been detected, exhibiting regional and institutional differences. The aim of this work was to determine the susceptibility profile of C. difficile clinical isolates to 14 antimicrobials, and to compare resistance among participating centers. A total of 208 consecutive isolates recovered from seven Argentinian hospitals between January 2018 and M

Multi-omic profiling a defined bacterial consortium for treatment of recurrent Clostridioides difficile infection.

Donor-derived fecal microbiota treatments are efficacious in preventing recurrent Clostridioides difficile infection (rCDI), but they have inherently variable quality attributes, are difficult to scale and harbor the risk of pathogen transfer. In contrast, VE303 is a defined consortium of eight purified, clonal bacterial strains developed for prevention of rCDI. In the phase 2 CONSORTIUM study, high-dose VE303 was well tolerated and reduced the odds of rCDI by more than 80% compared to placebo. VE303 organisms robustly colonized the gut in the high-dose group and were among the top taxa associ

Caffeic acid phenethyl ester protects Clostridioides difficile infection by toxin inhibition and microbiota modulation.

Clostridioides difficile infection (CDI) is the leading cause of hospital-acquired diarrhea that seriously threatens public health. The disruption of normal gut microbiota by the use of broad-spectrum antimicrobial agents enables C. difficile to proliferate in the colon. The emergence and prevalence of hypervirulent C. difficile strains result in increased morbidity, mortality, and high recurrence rates of CDI, thus creating a pressing need for novel therapeutics. The multi-domain toxins TcdA and TcdB are the primary determinants of CDI pathogenesis, rendering them ideal drug targets in the an

Clostridioides difficile Infection in Healthy Children After Antibiotic Use: A Report of Two Cases.

Although Clostridioides difficile infection (CDI) commonly affects the elderly, hospitalized, or immunocompromised patients with underlying diseases, cases of community-acquired CDIs in previously healthy children have recently been reported. Here, we describe the cases of two healthy children who developed CDI after the use of antibiotics. Patients were administered either high-risk antibiotics for CDI or a combination of multiple low-risk antibiotics. The clinical characteristics of the patients that differentiated CDI from antibiotic-associated diarrhea (AAD) include bloody stools, fever, a

Detection, characterization and antimicrobial susceptibility of Clostridioides difficile from seafood of Marmara Sea, Turkey.

The objective of this study was to determine the presence of C. difficile in seafood, to identify the ribotype of isolates, to reveal the distribution of their toxinogenic genes and to evaluate the antibiotic susceptibility of C. difficile strains. For this purpose, a total of 755 seafood samples were analysed. Sixteen isolates were verified as C. difficile [14 (5.28%) bivalve molluscs and 2 (2.0%) shrimp] whereas none of the ribotypes detected were human pathogenic ribotypes. Among C. difficile isolates only one of the bivalve mollusc (RT003) was found to be toxigenic (A+B+CDT+) that had also

Microbiota-Based Therapeutics as New Standard-of-Care Treatment for Recurrent Clostridioides difficile Infection.

BACKGROUND: Clostridioides difficile (C. difficile) is a spore-forming bacterial species that ubiquitously exists in the environment. Colonization by C. difficile is highly prevalent in infants, while fewer than 5% of adults are asymptomatic carriers. Disruption of the microbiome, such as through antibiotic treatment, triggers the germination of bacterial spores into numerous vegetative cells. These cells then produce enterotoxins that result in watery diarrhea and colonic inflammation. If left untreated, C. difficile infection (CDI) can lead to pseudomembranous colitis with the potentially li

NKp46+ ILC3s promote early neutrophil defense against Clostridioides difficile infection through GM-CSF secretion.

Clostridioides difficile infection (CDI) is a common cause of antibiotic-associated colitis. C. difficile proliferates and produces toxins that damage the colonic epithelium, leading to symptoms ranging from mild diarrhea to severe pseudomembranous colitis. The host's innate response to CDI occurs in two phases: an early phase in which neutrophils reduce the bacterial load and a late phase involving repair mechanisms to restore epithelial integrity. Group 3 innate lymphoid cells (ILC3s) are crucial in protecting the gut from CDI. Previous studies have shown that ILC3-derived IL-22 is essential

Prevalence, genetic characteristics, and antimicrobial resistance of Clostridioides difficile isolates from horses in Korea.

OBJECTIVES: Clostridioides difficile is an etiological agent of enteric diseases in humans and animals. Animals are considered a potential reservoir due to the genetic and antimicrobial resistance similarities between human and animal C. difficile isolates. In this study, we evaluated the genetic characteristics and antimicrobial resistance profiles of C. difficile isolated from 942 fecal samples collected from horses in South Korea during 2019-2020. METHODS: The C. difficile isolates were tested for toxin genes including tcdA (A), tcdB (B), and cdtAB (CDT) and deletions of the tcdC gene by P

Gut microbiota interspecies interactions shape the response of Clostridioides difficile to clinically relevant antibiotics.

In the human gut, the growth of the pathogen Clostridioides difficile is impacted by a complex web of interspecies interactions with members of human gut microbiota. We investigate the contribution of interspecies interactions on the antibiotic response of C. difficile to clinically relevant antibiotics using bottom-up assembly of human gut communities. We identify 2 classes of microbial interactions that alter C. difficile's antibiotic susceptibility: interactions resulting in increased ability of C. difficile to grow at high antibiotic concentrations (rare) and interactions resulting in C. d

Houston, We Have a Problem: Reports of Clostridioides difficile Isolates With Reduced Vancomycin Susceptibility.

During the past 4 decades, oral vancomycin has been a mainstay of Clostridioides difficile infection (CDI) therapy with no reports of treatment failure due to emergence of vancomycin resistance. However, C. difficile isolates with high-level phenotypic resistance to vancomycin have recently been reported in 3 distinct geographic regions. There is an urgent need for surveillance to determine if strains with reduced vancomycin susceptibility are circulating in other areas. In a Cleveland-area hospital, screening of 176 CDI stool specimens yielded no C. difficile isolates with reduced vancomycin
