Early-life cognitive intervention preserves brain function in aged TgF344-AD rats with sex-specific effects.

Alzheimer's disease is characterized by progressive cognitive decline, and its effects are mitigated by cognitive reserve. We investigated whether long-term cognitive stimulation, initiated before amyloid deposition, preserves brain function in male and female TgF344-AD rats. Transgenic and wild-type (WT) rats underwent cognitive training or remained untrained. Resting-state fMRI assessed functional connectivity, the novel object recognition test evaluated memory, and molecular analyses examined synaptic plasticity, inhibitory signaling, and microglial reactivity. At baseline, females showed g…

Early neuropsychological impairment in HIV-seropositive intravenous drug users: evidence from the Italian Multicentre Neuropsychological HIV Study.

The aim of the Italian Multicentre Neuropsychological HIV Study is to assess the prevalence and natural history of cognitive deficit in intravenous drug users (i.v.DUs) during the asymptomatic phase of HIV infection. The study is currently being conducted in four centres (Napoli, Benevento, Verona and Pavia) whose catchment areas are characterized by different levels of prevalence of HIV infection. Cognitive evaluation is being performed by means of a standardized neuropsychological test battery. A total of 251 subjects (167 males and 84 females) have been recruited in the cross-sectional phas

Managing money matters: Managing finances is associated with functional independence in MCI.

OBJECTIVE: Previous research suggests that overall experience participating in instrumental activities of daily living (IADLs) is associated with reduced IADL impairment in individuals with mild cognitive impairment, possibly because of an increased functional reserve. Given that difficulties managing finances tend to occur with mild cognitive impairment, this study explores whether experience managing one's finances is associated with independence across various IADLs. METHODS: Participants with a screen or baseline diagnosis of mild cognitive impairment (n = 862) were taken from the Alzheim

Predictors of cognitive resilience in the old-old: An observational study using real-world electronic health record data.

INTRODUCTION: Understanding factors associated with cognitive resilience is critical for developing interventions to preserve cognitive health in the old-old. Our aim is identifying clinical predictors of cognitive resilience in a large real-world cohort of adults aged 75+. METHODS: Longitudinal analysis of electronic health records from Johns Hopkins primary care clinics between January 1, 2014, and August 31, 2025. A minimum of two follow-up visits at least a year apart were required. To estimate the associations between predictors and cognitive resilience, we employed a Cox model with cogn

Correlation between clinical and cognitive aspects and nutritional indicators of elderly patients with new-onset epilepsy.

OBJECTIVE: Nutritional indicators were correlated with cognitive and clinical aspects of 25 elderly patients with new-onset epilepsy (EPWE). The nutritional indicators of the EPWE were compared with those of a similar control group at a significance level of p < 0.05. RESULTS: There was lower cognitive performance, greater risk of malnutrition and muscle tissue depletion, and higher waist circumference (WC) in the EPWE. Longer epilepsy duration was correlated with loss of muscle mass (Pearson's correlation: 0.433; p = 0.044). Performance in the verbal fluency test, in the clock-drawing test,

Transitions across cognitive states and death among older adults in relation to education: A multistate survival model using data from six longitudinal studies.

INTRODUCTION: This study examines the role of educational attainment, an indicator of cognitive reserve, on transitions in later life between cognitive states (normal Mini-Mental State Examination (MMSE), mild MMSE impairment, and severe MMSE impairment) and death. METHODS: Analysis of six international longitudinal studies was performed using a coordinated approach. Multistate survival models were used to estimate the transition patterns via different cognitive states. Life expectancies were estimated. RESULTS: Across most studies, a higher level of education was associated with a lower ris…

Improvement in cognitive function and cerebral perfusion after bur hole surgery in an adult with moyamoya disease. Case report.

Recent studies have suggested that cognitive impairment may be a common complication in adults with moyamoya disease (MMD). However, the mechanisms of cognitive dysfunction have not been clarified. Whether cognitive impairment may occur as a consequence of cerebral hypoperfusion and may improve after revascularization surgery has not been determined. A 39-year-old West Indian woman with subacute dysexecutive cognitive syndrome and no history of stroke was diagnosed with MMD. Magnetic resonance imaging showed an old, small cerebral infarction in the left frontal white matter and no evidence of

Alzheimer's disease and vascular dementia: one potentially preventable and modifiable disease? Part II: Management, prevention and future perspective.

The management of dementia and mild cognitive impairment (MCI) includes pharmacological, nonpharmacological and caregiver interventions. Acetyl-cholinesterase inhibitors and memantine have a small beneficial effect in mild-to-moderate dementia. Attention is increasingly focused on long-term measures that may prevent, delay or minimize MCI and dementia, including Mediterranean diet, exercise, early active treatment of hypercholesterolaemia hypertension, and diabetes starting in midlife and earlier. High cognitive activity and a high cognitive reserve may prevent or delay the onset of aging-rela…

Recent advances in geriatric anaesthesia.

The World Health Organization defines healthy ageing as 'the process of developing and maintaining the functional ability that enables well-being in older age'. This functional ability is the characteristic of the individual's own physical and mental status influenced by the environmental and socio-economic factors. Preoperative preparation of the elderly patients includes functional assessment to identify preexisting cognitive impairment or cardio-pulmonary reserve, frailty, nutrition, polypharmacy and anti-coagulation issues. Intraoperative management includes anaesthetic mode and pharmacolo

Segregation of functional networks is associated with cognitive resilience in Alzheimer's disease.

Cognitive resilience is an important modulating factor of cognitive decline in Alzheimer's disease, but the functional brain mechanisms that support cognitive resilience remain elusive. Given previous findings in normal ageing, we tested the hypothesis that higher segregation of the brain's connectome into distinct functional networks represents a functional mechanism underlying cognitive resilience in Alzheimer's disease. Using resting-state functional MRI, we assessed both resting-state functional MRI global system segregation, i.e. the balance of between-network to within-network connectivi

Efficacy of personalized repetitive transcranial magnetic stimulation based on functional reserve to enhance ambulatory function in patients with Parkinson's disease: study protocol for a randomized controlled trial.

BACKGROUND: Repetitive transcranial magnetic stimulation (rTMS) is one of the non-invasive brain stimulations that modulate cortical excitability through magnetic pulses. However, the effects of rTMS on Parkinson's disease (PD) have yielded mixed results, influenced by factors including various rTMS stimulation parameters as well as the clinical characteristics of patients with PD. There is no clear evidence regarding which patients should be applied with which parameters of rTMS. The study aims to investigate the efficacy and safety of personalized rTMS in patients with PD, focusing on indivi

Associations Between Social Network Characteristics and Brain Structure Among Older Adults.

INTRODUCTION: Social connectedness is associated with slower cognitive decline among older adults. Recent research suggests that distinct aspects of social networks may have differential effects on cognitive resilience, but few studies analyze brain structure. METHODS: This study includes 117 cognitively impaired and 59 unimpaired older adults. The effects of social network characteristics (bridging/bonding) on brain regions of interests were analyzed using linear regressions and voxel-wise multiple linear regressions of gray matter density. RESULTS: Increased social bridging was associated

Drivers of caregiver impact in Duchenne muscular dystrophy: a cohort study.

BACKGROUND: In our companion paper, we addressed the interplay between caregiver impact, out-of-pocket expenditures, and Duchenne Muscular Dystrophy (DMD) disability. We found that DMD caregiver impact could be characterized by four Latent Profile Analysis impact profiles: lowest, lower middle, upper middle, and highest impact. The impact on caregivers was often but not always worse with greater out-of-pocket expenditures. Further, while the lowest-, lower-middle, and highest-impact profiles reflected low, moderate and high disability-related caregiver burden, respectively, the upper-middle pr

Modeling brain reserve: experience-dependent neuronal plasticity in healthy and Huntington's disease transgenic mice.

OBJECTIVE: Experience-dependent modification of neuronal and synaptic connectivity may represent a mechanism of relevance to the theory of brain or cognitive reserve. The authors have investigated structural correlates of synaptic function and plasticity, through analysis of dendritic morphology after environmental enrichment, a paradigm for investigation of experience-dependent plasticity. DESIGN: Using a transgenic mouse model for Huntington's disease (HD), R6/1 and wild-type mice were exposed to either standard housing or environmental enrichment from 4 until 20 weeks of age. The impacts of early environmental adversity on cognitive functioning, body mass, and life-history behavioral profiles.

Early adverse experiences or exposures have a profound impact on neurophysiological, cognitive, and somatic development. Evidence across disciplines uncovers adversity-induced alternations in cortical structures, cognitive functions, and related behavioral manifestations, as well as an energetic trade-off between the brain and body. Based on the life history (LH) framework, the present research aims to explore the adversity-adapted cognitive-behavioral mechanism and investigate the relation between cognitive functioning and somatic energy reserve (i.e., body mass index; BMI). A structural equa
