[Cochlear implant management of young children].

INTRODUCTION: Since 1988, more than 450 children have received cochlear implants at the Department of Otolaryngology of the Medizinische Hochschule Hannover. Among them are 38 children who underwent surgery before the age of two. Due to increasing experience with this technique, the mean age at implantation has decreased over time so that most children nowadays receive implants between the ages of two and five. In terms of the critical periods of both development of the auditory system and the acquisition of language, it is advantageous for even younger children to receive implants soon after

Multichannel (Ineraid) cochlear implant update.

From April 1984 to May 1991, 49 profoundly deaf patients received implantation of the Ineraid multichannel cochlear implant at the University of Utah. The auditory results of 48 patients indicated improvement in mean pure-tone performance to 500 Hz (32.9 +/- 10.1 dB), 1000 Hz (27.1 +/- 10.0 dB), and 2000 Hz (30.1 +/- 9.8 dB). This group of patients had mean performances of 61.2% on auditory-only environmental sounds (Minimum Auditory Capabilities [MAC] battery), 48.8% on auditory-only CID sentences, and 95.5% on auditory-plus-visual CID sentences. The percutaneous pedestal has been well-tolera

Medical imaging in cochlear implant candidates.

A retrospective study of preoperative radiographic surveys obtained in 85 cochlear implant patients was performed. A total of 90 cochlear implant patients were seen at our institution during the last 10 years. In three, the implant was placed at another institution. In two otosclerosis patients, no preimplant radiographic examination was obtained. Meningitis was the cause of deafness in 45 of 90 patients (50 percent); 28 of 40 pediatric age group patients (70 percent), and in 17 of 50 adult patients (34 percent). Thirty-three of 45 postmeningitis patients (73 percent) showed characteristic rad

Histopathological findings in cochlear implants in cats.

This histopathological study on cats has shown that electrodes may be implanted into the cochlea through the round window or an opening drilled over the middle and apical turns without loss of the spiral ganglion cells or auditory nerve fibres. It has also shown that infection, leading to a labyrinthitis, was more likely to occur when an opening was drilled in the cochlea, and this could lead to a marked destruction of the cochlear structures, and loss of the spiral ganglion cells.

Robotic Arm Assisted Slow Electrode Insertion and Simultaneous Measurement of Cochlear Microphonics in Cochlear Implantation.

Cochlear microphonic potentials (CMPs) are an electrical response predominantly of the outer and, to a smaller degree, the inner hair cells to acoustic signals. They can be used to monitor hearing and structure preservation during cochlear implantation. Among other precautions, slow and steady electrode insertion has proven beneficial in this regard. In this article, the authors present a combination of robotic-assisted implantation and observation of CMPs. CMPs are recorded through the cochlear implant tip electrode before and after opening the round window membrane, during insertion, and in

[Neuromonitoring of the cochlear nerve during vestibular schwannoma resection and simultaneous cochlear implantation. German version].

Vestibular schwannomas (VS) are benign tumors that originate from the nerve sheath of one of the two vestibular nerves. VS can have a severe impact on everyday life of the patient and may lead to symptoms such as vertigo, hearing loss (e.g., as sudden deafness), deafness, and tinnitus. Treatment concepts include observational waiting with regular imaging control ("wait and scan"), radiotherapy, or surgical resection. Depending on the size of the tumor and status of functional hearing, the surgical approach may be retrosigmoidal, transtemporal, retrolabyrinthine, or translabyrinthine. The trans

Influence of cochlear coverage on speech perception in single sided deafness, bimodal, and bilateral implanted cochlear implant patients.

PURPOSE: Individualized cochlear implantation (CI) is essential to facilitate optimal hearing results for patients. Influence of cochlear coverage (CC) has been studied, however without consideration of different CI-categories, like single sided deafness (SSD), bimodal, and bilateral separately. METHODS: Retrospective analysis of preoperative CT scans was performed at a tertiary center. For each patient their individual CC with the selected electrode array was calculated off the complete CDL. Patients were categorized into SSD (n = 30), bimodal (n = 72), and bilateral CI patients (n = 29). Sp

The impact of cochlear implant microphone settings on the binaural hearing of experienced cochlear implant users with single-sided deafness.

OBJECTIVE: Cochlear implantation has become a well-accepted treatment option for people with single-sided deafness (SSD) and has become a clinical standard in many countries. A cochlear implant (CI) is the only device which restores binaural hearing. The effect of microphone directionality (MD) settings has been investigated in other CI indication groups, but its impact on speech perception in noise has not been established in CI users with SSD. The focus of this investigation was, therefore, to assess binaural hearing effects using different MD settings in CI users with SSD. METHODS: Twenty-

Are routine preoperative CT scans necessary in adult cochlear implantation? Implications for the allocation of resources in cochlear implant programs.

Our aim was to critically assess the influence of preoperative computed tomography (CT) scans on implantation decisions for adult cochlear implant candidates. The working hypothesis was that these routine scans might not provide critical additional information in most adult cochlear implant candidates. The charts of 175 adults with unilateral cochlear implantation were reviewed. Preoperative CT scan reports were audited, and scans with reported pathology were examined by an Otologist/ENT Surgeon. Clinic notes and multidisciplinary team meeting summaries were also analyzed to assess whether the

Power-efficient ramped stimulation in a fully implantable cochlear implant.

Cochlear implants (CIs) are among the most established neuromodulation devices, providing auditory perception through electrical stimulation of the auditory nerve. While conventional stimulation strategies rely on symmetric biphasic rectangular pulses, alternative pulse shapes may offer improvements in neural activation and energy efficiency-particularly for fully implantable CI systems where power consumption is a key limitation. In this study, we investigate the efficacy of anodic-first ramped biphasic pulse shapes compared to conventional anodic-first rectangular pulses, using a custom-desi

Silicone-based AC102-loaded cochlear implant coatings protect residual hearing in an animal model of cochlear implantation.

Cochlear implant users with residual hearing benefit synergistically from combined electrical stimulation via the cochlear implant and preserved residual hearing after surgery. However, direct mechanical trauma and subsequent inflammation may deteriorate hearing function. AC102, a novel otoprotective pyridoindole with anti-apoptotic and anti-oxidative properties significantly improved hearing recovery following cochlear implantation when administered intratympanically prior to surgery. Additionally, AC102 exerts neurotrophic effects, possibly aiding in the preservation of auditory nerve fibers

Cortical Auditory Evoked Potentials Recorded Directly Through the Cochlear Implant in Cochlear Implant Recipients: a Feasibility Study.

OBJECTIVES: The use of objective measures in cochlear implant (CI) mapping, has greatly contributed to the refinement of the setting of audible and comfortable stimulation levels, which serve as the basis of the mapping process, especially in cases of infants and young children. In addition, objective measures can also confirm the integrity of the CI system. Current CI objective measures mainly reflect neural activity from the auditory nerve and brainstem site. An objective cortical CI measure that reflects directly central auditory activity is greatly needed, especially since it is closely re

Intraoperative Assessment of Cochlear Nerve Function During Cochlear Implantation Using the Auditory Nerve Test Stimulator.

BACKGROUND/OBJECTIVES: A crucial factor for a successful cochlear implant (CI) outcome is an intact auditory nerve (AN). The integrity of the AN can be tested during implantation by measuring electrical auditory brainstem responses (eABR) via the CI. A method that does not require a CI is the use of the auditory nerve test stimulator (ANTS) from MED-EL (Innsbruck, Austria). The aim of the current study was to investigate the cases tested with the ANTS at our clinic and to describe the hearing results following CI for the cases who were implanted with a CI. METHODS: All patients underwent preo
