Cognitive-behavioral therapy for panic disorder: current status.

Controlled clinical trials evaluating cognitive-behavioral approaches for panic disorder are rapidly accumulating. In the aggregate, these studies suggest substantial efficacy for cognitive-behavioral approaches in both the short and long term. Summaries and meta-analyses of these results are briefly described, but new evidence is also presented indicating that, in this chronic condition, patients continue to experience some exacerbations and remissions over the long term. Current attempts to evaluate combination psychosocial and pharmacologic approaches are described as well as the beginnings

A comparison of cognitive profiles in schizophrenia and other psychiatric disorders.

This study compared patients across 5 psychiatric diagnostic groups: Depression, Mania, Schizophrenia, Schizoaffective Disorder, and Psychosis NOS, all of whom are psychotic. Differences in overall cognitive profiles and in dysfunctional memory mechanisms, as well as the effect of psychosis on cognitive functioning were explored using the Neurobehavioral Cognitive Status Examination (NCSE), a brief screening instrument. Results indicated pronounced deficit in memory and abstract reasoning associated with schizophrenic illness, which is not secondary to psychosis and points to localized brain d

Cognitive Potentially Inappropriate Medications, Quality of Life, and Healthcare Use in Older Adults With Cognitive Disorders.

BACKGROUND: Older adults with cognitive disorders, including dementia and mild cognitive impairment, are particularly vulnerable to adverse effects from centrally acting medications. Cognitive potentially inappropriate medications (CogPIMs), including anticholinergics, antipsychotics, benzodiazepines (BZDs), and non-benzodiazepine hypnotics (Z-drugs), may worsen well-being and increase healthcare utilization in this population. This study evaluates CogPIMs exposure and its association with health-related quality of life (HRQoL) and healthcare utilization among community-dwelling older adults w

Clinical and cognitive insight in panic disorder: phenomenology and treatment effects in internet cognitive behavior therapy.

Clinical observations suggest that individuals with panic disorder (PD) vary in their beliefs about the causes of their panic attacks. Some attribute these attacks to psychological factors, while others to physiological or medical factors. These beliefs also extend to whether individuals perceive panic attacks as dangerous. In other areas of psychiatric nosology, these phenomena are commonly called clinical insight (recognition of disorder and the need for treatment) and cognitive insight (the ability to reflect on one's beliefs). Despite its importance, limited research exists on insight in P

Does cognitive behavioral therapy alter mental defeat and cognitive flexibility in patients with panic disorder?

OBJECTIVE: Mental defeat and cognitive flexibility have been studied as explanatory factors for depression and posttraumatic stress disorder. This study examined mental defeat and cognitive flexibility scores in patients with panic disorder (PD) before and after cognitive behavioral therapy (CBT), and compared them to those of a gender- and age-matched healthy control group. RESULTS: Patients with PD (n = 15) received 16 weekly individual CBT sessions, and the control group (n = 35) received no treatment. Patients completed the Mental Defeat Scale and the Cognitive Flexibility Scale before th

Executive cognitive dysfunction and ADHD in cocaine dependence: searching for a common cognitive endophenotype for addictive disorders.

BACKGROUND: Cocaine-dependent individuals (CDI) present executive cognitive function (ECF) deficits, but the impact of psychiatric comorbidities such as Attention-Deficit Hyperactivity Disorder (ADHD) on neuropsychological functioning is still poorly understood. The aim of this study was to investigate if CDI with ADHD (CDI + ADHD) would have a distinct pattern of executive functioning when compared with CDI without ADHD (CDI). METHODS: We evaluated 101 adults, including 69 cocaine-dependent subjects (divided in CDI and CDI + ADHD) and 32 controls. ECF domains were assessed with Digits Forwar

The cerebellar mutism syndrome and its relation to cerebellar cognitive function and the cerebellar cognitive affective disorder.

The postoperative cerebellar mutism syndrome (CMS), consisting of diminished speech output, hypotonia, ataxia, and emotional lability, occurs after surgery in up to 25% of patients with medulloblastoma and occasionally after removal of other posterior fossa tumors. Although the mutism is transient, speech rarely normalizes and the syndrome is associated with long-term adverse neurological, cognitive, and psychological sequelae. The clinical, neuroradiographic, and neuropsychological findings associated with CMS as well as possible mechanisms of injury are reviewed. Theories about the pathophys

Vascular cognitive disorder: a new diagnostic category updating vascular cognitive impairment and vascular dementia.

Vascular cognitive impairment (VCI) was proposed as an umbrella term to include subjects affected with any degree of cognitive impairment resulting from cerebrovascular disease (CVD), ranging from mild cognitive impairment (MCI) to vascular dementia. VCI may or may not exclude the host of "focal" circumscribed impairments of specialized functions such as language (aphasia), intentional gesture (apraxia), or categorical recognition (agnosia), among others, that may result from a stroke. Therefore, there are no universally accepted diagnostic criteria for VCI. We conclude that this concept could

Neuropsychological assessment in functional cognitive disorder: a scoping review.

BACKGROUND: Functional cognitive disorder (FCD) involves potentially reversible cognitive complaints characterized by internal inconsistency, where subjective cognitive difficulties exceed or are incompatible with observed performance and cannot be explained by other neurological or psychiatric disorders. Although some patients perform below expected levels on formal tests, the role of neuropsychological assessment in FCD diagnosis remains unclear. OBJECTIVES: This review aimed to synthesize evidence on formal neuropsychological assessment in FCD and provide guidance for practitioners involve

Trajectories of Cognitive Functioning in Older Age Bipolar Disorder.

OBJECTIVES: The prevalence of cognitive impairment is high in older age bipolar disorder (OABD). It remains unclear whether the course of cognitive functioning in these individuals follows a course of cognitive decline, whether this is different among subgroups, and if so, which factors are associated with cognitive decline. METHODS: Our clinical sample included 206 patients aged over 50 years from the Dutch Older Bipolar (DOBi) cohort with a total range of included measures of 306 months (1997-2022). Growth Mixture Modeling (GMM) was used to find homogeneous subgroups based on their longitud

Cognitive correlates of Antisaccade Task performance in bipolar disorder.

BACKGROUND: The Antisaccade Task is often used to measure inhibitory control. While pooled effects in meta-analyses demonstrate antisaccade impairments in bipolar disorder (BD), individual studies show considerable variability in effect sizes and case-control significance, particularly in euthymic samples. This variability may reflect cognitive heterogeneity across BD samples. In this exploratory study we investigated whether antisaccade performance correlated with other cognitive domains in BD, and whether impairments were confined to specific cognitive subgroups. METHOD: 152 individuals (44

Cognitive impairment in bipolar disorder: How close to dementia?

Bipolar disorder (BD) is a severe mental illness associated with cognitive and functional impairment. This cross-sectional study evaluated the heterogeneity of cognitive and psychosocial functioning in BD, compared cognitive performance between BD and dementia groups, and identified factors associated with cognitive dysfunction. The study included 108 individuals with BD, of whom 32 were euthymic, while the remainder were symptomatic and 39 with major neurocognitive disorder. Sociodemographic data and mood symptoms were collected through structured interviews. Cognitive and functional status w

The mechanism of cerebellar involvement in cognitive disorders and therapeutic prospects.

The cerebellum has traditionally been regarded as a brain region primarily involved in motor coordination and balance. However, increasing evidence suggests that the cerebellum plays a significant role in multiple higher-order cognitive functions. This review explores the mechanisms underlying cerebellar involvement in cognitive impairment, focusing on four prevalent cognitive disorders: Alzheimer's disease, vascular cognitive impairment, Parkinson's disease dementia, and frontotemporal dementia. Analysis of existing neuroimaging, neuropathological, and clinical studies indicates that cerebell

Herpes zoster vaccination and cognitive disorders in older adults.

INTRODUCTION: Previous studies suggested a reduced risk of dementia after live zoster vaccination, but were limited by short follow-up and narrow populations. This study assessed the long-term association between live zoster vaccination and memory disorders and Alzheimer's disease (AD) in a nationwide Korean cohort. METHODS: This study, using linked national databases, included 2.5 million adults aged ≥ 50 years from 2012 to 2024. After exposure-based overlap weighting, Cox models estimated adjusted hazard ratios (aHRs) and restricted mean survival time (RMST). RESULTS: Among 1,041,964 parti

[Features of care of patients with cardiovascular diseases and cognitive disorders].

Due to the increase in life expectancy of the population, the proportion of people with cognitive impairment (CI) is also increasing. Today, it is known that cardiovascular diseases (CVD) are a significant risk factor for the development of CI. At the same time, the main substrate of CVD, including cerebrovascular events, is atherosclerosis. Statins are one of the most studied classes of drugs in the prevention of CVD. Atorvastatin (Liprimar) is one of the most studied and most frequently prescribed statins that has proven its effectiveness and safety in patients with a wide range of cardiovas

Retinal thickness: A window into cognitive impairment in bipolar disorder.

INTRODUCTION: Cognitive impairment (CI) in bipolar disorder (BD) significantly impacts overall functioning and quality of life. A better understanding of the neurobiological mechanisms associated with CI is needed. Studies on neurodegenerative diseases, such as Alzheimer's and Parkinson's diseases, have revealed promising findings related to retinal thickness alterations using optical coherence tomography (OCT). Similarly, retinal differences between healthy controls and individuals with BD or schizophrenia have been described. This study explores the utility of OCT in discerning retinal chang
