Twelve feet in 11 patients with constriction band syndrome age ranging from 3 to 18 months (mean age 11 months) were evaluated in this study. The constriction band was located at the in the calf, at distal, media and proximal third levels. All these patients were treated surgically by a wide peritalar release using the Cincinnati incision, followed by a two-stage Z-skin plasty. Both clinical and radiological results were evaluated. In our material an excellent result was achieved in 1 case, good results in 3 cases, satisfactory in 4 cases and poor in the remaining four cases. Very good and goo

This study was undertaken to determine the incidence of prenatally, sonographically diagnosed clubfoot; the incidence of associated anomalies; and the correlation with postnatal findings. Cases of prenatally diagnosed clubfeet were abstracted from a prospectively entered ultrasound database. Scans were reviewed for the presence of associated anomalies. Available neonatal charts were reviewed for correlation with prenatal findings. The incidence of prenatally diagnosed clubfoot was 0.43%. This was isolated in 33% of the cases and associated with other anomalies in 67%. All cases with associated…

Clubfoot deformity in arthrogrypotic patients is difficult to correct, and its recurrence rate is high. We retrospectively reviewed 37 clubfeet in 20 patients with arthrogryposis multiplex congenita who were treated from 1984 to 1993. There were 11 males and nine females. Bilateral involvement was seen in 17 patients and unilateral involvement in three. The mean age at the initial surgery was 2.5 years (range 4 mo to 25 yr). Soft tissue release alone was performed on 33 feet. Primary talectomy was done on both feet of one other patient. The remaining patient (two feet) underwent triple arthrod…

Surgical correction of the neglected, and severely deformed clubfoot in the adult is necessary to achieve plantigrade, pain free, and cosmetically acceptable feet. Also, it is important that the patient is able to wear shoes and participate in a more active and normal life. Three adult immigrants with neglected clubfoot underwent surgical correction for the first time. Posterior medial plantar soft tissue release and a double arthrodesis involving calcaneal cuboid wedge resection with talonavicular fusion have been effective in removing rearfoot varus, providing minimal shortening, and restori…

The bony pathoanatomy of clubfoot has been assessed by a three dimensional reconstruction of transverse CT images obtained from 27 feet in children aged 3-10 years. Principal axes of the bones were determined to quantitate interosseous deformity, while visual inspection of the reconstructed images demonstrated intraosseous deformity. "Medial spin" and midfoot adduction were analyzed on the AP view of the foot ("top" view), while hindfoot pronosupination was analyzed on the AP view of the ankle (posterior view). This technique allows visualization of deformities which normally cannot be analyze…

The navicular position was evaluated (according to Simons' criteria) on anteroposterior and lateral roentgenograms of 45 clubfeet that were treated with complete soft-tissue release without internal fixation. The evaluation was performed an average of 28 months after surgery. Position of the navicular correlates well with the functional rating score system. Navicular position can be viewed as an indicator for clubfoot correction. Internal fixation of the talonavicular joint was correlated with favorable correction of deformity.

In 50 patients (72 clubfeet), the posterior tibial tendon was excised during complete soft tissue clubfoot release. The end results after an average of 3 years were graded as follows: 55.5% excellent, 29.3% good, 8.3% fair, and 6.9% poor. Heel varus and forefoot adduction were the main causes for recurrence. Heel valgus occurred in one foot. Excision of the posterior tibial tendon is safe, does not lead to overcorrection, and may prevent further scarring created when the tendon is lengthened.

A large proportion of congenital talipes equinovarus is resistant to correction by nonoperative means. The authors reviewed 54 feet which required surgical intervention, and the operative management utilized was a radical one-stage posteromedial release. Application of plastic surgical techniques has obviated problems of skin healing. Comparable to results reported by others, a good-to-excellent functional and cosmetic result can be expected in 90% of the cases. Careful attention to a well-designed postoperative regimen will frequently obviate the need for additional surgery.

PURPOSE: Ensuring compliance to treatment protocol, especially regular visit to treating facility, is an important aspect of clubfoot management. However, the factors affecting compliance to follow-up schedule are myriad.
METHODS: A cross-sectional study was undertaken among caregivers of clubfoot patients from a tertiary referral clubfoot clinic in a developing country. Hospital records were reviewed to collect demographic data and subjects were classified as either "regular" or "irregular" if they missed ≤3 and >3 scheduled hospital visits, respectively. Various factors that could affect co…

BACKGROUND: Patients and families of school children with unilateral corrected talipes equinovarus (TEV, idiopathic clubfoot) have anecdotally noted a difference in sporting function from the unaffected side to the clubfoot. The purpose of this study was to assess difference in strength, balance, performance, and patient perception of their sporting ability using contemporary sports research technology.
METHODS: Following Institutional Review Board approval, 27 unilateral Ponseti-treated patients with clubfoot (ages 8-15) enrolled in a cross-sectional study. Demographics and patient-reported …

Flat-top talus (FTT) is a complication well-known to those treating clubfoot. Despite this, it receives little attention in textbooks, so our knowledge and understanding can be limited to anecdote and hearsay. In particular, its association with different treatments, especially the Ponseti method, is not well understood. This review provides some clarity on the possible pathoanatomical, radiological and iatrogenic causes of FTT. It summarises its prevalence following different treatments. It is seen after surgery, 'pre-Ponseti' casting and the Ponseti method, and seems least common following P…

BACKGROUND: Although Achilles tenotomy is performed to increase the ankle dorsiflexion in clubfoot patients, it may also increase the amount of the foot abduction due to the unique oval field of motion property of the ankle and subtalar joint complex. This study investigated the effect of Achilles tenotomy on foot abduction in patients with clubfoot.
METHODS: Equinus deformity before tenotomy (EBT), equinus deformity after tenotomy (EAT), abduction before tenotomy (ABT), and abduction after tenotomy (AAT) were measured. The decrease in the amount of equinus after tenotomy (Δ equinus) and the …

Clubfoot and positional foot deformities (eg, pes spinatus) may have the same aspects on prenatal ultrasound (US) imaging. Nevertheless, differentiating these entities is essential because their prognoses are different. This pictorial review illustrates the US findings of clubfoot and positional foot deformities. On the basis of clinical postnatal images, we describe a prenatal US technique that could give an accurate diagnosis. In this essay, we demonstrate that when a foot malposition is suspected, a systematic analysis with 3 rigorous planes could help differentiate positional foot deformit…

PURPOSE: We assessed the pattern of relapse as well as the correlation between the number of casts required for correction and Pirani and Dimeglio scores at presentation, and age at presentation. We hypothesized that the Ponseti method would be effective in treatment of relapsed clubfoot as well.
METHODS: We evaluated 115 idiopathic clubfeet in 79 children presenting with relapse following treatment by the Ponseti method. The mean age was 33.8 months with mean follow-up of 24 months. All patients were assessed for various patterns of relapsed deformities. Quantification of deformities was don…

OBJECTIVE: Routine prenatal ultrasound has often resulted in the early detection of musculoskeletal disorders. The purpose of this study was to determine which socioeconomic factors are associated with prenatal detection of clubfoot.
METHODS: The Slone Epidemiology Center at Boston University identified infants in three states (Massachusetts, New York, and North Carolina) who were reported as having a clubfoot. Mothers of these children were contacted and interviewed, and medical records obtained. Data were analyzed by using logistic regression to estimate odds ratios (ORs) and 95% confidence…

The Ponseti method is a widely accepted and highly successful conservative treatment of pediatric clubfoot involving weekly manipulations and cast applications. Qualitative assessments have indicated the potential success of the technique with cast materials other than standard plaster of Paris. However, guidelines for clubfoot correction based on the mechanical response of these materials have yet to be investigated. The current study sought to characterize and compare the ability of three standard cast materials to maintain the Ponseti-corrected foot position by evaluating cast creep respons…

The Ponseti method of clubfoot correction is now widely practiced worldwide. Initial correction rates are nearly 100%, but subsequent relapses may occur in up to one-third of patients. Very little has been written by anyone other than Dr Ponseti about the characterization and treatment of recurrent clubfoot deformity following use of the Ponseti method. This review paper is the first one which draws together the current literature on the topic.
