Targeting PACAP: Beyond Migraine to Cluster, Menstrual, and Post-traumatic Headaches.

The pituitary adenylate cyclase-activating peptide (PACAP) pathway is a critical focus in the pathophysiology of several headache disorders. PACAP influences headache pathophysiology through interactions with distinct receptors (PAC1, VPAC1, and VPAC2) and pathways linked to neurovascular and hormonal regulation. Evidence supports its role in the modulation of trigeminal and parasympathetic systems, implicated in cluster headaches and menstrual migraine, and its involvement in sensitizing pain pathways post trauma in post-traumatic headaches. Early clinical trials targeting PACAP signaling sho…

Efficacy of an educational website on headaches in schoolchildren: A cluster-randomized controlled trial.

OBJECTIVE: In this cluster-randomized controlled trial, we developed an educational website on tension-type headache and migraine for children and adolescents and evaluated its effectiveness in a school setting. BACKGROUND: Primary headaches are a widespread issue in children and adolescents, often persisting into adulthood and associated with considerable disabilities, costs, and reduced quality of life. Effective management of primary headaches may prevent chronicity and its associated consequences. DESIGN: Guided by a workbook, N = 814 fifth and sixth grade students explored the website d

Gender Differences in the Clinical Presentation of Cluster Headache: A Role for Sexual Hormones?

Introduction: Cluster Headache (CH) is a well-characterized primary headache that mostly affects men, although a progressive decrease in the male-to-female ratio has occurred over time. Available, but partly discordant, data on gender-related differences in CH suggest a more marked overlapping with migraine features in female subjects. The aim of this study is to carefully evaluate the female/male distribution of the typical migraine-associated symptoms and of other features of the disease in a large and well-characterized clinical population of CH subjects. Materials and Methods: We enrolled …

Cluster headache - a symptom of different problems or a primary form? A case report.

Headache with severe, strictly one-sided unilateral attacks of pain in orbital, supraorbital, temporal localisation lasting 15-180 minutes occurring from once every two days to 8 times daily, typically with one or more autonomic symptoms, is recognized as cluster headache (CH). Headache with normal neurological examination and abnormal neuroimaging studies, mimicking cluster headache, is reported by several authors. We present an elderly woman with a cluster-like headache probably associated with other comorbidities. We differentiate between primary, but 'atypical' CH and symptomatic cluster h…

Vagus nerve stimulation relieves chronic refractory migraine and cluster headaches.

Anticonvulsant and antidepressant medications have demonstrated efficacy in migraine treatment. Vagus nerve stimulation (VNS) is an effectiv … es VNS produced dramatic improvement with restoration of ability to work. Two patients with chronic cluster headaches had significant improvement of their headaches. VNS was well tolerated in five patients, while one developed nausea even at the lowest current strength. In conclusion, VNS may be an effective therapy for intractable chronic migraine and cluster hea …

Risk and reward seeking behaviour in episodic cluster headache - a psychometric and behavioural study.

BACKGROUND: Anecdotally, people with cluster headache (CH) seem to have an increased tendency towards risk-reward seeking. METHODS: We assessed and compared risk-reward seeking behaviour in people with either episodic CH (ECH), chronic CH (CCH), migraine (M) or without regular headaches (HC) (N = 35 in each group) with (i) two rounds of the Balloon Analogue Risk Task and (ii) eight psychometric questionnaires. RESULTS: Participants with ECH scored higher on the total Zuckerman sensation seeking scale compared to all groups (21.8 [95% CI: 18.6 to 24.9] vs. CCH: 8.2 [95% CI: 3.9 to 12.4], M: 6…

Cluster Headache Management: Evaluating Diagnostic and Treatment Approaches Among Family and Emergency Medicine Physicians.

Background and Objectives: Cluster headaches (CHs) are one of the most painful primary headaches and negatively affect the lives of patients due to misdiagnosis. Family medicine (FM) and emergency medicine (EM) physicians are one of the most important steps in making the correct diagnosis and directing patients to headache specialists. In this study, the knowledge and management approaches of these two groups regarding CH were evaluated. Materials and Methods: Two online questionnaires were developed to gather the demographic data of physicians and to assess their knowledge about the character…

Cluster headache diagnostic delay and its predictors: a systematic review with a meta-analysis.

BACKGROUND: Despite its characteristic clinical expression, cluster headache (CH) often remains unrecognized in clinical practice, with patients suffering from CH having to wait a long time before receiving a correct diagnosis and benefit from appropriate treatment. METHODS: This work is a systematic review of data accessible through PubMed and published up to December 2024, focusing on the delay in CH diagnosis and its predictors. A meta-analysis was performed to estimate the mean CH diagnostic delay using the inverse of variance as the weight. A qualitative analysis was performed to identif…

Factors associated with efficacy of occipital nerve stimulation in medically intractable chronic cluster headache.

BACKGROUND/OBJECTIVE: Occipital nerve stimulation (ONS) has become an established therapy for medically intractable chronic cluster headache (MICCH), but unfortunately, one third of the patients do not respond satisfactorily. Reliable predictors of treatment success would help physicians improve indication for ONS in MICCH. Although a recent report suggested several factors that were associated with treatment failure (early onset of cluster headache [CH], chronic cluster headache [CCH], and smoking) this study was small, did not use a formal model, efficacy was poorly defined, and the follow-u…

Chronic cluster headache is a rare disease: Implications for diagnosis, treatment and public health.

Cluster headache (CH) is a rare and painful primary headache disorder characterized by severe unilateral pain and cranial autonomic symptoms. This perspective examines the epidemiological evidence supporting the classification of chronic cluster headache (CCH) as a rare disease, noting a prevalence of CH of approximately 124 per 100,000 individuals, with only 3.5-13.7% manifesting CCH. This prevalence meets criteria established by both the US Food and Drug Administration and European Medicines Agency for rare disease designation. The rarity of CCH creates substantial clinical and research chal…

Can atogepant be a preventive treatment for cluster headache?-Insights from a case series.

Cluster headache (CH) is a disabling primary headache disorder with limited therapeutic options. Calcitonin gene-related peptide (CGRP) is known to be involved in CH pathophysiology; however, except for galcanezumab (300 mg) in episodic CH, anti-CGRP monoclonal antibodies did not reduce CH attacks in randomized clinical trials. Atogepant is an oral, small-molecule, CGRP receptor antagonist, which is approved for the preventive treatment of migraine. Here, we describe four case reports of CH (two episodic CH and two chronic CH), unresponsive to previous prophylactic treatments, who responded to…

Clinical Characteristics and Patient Burden of Cluster Headache in Brazil: A Real-World Study.

INTRODUCTION: Cluster headache (CH) is a debilitating condition with health-related and economic burden to patients, including reduced quality of life (QoL) and suicidality. Brazilian data on comorbidities, symptoms, or impact on QoL and productivity remain limited. This study aims to assess CH's impact on patients' QoL, overall health, and employment in Brazil from physicians' and patients' perspectives. METHODS: Data were extracted from the Adelphi Real World CH Disease Specific Programme™, a real-world, cross-sectional survey of physicians and their patients with CH in Brazil from November…

Non-invasive neuromodulation for migraine and cluster headache: a systematic review of clinical trials.

Non-invasive neuromodulation therapies for migraine and cluster headache are a practical and safe alternative to pharmacologics. Comparisons of these therapies are difficult because of the heterogeneity in study designs. In this systematic review of clinical trials, the scientific rigour and clinical relevance of the available data were assessed to inform clinical decisions about non-invasive neuromodulation. PubMed, Cochrane Library and ClinicalTrials.gov databases and the WHO's International Clinical Trials Registry Platform were searched for relevant clinical studies of non-invasive neuromo…

Cluster headache as a first manifestation of multiple sclerosis: case report and literature review.

Cluster headache (CH) is estimated to be the most common primary trigeminal autonomic headache, although it is a rare disabling medical condition. Dominant symptoms of CH include severe unilateral orbital, supraorbital, and/or temporal pain, lasting from 15 to 180 minutes if untreated, associated with at least one of various autonomic symptoms during the headache, such as conjunctival injection, lacrimation, nasal congestion and rhinorrhea, facial sweating, miosis, ptosis, and eyelid edema. Headache is not frequently a symptom of multiple sclerosis (MS). The most commonly reported primary head…

Cluster-like headache after surgical crystalline removal and intraocular lens implant: a case report.

Cluster headache (CH) is a well characterized primary headache disorder. Nevertheless, symptomatic CH has been reported in association with various underlying diseases. Symptomatic cluster headache related to ocular pathologies have been rarely described. We report a case consequent to a surgical operation for cataract.

Systemic autonomic involvement in episodic cluster headache: a comparison between active and remission periods.

In this study we compared systemic autonomic involvement in episodic cluster headache during active and silent periods. Seventeen patients were studied with sympathetic skin response, lying to standing test, Valsalva manoeuvre, deep breathing test and orthostatic hypotension evaluation. Each of them underwent these tests during active and silent periods. Values were then compared with normal controls. Our data show a parasympathetic, but not sympathetic, involvement. Moreover, this impairment seems to be "chronic", as it persists beyond the active period.

Hypothalamic deep brain stimulation for intractable chronic cluster headache: a 3-year follow-up.

Cluster headache is the most severe among primary headaches. Positron emission tomography and functional MRI studies have demonstrated that the ipsilateral posterior hypothalamus is activated during cluster headache attacks and is structurally asymmetric in these patients thus indicating that cluster headache may originate at that level. These hypothalamic abnormalities in cluster headache led to the suggestion that deep brain stimulation of ipsilateral posterior inferior hypothalamus might produce clinical improvement in otherwise treatment refractory chronic cluster headache patients. In a p…

Cluster headache with trigeminal neuralgia. An uncommon association that may be more than coincidental.

Cluster headache and trigeminal neuralgia (tic douloureux) share a common pattern of exacerbation and remission of pain that is described in similar terms by patients. Although the treatment of these conditions is markedly different, the results of adequate prophylaxis can be extremely impressive in both. The physician who treats headache patients should be aware of the common characteristics of each condition and of the possibility of their concomitant occurrence.
